Silica exposure and systemic vasculitis.

Work in Department of Energy (DOE) facilities has exposed workers to multiple toxic agents leading to acute and chronic diseases. Many exposures were common to numerous work sites. Exposure to crystalline silica was primarily restricted to a few facilities. I present the case of a 63-year-old male who worked in DOE facilities for 30 years as a weapons testing technician. In addition to silica, other workplace exposures included beryllium, various solvents and heavy metals, depleted uranium, and ionizing radiation. In 1989 a painful macular skin lesion was biopsied and diagnosed as leukocytocla

Medico-legal aspects of disabilities due to orthopedic injuries and compensations in Egypt.

Disability is a complex problem that results in detrimental effects on the victim. The number of disabled has increased considerably. The development constitutes a good reason for paying more specific attention to society's growing number of disabled. The physician's duties are not only to describe the anatomic and physiological defects present in the injured individual, but also to answer questions regarding the patient ability to perform specific economic or social function. This study aimed to clear up some of the problems or difficulties that may face the medico-legal physician or any pers

Validity and reliability of the Fear-Avoidance Beliefs Questionnaire (FABQ) in workers with upper extremity injuries.

INTRODUCTION: Fear Avoidance Beliefs (FAB) have been associated with increased pain, dysfunction and difficulty returning to work in Upper Extremity (UE) injures. The FABQ is used to assess FAB, but its measurement properties have not been established in UE. The purpose of this study is to evaluate the reliability and validity of the FABQ to screen UE compensated injured workers for FAB. METHODS: Consenting workers attending a specialty clinic completed a modified FABQ, QuickDASH (Disability), SPADI Pain Score and von Korff Chronic Pain Grade (Pain), SF-36v2 (General Health), and Work Instabi

Human errors and occupational injuries of older female workers in residential healthcare facilities for the elderly.

This study aimed to describe the characteristics of occupational injuries of female workers in residential healthcare facilities for the elderly, and to analyze human errors as causes of accidents. From the national industrial accident compensation data, 506 female injuries were analyzed by age and occupation. The results showed that medical service worker was the most prevalent (54.1%), followed by social welfare worker (20.4%). Among injuries, 55.7% had <1 year of work experience and 37.9% were aged ≥60 years. Slips/falls were the most common type of accident (42.7%), and the proportion inju

Occupational injury and disease among patients presenting to general practitioners in a community health centre.

A prospective survey was conducted of all patients presenting over a six-month period to the primary medical care unit of a community health centre in an urban industrial area to determine the number and types of work-related injuries and disease, the causes, and details of the injured workers and their workplaces. Two-hundred and eighty-three patients, 7.2 per cent of the total number of patients attending, were diagnosed as having a work-related injury or disease; 250 patients had occupational injuries and 33 had occupational diseases. The most common injuries were open wounds, sprains and s

Comparison of open versus closed reduction of intraarticular calcaneal fractures: a matched cohort in workmen.

Seventeen displaced intraarticular calcaneal fractures treated operatively were matched with 17 nonoperatively treated displaced intraarticular calcaneal fractures among Workers' Compensation Board patients. Matching criteria included type of fracture, age, year of injury, and occupation. After follow-up averaging 6.3 years and 5.4 years, respectively, no significant differences existed between the two groups in heel pain, subtalar motion, and return to work. However, in those fractures treated operatively, the overall clinical result was better when an anatomic reduction of the subtalar joint…

The impact of occupational injury on injured worker and family: outcomes of upper extremity cumulative trauma disorders in Maryland workers.

BACKGROUND: Surveys have identified a dramatically rising incidence of work-related upper extremity cumulative trauma disorders (UECTDs). Outcome studies have addressed time lost from work and cost of compensation; omitting other significant consequences. We assess health, functional and family outcomes. METHODS: We identified 537 Workers' Compensation UECTD claimants. A computer-assisted telephone questionnaire was used to elicit symptom prevalence, functional impairment, depressive symptoms (CES-D scale), employment status. RESULTS: One to 4 years post-claim, respondents reported persisten…

Patterns of lung diseases in former mine workers of the former Republic of the Transkei: an X-ray-based study.

The Transkei is one of two regions from which a majority of South African mine workers used to be recruited, and the destination to which many return. The study was done to determine the prevalences of lung diseases in these ex-workers, links between these diseases and age, links between diseases and duration of mining, and the association between silicosis and tuberculosis. It was done in Umtata General Hospital. Participants' (n = 300) ages ranged from 35 to 66+ years (mean 51.6). Durations of mining ranged from < 1 to 31+ years. 29/300 X-rays (9.6%) were unreadable; 59 (21.8%) were disease-

Official incidence and trends of occupational musculoskeletal disorders in Turkey in 2014-2024: implications of possible underreporting.

In Turkey, official incidence figures for work-related musculoskeletal disorders (MSDs) remain notably low despite established ergonomic risks. The aim of this longitudinal study was to determine national incidences of all recognised occupational MSD cases among insured workers included in the publicly available registry of the Social Security Institution of Turkey (SGK) between 2014 and 2024, to calculate annual rates per million insured workers, and to establish temporal, gender-specific, and diagnostic trends. A total of 744 cases were recorded, with recognised incidence increasing from 1.7

The Effect of Changes in Employment on Health of Work-Related Injured Workers: A Longitudinal Perspectives.

This longitudinal study attempted to identify changes in employment status and overall health status. The participants were workers who had experienced work-related injuries in the past. In this study, we used the Panel Study of Workers' Compensation Insurance from 2013 to 2017. This study utilized propensity score matching for a quasi-experimental design study of the first year to exclude the effects of the confounding variables and exclude the effect of employment status, which is the main independent variable. After applying propensity score matching the research subjects totaled 1070. Chan…

Trends in Opioid-Related Poisonings and Mental and Behavioral Disorders From 2006 to 2022 Among a Large Cohort of Injured Workers in Ontario, Canada.

BACKGROUND: Work-related injuries may increase risk for opioid-related harms. Yet there remains a gap in our understanding of trends in opioid-related harms over time among injured workers. We estimated trends in hospital encounters for opioid-related harms among injured workers in Ontario, Canada from 2006 to 2022. METHODS: This study used linked workers' compensation (1983-2019) and hospitalization and emergency department (ED) (2006-2022) data from 1.7 million injured workers. Rates of opioid-related poisonings and mental and behavioral disorders were estimated for all workers, as well as …

Occupational burns in Washington State, 1989-1993.

Occupational burns cause significant morbidity in the United States each year; however, there are few studies that report industries or workplaces where workers are at an increased risk of burn injuries. Washington State's Department of Labor and Industries (L and I) computerized workers' compensation database was used to describe work-related burns over 5 years. From 1989 to 1993, L and I accepted 27,323 claims for occupational burns, 71.4% of them thermal burns and 26.8% chemical burns. The most common sources of injury were cooking oils (14%) and hot water/steam (13%). Workers involved in f…

The ideal of consumer choice in social services: challenges with implementation in an Ontario injured worker vocational retraining programme.

PURPOSE: Social service programmes that offer consumer choices are intended to guide service efficiency and customer satisfaction. However, little is known about how social service consumers actually make choices and how providers deliver such services. This article details the practical implementation of consumer choice in a Canadian workers' compensation vocational retraining programme. METHOD: Discourse analysis was conducted of in-depth interviews and focus groups with 71 injured workers and service providers, who discussed their direct experience of a vocational retraining system. Data a…

The 'ability' paradigm in vocational rehabilitation: challenges in an Ontario injured worker retraining program.

INTRODUCTION: In recent years, a focus on workers' ability, rather than impairment, has guided disability management services. However, a challenge with the notion of 'ability' is identification of the border between ability and inability. This article considers this gray zone of disability management in the case of a workers' compensation vocational retraining program for injured workers in Ontario. METHODS: In-depth interviews and focus groups were conducted with a purposive sample of 71 participants who were directly involved with the vocational retraining process. Workers in the program h…

Active pulmonary tuberculosis discovered at post-mortem examination of the lungs of black miners.

The prevalence of active pulmonary tuberculosis and pneumoconiosis in black mine workers who died while working on mines and came to autopsy is reported. All black mine workers are regularly examined radiologically during life and those with pulmonary tuberculosis or pneumoconiosis are compensated and sent home (after treatment in the case of tuberculosis) and would not enter this autopsy series. Details are given of the prevalence of tuberculosis separately for gold, coal and asbestos miners. Tuberculosis was age-related in all three cases. In black gold miners the presence of a slight degree

Aging, sex, and cost of medical treatment.

OBJECTIVE: Association between medical cost from workplace injuries and aging and its effect modification by sex were examined. METHODS: Medical costs reimbursed from workers' compensation between 2003 and 2009 were used. A multiple zero-truncated negative binomial regression predicted percent changes in medical cost. Cubic regression spline smoothers tested effect modification. RESULTS: Reimbursed medical costs comprised 3452 claims. Medical costs increased with aging; however, the trends differ by sex. Medical cost increase after 10 years of age increase was 27% among men (95% CI = 17% to …
