[Treatment of acute left cardiac insufficiency: a labyrinth without the thread of Ariane?].

The final picture of left ventricular failure depends at least as much upon its four mechanisms of compensation (increase in preload, in afterload, in heart rate and, as an attempt, in myocardial contractility) as upon its primary factor (decrease in contractility). The complexity of all possible combinations between those various factors has good chances to bring about confusion in therapeutics if one doesn't dispose of a method, a kind of Ariane's thread, making it possible (1) to define some wide orientations to start with, and thereafter (2) to state precisely the way out of that labyrinth

Captopril: a preliminary review of its pharmacological properties and therapeutic efficacy.

Captopril is the first orally active inhibitor of angiotensin-converting enzyme to become available. It has been studied primarily in hypertension. In mild to moderate essential hypertension captopril is about as effective as usual doses of hydrochlorothiazide or propranolol, about one-half of such patients needing the addition of a diuretic to achieve satisfactory control of blood pressure. In severe hypertension captopril plus a diuretic (and in some patients a beta-blocker) usually reduced blood pressure significantly more than could be achieved with 'standard triple therapy' in patients no

Angiotensin and CNS regulation of blood pressure.

It appears that angiotensin II (AII) has access to the CNS by three routes: endogenous formation within the brain, passage into CSF, or transfer across "gaps" in the blood-brain barrier. The mode of access influences both the characteristics and the magnitude of the cardiovascular effects exerted by AII within the CNS. In the dog the effects of blood-borne AII on the area postrema (AP) in the IV ventricle seem to have the most physiological significance. In recent experiments we have characterized both the histological composition of this structure and its physiological effects. While electric…

Humoral and neurogenic factors in two-kidney renovascular hypertension.

A "bolus" dose (110 microgram) of the angiotesin II (A II)-blocker 1-Sar-8-Ala-A II (Saralasin, S) followed by its slow rate infusion (5 microgram/min/rat) for thirty min, was injected before and after the complete ganglionic blockade by pentolinium (P) in unanaesthetized unilaterally clipped renal hypertensive rats (the opposite kidney remained untouched). Pentolinium was also injected like a "bolus" dose (3 mg) followed by slow infusion (0.1 mg/min/rat) for thirty min. The observations were made until the fifth week after clipping the left renal artery. A consistent maximal hypotensive respo

Hepatorenal syndrome: a disease mediated by the intrarenal action of renin.

The functional renal failure accompanying advanced liver disease is characterized by azotemia, a urine of very low sodium concentration and … in substrate is available for human infusion, this hypothesis could be tested either by infusion of angiotensin II to restore systemic blood pressure and renal perfusion or by beta adrenergic blockade with propranolol to attempt to decrease the intrarenal effects of renin, restore glomerular blood flow and filtration and thus return of renal function.

The relative importance of central nervous catecholaminergic and cholinergic mechanisms in drinking in response to antiotensin and other thirst stimuli.

1. Intracranial or subcutaneous doses of atropine or atropine methyl nitrate that were fully effective at preventing drinking in response to intracranial carbachol did not block angiotensin-induced drinking. 2. The nicotinic antagonist dihydro-beta-erythroidine given intracranially affected neither angiotensin- nor carbachol-induced drinking. 3. The dopaminergic antagonists haloperidol and spiroperidol injected intracranially blocked angiotensin-induced drinking but did not affect carbachol-induced drinking. 4. Angiotensin- and carbachol-induced drinking were unaffected by alpha- or beta-adren

Site of stimulation of aldosterone biosynthesis by angiotensin and potassium.

Studies were undertaken to determine what part of the aldosterone biosynthetic pathway is stimulated by angiotensin and potassium. The availability of a method for isolating the early portion of the aldosterone pathway and a new method for measuring plasma deoxycorticosterone permitted the design of experiments to determine whether angiotensin and potassium stimulate the pathway before deoxycorticosterone. To eliminate ACTH-dependent steroid synthesis, the experiments were performed in subjects receiving constant dosage of dexamethasone. To minimize the intra-adrenal conversion of deoxycortico

Matrix metalloproteinase imaging and survival prospects with aging in murine aortic aneurysm.

BACKGROUND: Matrix metalloproteinases (MMPs) are elevated in aortic aneurysm (AA), where older age is a risk factor. The relationship among aging, aortic MMP activity, and AA development has not been systematically evaluated. This study leveraged MMP-targeted molecular imaging to investigate how aging affects aortic MMP activity, aneurysm development, and survival. RESULTS: AA development and animal survival were monitored for 28 days after Angiotensin (Ang)-II infusion in 8-10-week-old (young) and > 51-week-old (old) apolipoprotein E (Apoe)-/- mice. Old animals' survival at 28 days was signi

N-Lac-Phe Ameliorates Myocardial Cell Death by Improving Mitochondrial Dysfunction, Ultimately Improving TAC-induced Heart Failure.

PURPOSE: Mitochondrial dysfunction and cardiomyocyte death drive pressure overload-induced heart failure (HF). N-lactylphenylalanine (N-Lac-Phe) plays an important role in the myocardial metabolic network after infarction. However, its role in chronic pressure overload heart failure remains unclear. This study examines N-Lac-Phe’s effects on cardiac function in transverse aortic constriction (TAC)-induced heart failure and mitochondrial function in Angiotensin II (Ang II)-injured H9c2 cardiomyoblasts. METHODS: Human plasma Lac‑Phe levels were measured by liquid chromatography-mass spectrometr…

Hypertension induces glomerulosclerosis in phospholipase C-ε1 deficiency.

Loss-of-function mutations in phospholipase C-ε1 (PLCE1) have been detected in patients with nephrotic syndrome, but other family members with the same mutation were asymptomatic, suggesting additional stressor are required to cause the full phenotype. Consistent with these observations, we determined that global Plce1-deficient mice have histologically normal glomeruli and no albuminuria at baseline. Angiotensin II (ANG II) is known to induce glomerular damage in genetically susceptible individuals. Therefore, we tested whether ANG II enhances glomerular damage in Plce1-deficient mice. ANG II…

Vinpocetine Attenuates Pathological Cardiac Remodeling by Inhibiting Cardiac Hypertrophy and Fibrosis.

PURPOSE: Pathological cardiac remodeling, characterized by cardiac hypertrophy and fibrosis, is a pathological feature of many cardiac disorders that leads to heart failure and cardiac arrest. Vinpocetine, a derivative of the alkaloid vincamine, has been used for enhancing cerebral blood flow to treat cognitive impairment. However, its role in pathological cardiac remodeling remains unknown. The aim of this study is to examine the effect of vinpocetine on pathological cardiac remodeling induced by chronic stimulation with angiotensin II (Ang II). METHODS: Mice received Ang II infusion via osm…

Antihypertensive medication use and incident breast cancer in women.

The purpose of this study was to evaluate whether antihypertensive medication use, including long-term use, is associated with increased breast cancer incidence in women. We studied 210,641 U.S. registered nurses participating in the Nurses' Health Study (NHS) and Nurses' Health Study II (NHS II). Information on antihypertensive medication use was collected on biennial questionnaires in both cohorts, and breast cancer cases were ascertained during this period. Multivariable-adjusted Cox proportional hazard models were used to estimate relative risks of invasive breast cancer over follow-up (19

Placenta-Derived Secretions Promote Liver Dysfunction, and Hepatic Serum Amyloid A Mediates Kidney Inflammatory Response in a Preeclampsia-like Mouse Model.

Preeclampsia (PE) is characterized by maternal hypertension accompanied with multi-organ dysfunction, such as maternal hepatic and renal dysfunction. Abnormal placental conditions may play a key role in regulating maternal organ function by promoting systemic inflammation. This study aimed to test the hypothesis that placenta-derived secretions contribute to hepatic and renal injury through interorgan communication using a PE-like mouse model. Pregnant mice were infused with angiotensin II (Ang II) from gestational day (GD) 12 (GD1 defined as the day of plug detection). Ang II infusion induced…

Dipeptidyl peptidase-4 inhibitor decreases abdominal aortic aneurysm formation through GLP-1-dependent monocytic activity in mice.

Abdominal aortic aneurysm (AAA) is a life-threatening situation affecting almost 10% of elders. There has been no effective medication for AAA other than surgical intervention. Dipeptidyl peptidase-4 (DPP-4) inhibitors have been shown to have a protective effect on cardiovascular disease. Whether DPP-4 inhibitors may be beneficial in the treatment of AAA is unclear. We investigated the effects of DPP-4 inhibitor sitagliptin on the angiotensin II (Ang II)-infused AAA formation in apoE-deficient (apoE-/-) mice. Mice with induced AAA were treated with placebo or 2.5, 5 or 10 mg/kg/day sitagliptin…
