Telemark skiing injuries: characteristics and risk factors.

OBJECTIVE: To determine the types of injuries associated with telemark skiing and the effects of ability level, equipment, and terrain. METHODS: A survey was mailed to a sample of North American telemark skiers. RESULTS: Completed surveys were returned by 548 telemarkers (response rate = 74.5%). The mean age was 42.7 (+/- 9.3) years, and 69% were male. A total of 439 injury events resulted in 494 body injuries, reported by 285 skiers (52%). Lower-extremity injuries (n = 231) were more frequent than upper-extremity injuries (n = 187). Knee injuries were most common with 128 cases, followed by

Mojave rattlesnake (Crotalus scutulatus scutulatus) identification.

Mojave rattlesnake (Crotalus scutulatus scutulatus) identification has important diagnostic and therapeutic implications. Envenomation by certain populations of Mojave rattlesnakes may cause a different clinical presentation than that caused by other rattlesnakes. Specifically, Mojave rattlesnake envenomation may cause fewer local effects and more neurologic effects (including respiratory difficulty) than are typically seen after bites from other types of rattlesnake. Thus, it is useful for clinicians to distinguish the Mojave rattlesnake from other rattlesnakes in order to prevent underestima

Medical mountain rescue in the Mont-Blanc massif.

Because of its peculiar geographic location beneath Mont-Blanc, the Chamonix Hospital plays an important role in the management of mountaineering- and skiing-related traumatic injuries. The authors, thanks to the study of 5200 mountain rescue medical reports, set out to explain the different aspects of this activity: epidemiology, specific equipment, care in the field, reception and primary survey, dispatching, common therapeutic attitudes, progress, and results. In the dangerous environment of the high mountains, emphasis is put on the physicians' experience, allowing an early evacuation in t

Windsurfing injuries: results of a paper- and Internet-based survey.

OBJECTIVE: To describe the frequency, pattern, and mechanism of windsurfing injuries. METHODS: A 24-question multiple-choice paper-based survey was distributed to windsurfers at beaches in the United States and the Dominican Republic, and an identical survey was placed on the Internet. RESULTS: Two hundred ninety-four surveys in all were completed describing 339 acute and 150 chronic injuries. The foot, knee, chest wall, and ankle were the most commonly injured body parts. Sprains (26.3%) were most common, followed by lacerations (21.2%), contusions (16.2%), and fractures (14.2%). Direct inj

"Puss caterpillar" envenomation: a report from North Carolina.

A 41-year-old white man with no specific past medical history presented to the Emergency Department on October 13, 1997, following what he described as a "caterpillar bite." The insect was subsequently identified as a "puss caterpillar" (Megalopyge opercularis). The patient experienced immediate excruciating pain that radiated throughout his left arm and gradually spread to the left chest area. He presented to a nearby emergency department several hours later and received treatment. The patient was subsequently admitted for observation and released several days later. Viewed in the context of

Management of thai cobra bites with a single bolus of antivenin.

Between January 1981 and December 1991, 68 adults were treated at Chulalongkorn University for cobra bites. Respiratory failure developed in the bite victims even when an adequate genus-specific antivenin was administered at the onset of neuromuscular paralysis. The duration of time spent on a respirator was, however, significantly less in patients treated with an intravenous 100-ml antivenin bolus given at the onset of neurotoxic signs. We found that a bolus of 100 ml of purified equine antivenin produced by the Queen Saovabha Memorial Institute, Bangkok, from Naja kauthia venom was adequate

Deaths resulting from animal attacks in the United States.

The objective of this study was to elucidate the etiology and frequency of deaths in the United States from encounters with animals. The number of deaths from venomous and nonvenomous animals is reported annually to the US Department of Health and Human Services and published in Vital Statistics of the United States. This study is a survey of all animal-related fatalities listed as E-codes 905-906 reported in the Vital Statistics of the United States from 1979 through 1990. Data were extracted uniformly from the annual reports and analyzed using descriptive statistics. From 1979 through 1990,

Cutaneous myiasis from Dermatobia hominis.

We present a case report of cutaneous myiasis in a foreign traveler who was infected by Dermatobia hominis while visiting South America. This patient developed a painful furuncular lesion on the anterior scalp and noted that the lesion drained a serosanguinous fluid for more than a month before definitive treatment. Invasion of mammalian tissue by the larval forms of D. hominis typically results in the formation of a classic furuncular lesion. For persons who present with a lesion that contains a central draining stoma located on an exposed body surface, the diagnosis of myiasis should always

Primary amebic meningoencephalitis: a review of the clinical literature.

Primary amebic meningoencephalitis (PAM) is a rapidly progressive and potentially fatal infection frequently contracted by swimming in bodies of warm fresh water. The etiological agent in most reported cases of PAM is the ameboflagellate Naegleria fowleri. Infection with this organism closely mimics and is often mistaken for a bacterial or a viral pyogenic meningitis. Recovery is rare and depends on rapid diagnosis and treatment. Physicians treating individuals who present with an acute pyogenic meningitis should consider the diagnosis of PAM, particularly if the patient has a recent history o

Cobras.

Human encounters with cobras in Africa and Asia are common with substantial mortality and primarily respiratory paralysis in untreated cases. First aid includes maintaining calm, placing a lymphatic retardant on the bitten extremity, and transportation to a medical facility. Medical management is antivenin. Physiologic monitoring and the ability to manage the compromised airway are important.

Ciguatera poisoning after ingestion of imported jellyfish: diagnostic application of serum immunoassay.

Ciguatera fish poisoning is an important public health problem wherever humans consume tropical and subtropical fish. It accounts for over half of fish-related poisonings in the United States but is uncommonly diagnosed and underreported. Produced by dinoflagellates, ciguatoxin accumulates up the food chain in herbivorous and carnivorous fishes. Cnidaria jellyfish and related invertebrates) have not previously been associated with direct ciguatera intoxication in humans. We report the first case of ciguatera fish poisoning associated with cnidarian ingestion. A 12-year-old Tongan female presen

Acclimatization and tolerance to extreme altitude.

During the last ten years, two major experiments have elucidated the factors determining acclimatization and tolerance to extreme altitude (over 7000 m). These were the American Medical Research Expedition to Everest, and the low pressure chamber simulation, Operation Everest II. Extreme hyperventilation is one of the most important responses to extreme altitude. Its chief value is that it allows the climber to maintain an alveolar PO2 which keeps the arterial PO2 above dangerously low levels. Even so, there is evidence of residual impairment of central nervous system function after ascents to

Current and future applications of ultrasound imaging in peripheral nerve disorders.

Neuromuscular ultrasound (NMUS) is a rapidly evolving technique used in neuromuscular medicine to provide complimentary information to standard electrodiagnostic studies. NMUS provides a dynamic, real time assessment of anatomy which can alter both diagnostic and management pathways in peripheral nerve disorders. This review describes the current and future techniques used in NMUS and details the applications and developments in the diagnosis and monitoring of compressive, hereditary, immune-mediated and axonal peripheral nerve disorders, and motor neuron diseases. Technological advances have

Valorising and creating access to innovative medicines in the European union.

This Perspective describes (a) the current situation, (b) challenges and initiatives, (c) and formulates recommendations to valorize and create access to innovative medicines in the EU. We are currently still far away from optimal assessment of value for money in the EU. On the one hand, valorizing innovative medicines involves a local appraisal by health technology assessment (HTA) bodies and competent authorities about the value for money, the budget impact, and the local medical need that can be filled with new medicines. Therefore, local priorities and national health care policy environme

Therapy of newly diagnosed follicular lymphoma.

Newly diagnosed follicular lymphoma is relatively common and can be effectively treated with several differing approaches. Although the disease is often considered incurable, it is highly responsive to therapy when indicated. This review discusses the indications for treatment, risk stratification systems, treatment options with supporting clinical trial data, and expected therapeutic outcomes in newly diagnosed follicular lymphoma.

Ketogenic Diet in Cancer Prevention and Therapy: Molecular Targets and Therapeutic Opportunities.

Although cancer is still one of the most significant global challenges facing public health, the world still lacks complementary approaches that would significantly enhance the efficacy of standard anticancer therapies. One of the essential strategies during cancer treatment is following a healthy diet program. The ketogenic diet (KD) has recently emerged as a metabolic therapy in cancer treatment, targeting cancer cell metabolism rather than a conventional dietary approach. The ketogenic diet (KD), a high-fat and very-low-carbohydrate with adequate amounts of protein, has shown antitumor effe
