[Langerhans cell histiocytosis and sclerosing cholangitis in adults].

INTRODUCTION: Langerhans cell histiocytosis and sclerosing cholangitis are two rare diseases that are frequently linked in children, but very rarely so in adults. CASE REPORT: A 40 year old woman with a 17 year history of Langerhans cell histiocytosis with chronic respiratory failure and diabetes insipidus presented with cholestatic jaundice whilst being assessed for lung transplantation. Pathological examination demonstrated sclerosing cholangitis. No Langerhans histiocytosis lesions were found in the liver or the biliary tract. Plans for pulmonary and hepatic transplantation were abandoned …

Evaluation of excretory/secretory Fasciola (Fhes) antigen in diagnosis of human fascioliasis.

No doubt, human fascioliasis is an increasing worldwide zoonotic liver fluke. Clinically, human fascioliasis has to be differentially diagnosed from many hepatic diseases as acute & chronic hepatitis, schistosomiasis mansoni, visceral toxocariasis, visceral leishmaniasis, hepatic amoebiasis, biliary tract diseases and others. The parasitological diagnosis based on the demonstration of the eggs in stool, duodenal contents or bile is usually unsatisfactory due to false passage of eggs, ectopic fascioliasis, and failure of immature worm to maturation. So, ELISA-Fhes antigen (Fasciola hepatica exc…

Sonographic diagnosis of cholangitis in AIDS patients.

Eleven acquired immunodeficiency syndrome (AIDS) patients were initially evaluated by ultrasound of the liver because of biochemical evidence of cholestasis. Sonography was very useful for demonstrating dilatation and/or wall thickening of the common bile duct associated with dilated intrahepatic bile ducts. Periportal hypo- or hyperchoic areas were also visualized. In seven patients, gallbladder walls were thickened. In six patients, the diagnosis of cholangitis was confirmed by endoscopic retrograde cholangiography and/or histology. Cytomegalovirus infection or digestive tract cryptosporidio

[Non-cirrhotic portal hypertension complicated by portal biliopathy].

The earliest references to association between mechanical jaundice and non-cirrhotic portal hypertension (NCPH) are by Fraser et al. (1944) and Gibson et al. (1965). The term "portal hypertension biliopathy" was proposed by Dhiman et al. (1999). The incidence of asymptomatic form of portal hypertension biliopathy (PHB) in patients with NCPH ranges from 81% to 100% that is significantly higher compared to patients with hepatic block (0-33%). Symptomatic form of PHB occurs only in 5-33% of patients. Any intervention on biliary tract in patients with PHB may result in significant blood loss due t…

Gallstone ileus with spontaneous passage of the obstructing gallstone and resolution of small bowel obstruction: a case report.

Gallstone ileus is a rare cause of mechanical small bowel obstruction that typically occurs in elderly patients with longstanding gallstone disease. It results from migration of a gallstone into the gastrointestinal tract through a biliary-enteric fistula, where it may become impacted within the bowel lumen. We present the case of an elderly female with insulin-dependent diabetes mellitus and heart failure who presented with abdominal pain, nausea and vomiting. Imaging established the diagnosis of gallstone ileus, and prior imaging demonstrated underlying cholelithiasis. Follow-up imaging seve

Pulmonary nocardiosis following pembrolizumab use: case report and literature review of nocardiosis associated with immune checkpoint inhibitors.

A woman in her 80s, who had a history of treatment with pembrolizumab for biliary tract cancer and was taking low-dose corticosteroids for immune-related adverse events (irAEs) involving nephritis and hepatitis, was admitted to the hospital complaining of a cough and increased sputum production that had begun two weeks prior. Computed tomography (CT) revealed patchy infiltrates predominantly in the lower lobes of both lungs. Since one of the Nocardia species was detected in the sputum collected at admission, we diagnosed her with pulmonary nocardiosis. In addition to this case, other cases of …

Unexplained portal hypertension and confusion in an elderly patient: a late presentation of congenital hepatic fibrosis.

A 71-year old male patient with myelodysplastic syndrome presented with severe signs of portal hypertension. There was no underlying cirrhosis nor portal vein thrombosis. Despite liver stiffness being high, hepatic vein catheterisation failed to show an increased hepatic venous pressure gradient compatible with clinically significant portal hypertension. Finally, a liver biopsy showed enlarged fibrotic portal tracts with multiple dilated immature bile ducts compatible with congenital hepatic fibrosis - there was no macroscopic biliary disease. Mostly presenting in childhood or adulthood, conge

Evaluation of Mayo endoscopic subscore in ulcerative colitis with concomitant primary sclerosing cholangitis.

This article relates to the discussion of a recent study published by Wohl et al. Primary sclerosing cholangitis (PSC) is a chronic inflammatory liver disease that affects the intra- and extrahepatic bile ducts and is strongly associated with ulcerative colitis (UC). Endoscopic evaluation of UC is feasible and reliable in routine clinical practice, and the Mayo endoscopic subscore (MES) is one of the most commonly used endoscopic evaluation measures for UC. Patients with PSC-UC are at higher risk of developing cancer and biliary tract cancer. Endoscopic scoring alone appears unreliable, and hi…

MicroRNAs in opisthorchiids and their definitive hosts: Current Status and Perspectives.

Opisthorchis felineus, Opisthorchis viverrini, and Clonorchis sinensis (family Opisthorchiidae) are parasitic flatworms that pose serious threats to humans in certain countries and cause opisthorchiasis/clonorchiasis. Opisthorchiid flukes parasitize the biliary tract of the host, causing cholangitis, cholecystitis, cholelithiasis and cholangiocarcinoma. In this review, we primarily focus on recent microRNAs (miRNAs) studies of opisthorchiid flukes and their definitive hosts. Many miRNAs are conserved and expressed in a developmentally stage specific manner in the three opisthorchiid flukes, wh…

Bacteremia from streptococcus constellatus revealing a gastrointestinal stromal tumor.

BACKGROUND: Pyogenic Liver Abscesses (PLA) are the most common type of visceral abscess. They generally develop in a context of biliary disease or hematogenous seeding, but a complete diagnostic work-up is always required in order not to miss other important causes, including above all malignancies of the gastro-intestinal tract. CASE PRESENTATION: Herein, we report a particular case of a 80 years-old immunocompetent woman hospitalized for sepsis. At the end of the diagnostic process, Streptococcus constellatus (Sc) was identified as the cause of sepsis, multiple PLA were found together with

Triple Covered Metal Stent Deployment Using Side-by-Side Technique for Hemobilia due to Hepatocellular Carcinoma (with Video).

A hepatocellular carcinoma (HCC) rarely expands into the biliary tract. In this situation, because of its hypervascular nature, cholangitis or hemobilia may sometimes occur. Surgery is one of the options in this situation. However, patients with HCC and bile duct invasion are sometimes in a poor general condition, as in the case presented in this report. For such patients, surgical treatment may need to be invasive. Thus, here we report technical tips for triple covered metal stent deployment using side-by-side technique for hemobilia due to HCC. After guidewire deployments at the left, anteri…

Dose to organ at risk and dose prescription in liver SBRT.

Stereotactic body radiation therapy (SBRT) is delivered in a curative intent to many primary and secondary tumors. Concerning liver metastas … ions of liver enzymes are often observed. Other toxicities of SBRT on the duodenum, small bowel and biliary tract are also described. With respect to contraindications and dose limitations on surrounding structures, SBRT is well tolerated and takes place among curative treatment of liver tumors, as surgery, radiofrequency and embolization.

Radiation therapy in cholangiocellular carcinomas.

Cholangiocarcinoma can arise in all parts of the biliary tract and this has implications for therapy. Surgery is the mainstay of therapy however local relapse is a major problem. Therefore, adjuvant treatment with chemoradiotherapy was tested in trials. The SWOG-S0809 trial regimen of chemoradiotherapy which was tested in extrahepatic cholangiocarcinoma and in gallbladder cancer can currently be regarded as highest level of evidence for this indication. In contrast to adjuvant therapy where only conventionally fractionated radiotherapy plays a role, stereotactic body radiotherapy (SBRT) today …

Multimodality imaging in diagnosis and management of alveolar echinococcosis: an update.

Alveolar echinococcosis is a parasitic disease limited to the northern hemisphere. The disease occurs primarily in the liver and shows a profile mimicking slow-growing malignant tumors. Echinococcus multilocularis infection is fatal if left untreated. It can cause several complications by infiltrating the vascular structures, biliary tracts, and the hilum of the liver. As it can invade the adjacent organs or can spread to distant organs, alveolar echinococcosis can easily be confused with malignancies. We provide a brief review of epidemiologic and pathophysiologic profile of alveolar echinoco…

Heterotopic Gastric Mucosa in the Distal Part of Esophagus in a Teenager: Case Report.

Heterotopic gastric mucosa (HGM) of the esophagus is a congenital anomaly consisting of ectopic gastric mucosa. It may be connected with disorders of the upper gastrointestinal tract, exacerbated by Helicobacter pylori. The diagnosis of HGM is confirmed via endoscopy with biopsy. Histopathology provides the definitive diagnosis by demonstrating gastric mucosa adjacent to normal esophageal mucosa. HGM located in the distal esophagus needs differentiation from Barrett's esophagus. Barrett's esophagus is a well-known premalignant injury for adenocarcinoma of the esophagus. Symptomatic cholecystolithiasis after cholecystectomy.

A 43-year-old woman was admitted to the gastroenterology department with colicky pain in the upper abdomen. Four years earlier, she had undergone a laparoscopic cholecystectomy because of cholecystitis. She recognised her current complaints from that previous episode. An endoscopic retrograde cholangiopancreatography showed a cavity with a diameter of 2 cm which contained multiple concrements near the liver hilus. An elective surgical exploration was performed. Near the clip of the previous cholecystectomy a bulging of the biliary tract with its own duct was visualised and resected. Histologic…

Composite small cell and mucinous carcinoma originating from the intrahepatic bile duct: report of a case.

The biliary tract is a very rare site for the occurrence of extrapulmonary small cell carcinoma. A 68-year-old Japanese female was being followed up for autoimmune hepatitis, and was referred to our hospital because segmental intrahepatic bile duct dilation was found on routine imaging studies, suggesting intrahepatic cholangiocarcinoma. She underwent left lobectomy of the liver and concomitant resection of the caudate lobe. Microscopic examination of the explanted liver showed a primary composite tumor comprising small cell and mucinous carcinomas that originated in the intrahepatic bile duct…

Cutting-edge issues in autoimmunity and allergy of the digestive system.

Autoimmunity and allergy involving the digestive system may be considered as paradigmatic for numerous common themes of complex diseases secondary to tolerance breakdown. Among gastrointestinal autoimmune diseases, for example, we encounter diseases in which a clear environmental trigger is identified (i.e., celiac disease), serum autoantibodies are most specific (i.e., primary biliary cirrhosis), or in which the disease pathophysiology is clearly understood (i.e., autoimmune gastritis). Similarly, it is intriguing that the gastrointestinal tract and the liver circulation represent the crucial
