Time course of vestibular function in patients with menière's disease following vestibular nerve section.

Harmonic acceleration rotational test and vestibular autorotation test (VAT) were performed on 8 patients with Menière's disease before and after vestibular nerve section. Sinusoidal rotations of 0.01-0.64 Hz were used in the HA rotational test. VAT was performed at frequencies between 0.5 and 6 Hz. Two weeks after operation, almost all the patients consistently showed response asymmetry toward the healthy ear at all stimulus frequencies and a reduction in sensitivity to rotational stimuli at 0.01 Hz in the HA rotational test. Prior to surgery, the time constant of the vestibulo-ocular reflex

Enlargement of the vestibular aqueduct.

Enlarged vestibular aqueduct, a recently identified anomaly, is typified by an enlarged vestibule, dilation of the ampullated portions of the horizontal and superior semicircular canals, an abnormal cochlea, and hearing loss. In the case described a 16-year-old boy had congenital hearing loss, episodic vertigo, and abnormal vestibular function testing. Tomograms and CT scans confirmed the diagnosis of bilaterally enlarged vestibular aqueducts. The vertiginous episodes decreased in frequency and severity with a no-salt-added diet. The authors conclude that the enlarged vestibular aqueduct is as

[Stabilometric diagnosis of vertigo in elderly patients with vestibular migraine.].

The purpose of the study was to study the possibility of computer stabilometric diagnostics for the objectification of vertigo in elderly patients with vestibular migraine. The study included 57 elderly patients with vestibular migraine and 57 elderly patients with central vestibular vertigo, without migraine. Specialized tests of functional computer stabilometry were used to objectify vertigo. The study revealed the features of the basic stabilometric parameters in specialized tests in elderly patients, which can be used to obtain a quantitative assessment of vestibular dysfunction and object

Preservation of tap vestibular evoked myogenic potentials despite resection of the inferior vestibular nerve.

Sound and skull-tap induced vestibular evoked myogenic potentials (VEMP) were studied in a 43-year-old man following inferior vestibular neurectomy. Surgery was performed because of a small acoustic neuroma. Postoperative caloric testing suggested sparing of superior vestibular nerve function on the operated side. In response to sound stimulation there were no VEMP on the operated side, irrespective of whether sounds were presented by air- or bone-conduction. This suggests sound-induced VEMP to be critically dependent on inferior vestibular nerve function and this is in agreement with present

Clinical, oculographic, and vestibular test characteristics of vestibular migraine.

BACKGROUND: We characterise the history, vestibular tests, ictal and interictal nystagmus in vestibular migraine. METHOD: We present our observations on 101 adult-patients presenting to an outpatient facility with recurrent spontaneous and/or positional vertigo whose final diagnosis was vestibular migraine (n = 27) or probable vestibular migraine (n = 74). Ictal and interictal video-oculography, caloric and video head impulse tests, vestibular-evoked myogenic potentials and audiometry were performed. RESULTS: Common presenting symptoms were headache (81.2%), spinning vertigo (72.3%), Mal de

Update on HINTS Plus, With Discussion of Pitfalls and Pearls.

BACKGROUND AND PURPOSE: The team that manages acute vertigo in patients requires multispecialty skills. This special interest article offers a simplified list of common pitfalls to be avoided when applying the HINTS (Head-Impulse, Nystagmus, Test-of-Skew) Plus examination protocol in patients with acute vertigo. Benign paroxysmal positional vertigo is the most frequent cause of positional vertigo. The lack of systematic evaluation of symptom duration and triggers, and/or incomplete target examination, leads to improper diagnosis and management. Pitfalls to avoid and pearls to apply are offered

[Comparative evaluation of vertigo in patients after stapedotomy and stapedectomy].

INTRODUCTION AND AIM: The reason of gradually developing conductive hearing loss in otosclerotic patients is the ossification of the stapes footplate to the surrounding bony structures and the therapy of stapes fixation is mainly surgical. In stapedotomy the footplate of the stapes is fenestrated with laser and microdrill in a diameter of 0.8 mm, whereas in stapedectomy there is complete removal of the footplate followed by the reconstruction of the ossicular chain. In the early postoperative period, temporary vertigo is frequently recorded which significantly influences the recovery. METHOD:

Galvanic testing in neuro-otological diagnostics.

From 1980 to 1986, a total of 436 patients were investigated by galvanic testing. Pathological findings resulted in 45 (out of 54) patients with a suspect tumour of the cerebellopontine angle (CPA), 7 (out of 7) patients with a Herpes zoster infection, 25 (out of 32) patients with an isolated, unilateral loss of vestibular function, 4 (out of 14) patients with head injuries. No pathological results of galvanic testing were found in patients with sudden deafness, Meniere's disease, vertebrobasilar or cerebrovascular insufficiency, etc.

[Possibilities for quantifying vestibular vertigo].

The problem of the quantification of vertigo is still unsolved. In this article, two methods in the field of visual orientation in which the vestibular system is involved, are proposed. The visual subjective vertical allows to determine the degree of deviations, which is augmented in acute vestibular lesions. - The degree of deviation correlates well with the subjective intensity of vertigo. Both methods seem to be useful in the quantification of subjective vestibular complaints.

[Nystagmus and benign paroxysmal vertigo (author's transl)].

Audio-impedence, vestibular, caloric, and electronystagmograph tests were conducted in 22 patients with benign paroxysmal vertigo. The disorder is provoked by certain sudden head movements, a latency period of several seconds occurring between the end of the movement and the onset of vertigo. This markedly differentiates it from other types of vertigo. It is of brief duration and associated with autonomic nervous system signs. Nystagmus, usually rotatory, is provoked by the particular head movement involved or by Hallpike's manoeuver. Results of the majority of tests were normal, occasional sl

Staging Meniere's disease and reporting results in Meniere's disease: symposium panel discussion.

At its recent spring meeting, the American Neurotologic Society devoted a portion of its program to a symposium on staging Meniere's disease and reporting results. One of the problems discussed, was which preoperative audiogram should be used for reporting results of any treatment. I think that it is very important that we, as otologists, try to look at Meniere's disease or any inner ear dysfunction in a more scientific way. Some of you may feel, on initial evaluation of this material, that it is too cumbersome to be practical in your daily clinical work. However, we feel we have put forth a r
