Treatment of Chronic Insomnia in Adults.

Insomnia affects 30% of the U.S. population, with 5% to 15% meeting criteria for chronic insomnia. It can negatively impact quality of life, decrease productivity, increase fatigue and drowsiness, and put patients at higher risk of developing other health problems. Initial treatment focuses on nonpharmacologic therapies such as cognitive behavior therapy, which improves negative thought patterns and behaviors through sleep restriction, stimulus control, and relaxation techniques. Other nonpharmacologic treatments include exercise, mindfulness, and acupuncture. If these approaches are ineffecti

The staging model in schizophrenia, and its clinical implications.

INTRODUCTION: Illnesses which develop in a complex way are best described in stages, and those stages will describe not only a particular point in the course of the disease but also the appropriate treatment for that stage. This approach has, over the years, proved to be very appropriate for planning the treatment of various cancers. It is suggested that in the same way, it can be very important in planning the treatment of a complex illness such as schizophrenia. We aim to describe the staging model of schizophrenia, show the neuroimaging and clinical evidence for it, and discuss its implicat

Methods of training lay individuals in the use of evidence-based services for the management of mental and behavioral health disorders: a scoping review.

OBJECTIVE: This objective of this scoping review was to describe how lay individuals have been trained to deliver evidence-based services to manage mental and behavioral health disorders. INTRODUCTION: Health service delivery by lay individuals is commonly used for managing mental and behavioral health disorders in low-resource settings. Prior systematic reviews have examined the characteristics of lay health workers, the types of services they provide, and the efficacy of their services; however, a significant gap exists in documenting the training these individuals receive. ELIGIBILITY CRI

Fluoxetine Administration in Juvenile Monkeys: Implications for Pharmacotherapy in Children.

Fluoxetine therapy has been approved for children with major depressive disorder and obsessive compulsive disorder for over 14 years and has expanded to other childhood behavior disorders. As use increases, more detail on fluoxetine effects during juvenile brain development can help maintain safe and effective use of this therapy. Here, a narrative review is provided of previously published findings from a large nonhuman primate project. Fluoxetine was administered to juvenile male rhesus monkeys for an extended period (2 years) prior to puberty. Compared to controls, treated monkeys showed sl

Unveiling the neuroprotective potential of Ipomoea carnea ethanol extract via the modulation of tau and β-secretase pathways in AlCl3-induced memory impairment in rats in relation to its phytochemical profiling.

Alzheimer's disease (AD) is a widespread condition that affects adults and the community considerably. The causes are yet unknown, except from advanced age and genetic predisposition. Natural products provided advantageous advantages for managing AD due to their efficacy, safety, and accessibility. The memory boosting effects of chemically characterized Ipomoea carnea ethanol extract (IPC-EtOH) on behavioral, biochemical, histological, and molecular levels against cognitive impairment induced by AlCl3 exposure in rats were assessed using donepezil as a reference drug. Behavioral tests (spontan

NADPH Oxidase: a Possible Therapeutic Target for Cognitive Impairment in Experimental Cerebral Malaria.

Long-term cognitive impairment associated with seizure-induced hippocampal damage is the key feature of cerebral malaria (CM) pathogenesis. One-fourth of child survivors of CM suffer from long-lasting neurological deficits and behavioral anomalies. However, mechanisms on hippocampal dysfunction are unclear. In this study, we elucidated whether gp91phox isoform of nicotinamide adenine dinucleotide phosphate oxidase 2 (NOX2) (a potent marker of oxidative stress) mediates hippocampal neuronal abnormalities and cognitive dysfunction in experimental CM (ECM). Mice symptomatic to CM were rescue trea

A pilot randomized controlled trial of an integrative psychotherapy incorporating an attachment-based approach for adolescents with cannabis use disorder (TIMCA).

BACKGROUND: Cannabis is the most used illicit substance among adolescents. Regular cannabis use is linked to cognitive and psychosocial difficulties. As no pharmacological treatment has proven effective, psychological interventions, including motivational interviewing (MI), cognitive-behavioral therapy (CBT), and family-based approaches, remain central. However, adolescent-specific evidence remains limited. TIMCA was developed as a ten-session integrative psychotherapy combining MI, CBT, and integrated attachment therapy. While family therapy may not be indicated for all adolescents, parental

OBJECTIVE: Although eating disorders are common psychiatric disorders that usually onset during adolescence, few evidence-based treatments f … family approaches are generally reduced. For adolescent BN, one study found no differences between Cognitive Behavioral Therapy and FBT at the end of treatment or follow-up, while the other found FBT superior to individual therapy. CONCLUSIONS: Although the evidence remains limited, FBT appears to be the first line treatment for adolescent AN. There is little evidence to sup …

BusWorld: an analog pilot test of a virtual environment designed to treat posttraumatic stress disorder originating from a terrorist suicide bomb attack.

Exposure therapy treatment can lead to large reductions in posttraumatic stress disorder (PTSD) symptoms. Virtual reality exposure (VRE) therapy is designed to facilitate cognitive behavioral therapy for PTSD. During VRE therapy, patients go into an immersive computer-generated environment (go back to the scene of the traumatic event) to help them gain access to their memories of the traumatic event, change unhealthy thought patterns, gradually habituate to their anxiety, and reduce the intensity of associated emotions. The therapist's ability to manipulate the amount of anxiety experienced by…

A multidisciplinary nutritional-behavioral intervention improves weight loss and liver-related outcomes in patients with metabolic dysfunction-associated steatotic liver disease: A randomized controlled trial.

BACKGROUND & AIMS: Metabolic dysfunction-associated steatotic liver disease (MASLD) is strongly associated with cardiometabolic comorbidities and increased cardiovascular risk. Despite emerging pharmacological therapies, lifestyle modification remains the cornerstone of management. However, long-term adherence to dietary and behavioral recommendations remains poor. This study evaluated the benefits of a multidisciplinary intervention integrating hepatological, nutritional, and cognitive behavioral therapy (CBT)-based support ("Co-Co-Nut") on MASLD outcomes and investigated the social determina…

Treatment of posttraumatic stress disorder (PTSD).

The present review summarises recommendations for the management of posttraumatic stress disorder (PTSD) based on a consensus among international experts in the field. Cognitive-behavioural therapy (CBT) has the best evidence base as a psychotherapy modality. Virtual reality exposure therapy appears to be an emerging treatment option for PTSD. Eye Movement Desensitisation and Reprocessing (EMDR) therapy and digital CBT-based interventions did not demonstrate superiority compared to controls. Controlled studies supporting the use of psychodynamic therapy are lacking. Early prophylactic psychoth

[Treating specific childhood phobia in a single session? A systematic review of the literature].

OBJECTIVE: The essential feature of specific phobia is a persistent fear of clearly discernable, circumscribed objects or situations. The DSM-IV distinguishes four subtypes: animal, natural environment, blood-injection-injury, and situational. Specific phobias frequently co-occur. Specific phobia is one of the most common psychiatric disorders with a lifetime prevalence of 12.5% and is about twice as common in women as in men. Most phobias have a childhood onset except for the situational subtype which usually occurs during patients' twenties. It is well known that childhood anxiety increases

Effect of a prescription digital therapeutic for chronic insomnia on post-treatment insomnia severity, depression, and anxiety symptoms: results from the real-world DREAM study.

INTRODUCTION: Chronic insomnia is a substantial public health burden that often presents with co-occurring depression and anxiety. Randomized clinical trials and preliminary real-world evidence have shown that digitally delivered cognitive-behavioral therapy for insomnia (dCBT-I) is associated with improvements in insomnia, but real-world evidence is needed to determine the true impact of digital CBT-I. This pragmatic study aimed to evaluate the benefits of treating chronic insomnia with a tailored prescription digital therapeutic in a real-world population. METHODS: This prospective, single-

Cost-effectiveness of screening and treating alcohol use and depression among people living with HIV in Zimbabwe: a mathematical modeling study.

BACKGROUND: Alcohol use disorder (AUD) and major depressive disorder (MDD) drive HIV transmission in many sub-Saharan African settings. The impact of screening and treating AUD and MDD on HIV outcomes is unknown. We aimed to identify the cost-effectiveness of AUD and MDD interventions in Zimbabwe, and their potential contribution to reaching Zimbabwe's Ending the HIV Epidemic 2030 goal. METHODS: Using a validated HIV compartmental transmission model in Zimbabwe, we compared four policy scenarios: prevention as usual (baseline); implement AUD screening (using AUDIT) and treatment (motivational
