BACKGROUND: The scheme of the core vestibular membranes, consisting of serially connected utricle, ampulla and semicircular canal, first appeared hundreds of millions of years ago in primitive fish and has remained largely unchanged during the subsequent course of evolution. The labyrinths of higher organisms build on this core structure, with the addition of the phylogenetically newer membrane structures, namely, saccule, lagena and cochlea. INTRODUCTION: Motion sickness is a recurrent problem in pilot schools. Many techniques to overcome motion sickness are used worldwide, mostly including sedative medication and psychotherapy. Motion sickness does not correlate with future pilot skills: it should not be a criterion for selection of candidates. The problem must be controlled from the onset in an objective way. In the Swiss Air Force selection program, time compression does not allow any delay between the selection flights, nor can drugs be used.

MATERIAL AND METHODS: Human endolymphatic sacs were sampled during removal of vestibular schwannomas via a translabyrinthine approach. Motion sickness is a well known nausea and vomiting syndrome whose physical signs occur during travel by sea, automobile, airplane, and space. This review describes current concepts concerning the aetiology, nature and therapy of this common phenomenon. Motion sickness involve a neural mismatch or confusion between the vestibular, visual, and proprioceptive systems. Therapy is directed towards decreasing conflicting sensory input, controlling nausea, and speeding the process of adaptation.

Electrical stimulation in the monkey vestibulocerebellum has previously been shown to produce ocular nystagmus, but large stimulating current values were used. The three Spacelab D-1 Scientist Astronauts were exposed to a 1 1/2 h 3 g centrifuge run in the supine position, resulting in a linear 3 g acceleration in the x-direction. They used their space experience to evaluate their readapting to normal gravity and compared their observations with 'Space Adaptation Syndrome'. In order to assess objectively the equilibrium function in Ménière's disease, stabilographic measurements were performed in combination with a battery of provocative tests. In 55 patients, static and dynamic equilibrium was investigated on a differential basis and compared with the data calculated for healthy people of the same age. In Ménière's disease, patients showed a decline of the equilibrium function (P less than 0.001) between attacks, the decline enhancing with the duration of the disease. Changes in the blood plasma cyclic nucleotide (cAMP and cGMP) level under the effect of vestibuloprotectors: bemytil and etoxibemytil were studied in rats with experimental motion sickness. It is established that rotation causes increase in the cAMP level and decrease in the cGMP level. The effect of the vestibuloprotectors is determined by the dose of the drug and is aimed first of all at maintaining a stable cAMP level in vestibular exertion. Under conditions of this experiment etoxibemytil was more effective than bemytil.
