Periprocedural allergy evaluation is safe and effective to guide subsequent anesthesia administration.

BACKGROUND: There are limited data on allergy evaluation following suspected periprocedural hypersensitivity reactions, and skin testing for these reactions is not yet validated. OBJECTIVE: We sought to describe the epidemiology of periprocedural reactions and safety and effectiveness of allergy testing in a contemporary US patient cohort. METHODS: We performed a retrospective cohort study of adult patients evaluated at a tertiary-care academic center for presumed periprocedural hypersensitivity reaction from March 2013 to June 2023. We examined patient demographics, index reactions, tryptas

The Two-Dimensional Framework of Nerve Injury.

Classifying nerve injury severity is essential for communication, prognosis, and treatment decisions. Traditional classification systems (Seddon, Sunderland) cannot account for the full spectrum of nerve pathology, from familiar phenomena such as local anesthetics to recently discovered autoimmune nodopathies. A two-dimensional framework is proposed that separates two aspects of severity conflated in previous classifications and applies to focal and diffuse nerve injuries. The first dimension, quantitative degree, is the proportion of axons involved; the second, pathological type, is the under

Injectable thermosensitive hydrogel embedding lignin-modified PLGA microspheres enables sustained lidocaine release and prolonged neuromodulation.

Malignant ventricular arrhythmias (MVA) post myocardial infarction (MI) is a leading cause of sudden death in affected patients. Hyperactivation of the sympathetic nervous system plays a crucial role in the onset and persistence of MVA after MI. While percutaneous stellate ganglion block with local anesthetics is an effective minimally invasive approach for transient MVA attenuation, achieving prolonged sympathetic blockade remains challenging. This study developed a novel injectable thermosensitive hydrogel-lignin-microsphere system for the sustained release of lidocaine (Gel-LMS Lido), aimin

LIMIT Trial: Lidocaine with Intramuscular Injection of Benzathine Penicillin G for Treponema pallidum Treatment.

BACKGROUND: Studies have shown that intramuscular benzathine penicillin G (BPG) or bicillin injections cause severe pain in many patients, which may reduce adherence. Local anesthetics mixed with BPG may reduce injection pain without affecting drug effectiveness. The objective of this study was to examine if there is any benefit of adding 0.5 ml 1% lidocaine to prepackaged Benzathine Penicillin G, with respect to pain, compared to standard of care, in the treatment of Treponema pallidum infections, both 10 minutes and 24 hours after injection. METHODS: This study is a double blinded randomize

Real-world Effectiveness of Dupilumab in Eosinophilic Esophagitis: A Systematic Review and Meta-analysis.

BACKGROUND: Dupilumab is the only FDA-approved medication for eosinophilic esophagitis (EoE). Clinical trials have shown its effectiveness in alleviating symptoms and decreasing inflammation associated with EoE. However, real-world data on its efficacy is still limited. METHODS: We searched multiple databases for articles reporting the outcomes of dupilumab for the treatment of EoE and conducted a meta-analysis. RESULTS: Five retrospective studies including 209 subjects (mean age: 22.12±12.01 y and 61.24% males) were analyzed. The pooled outcome for symptom improvement with dupilumab was 89.

Thoracic Segmental Spinal Anesthesia for Laparoscopic Surgeries in Pediatric Patients: A Retrospective Case Series.

Thoracic segmental spinal anesthesia (TSSA) is a developing regional anesthesia approach that involves administering local anesthetics into the thoracic spine to produce a targeted nerve block. This segmental block provides stable hemodynamics and early recovery. Lumbar spinal anesthesia is a common practice in children, and TSSA can be a promising technique due to its advantages. Despite the theoretical advantages of TSSA in pediatric patients, there is a paucity of literature describing its clinical use, safety profile, and outcomes in this population. This case series presents our initial c

Comparative Evaluation of Dexmedetomidine and Dexamethasone as Adjuvants in Supraclavicular Brachial Plexus Block.

Introduction Currently, peripheral nerve block has shown immense potential with effective patient satisfaction. In the event of upper limb surgeries, the supraclavicular brachial plexus approach under ultrasound guidance renders quick and dense anesthesia. In addition, the clinical utility of adjuvants with local anesthetics elicits a good quality of nerve block with improved duration and inset. So the aim of the present study was to compare the block characteristics of dexmedetomidine and dexamethasone during supraclavicular brachial plexus block in patients undergoing upper limb surgeries. A randomized controlled trial evaluating the effects of transversus abdominis plane block with compound lidocaine hydrochloride injection on postoperative pain and opioid consumption and gastrointestinal motility in patients undergoing gynecological laparotomy.

INTRODUCTION: Incorporation of transversus abdominis plane (TAP) block into multimodal analgesia has been emphasized in Enhanced Recovery protocols (ERPs). However, benefit is limited in clinical practice. A potential explanation is the short duration of analgesia of standard local anesthetics. Herein, this randomized, double-blind, controlled trial evaluated whether TAPB with long-acting compound lidocaine hydrochloride injection reduces postoperative pain. METHODS: 164 patients undergoing elective gynecological laparotomy under sevoflurane anesthesia randomly received ultrasound-guided TAP

[Clinical study PEPCA : The effect of standardized preoperative patient education for patient-controlled regional analgesia on postoperative pain].

BACKGROUND: Patient-controlled analgesia (PCA) is a well-established form of postoperative pain management. One form of administration is patient-controlled regional analgesia (PCRA), where local anesthetics are administered via peripheral regional catheters; however, a prerequisite is that the patients are instructed on its use. A multitude of sources recommend that these instructions are given before surgery as preoperative training on pain management procedures has been shown to significantly reduce patients' postoperative pain and increase their well-being. OBJECTIVE: The aim was to asses

Evaluation of Pharmaceuticals for DNA Damage in the Chicken Egg Genotoxicity Assay (CEGA).

DNA damage is an established initiating event in the mutagenicity and carcinogenicity of genotoxic chemicals. Accordingly, assessment of this endpoint is critical for chemicals which are being developed for use in humans. To assess the ability of the Chicken Egg Genotoxicity Assay (CEGA) to detect genotoxic pharmaceuticals, a set of 23 compounds with different pharmacological and reported genotoxic effects was tested for the potential to produce nuclear DNA adducts and strand breaks in the embryo-fetal livers using the 32P-nucleotide postlabeling (NPL) and comet assays, respectively. Due to hi

Electrocatheter-mediated High-voltage Pulsed Radiofrequency of the Dorsal Root Ganglion in the Treatment of Chronic Lumbosacral Neuropathic Pain: A Randomized Controlled Study.

ObjectivesDespite the interest in scientific community, there is still poor evidence about pulsed radiofrequency (PRF) efficacy in the treatment of neuropathic pain. In order to determine whether high-voltage PRF and epidural adhesiolysis (PRF-EA) showed better results than epidural adhesiolysis alone (EA), a randomized, double-blind, comparative-effectiveness study was conducted in patients with chronic lumbosacral radiating pain and neuropathic features.Materials and methodsA total of 41 patients were randomly allocated to 2 groups. Twenty-one patients were randomized to receive 2 cycles of

Effectiveness, Safety, and Predictive Potential in Ultrasound-Guided Stellate Ganglion Blockades for the Treatment of Sympathetically Maintained Pain.

BACKGROUND: Evaluating the effectiveness of stellate ganglion blockades (SGBs) proves challenging, since the criteria defining a successful blockade are controversial. This may be one reason for the scarcity of studies on this topic, thus forcing clinical guidelines to remain conservative in recommending SGBs. Moreover, factors to predict which patients will benefit from blockade series are not yet available. OBJECTIVES: The objectives of this study were to evaluate through a clinical approach SGBs' effectiveness performed under ultrasound guidance (us-SGB) and to identify factors to predict
