Primary colonic lymphoma.

Surgical resection of primary colonic lymphoma can be an important therapeutic tool. We performed a nonrandomized retrospective descriptive study at the University hospital tertiary care center. From January 1990 to June 2002, a total of 15 patients with primary colonic lymphoma were identified from the tumor registry at University of Alabama at Birmingham and retrospectively reviewed under Institutional Review Board approved protocol. Demographic data, clinical features, treatment method (surgery and/or chemotherapy), recurrence rate, and survival were analyzed. The results are presented as m

Sacral neuromodulation; does it affect colonic transit time in patients with faecal incontinence?

OBJECTIVE: Sacral neuromodulation (SNM) has been a successful treatment in urinary voiding disorders for years. A concomitant effect on bowel function was observed leading to the treatment of faecal incontinence with SNM. In this study we describe the effect of SNM on bowel frequency and (segmental) colonic transit time. PATIENTS AND METHODS: Fourteen patients with faecal incontinence who qualified for permanent SNM underwent a colon transit study before and one month after permanent implant. Patients completed a three-week bowel habits diary before and during stimulation. RESULTS: Median in

Positional change in colon polyps at CT colonography.

PURPOSE: To determine the frequency with which polyps change positions with respect to the bowel surface and the cause of this movement. MATERIALS AND METHODS: From December 2001 to March 2003, 113 patients underwent computed tomographic (CT) colonography prior to colonoscopy. For all confirmed polyps that were 5 mm and larger, images obtained with CT colonography were retrospectively analyzed by one author to determine if the polyp was present on both data sets or on only one data set. Retrospective evaluation of these polyps for ventral or dorsal location within the colonic lumen was perfor

A modified method for precise anastomosis during laparoscopic low anterior resection for rectal cancer: the first clinical experience and application.

BACKGROUND: There is no criterion to guide and evaluate the anastomosis of laparoscopic low anterior resection (LAR). We developed a new technique for precise anastomosis. This study endeavored to evaluate the effectiveness and safety of this new technology. METHODS: Patients with mid-low rectal cancer who underwent laparoscopic LAR in our department were enrolled retrospectively between January 1, 2021 and July 1, 2023. During the LAR, the distance between the sacral promontory and the rectal stump was measured and used to determine the length of the sigmoid colon, which was preserved for an

Characteristics of adenomatous colorectal polyps among a Saudi population.

OBJECTIVES: Colorectal cancer is a common cause of cancer-related mortality in KSA with a rising incidence. Although adenomatous polyps are well-recognized as precursors of colorectal cancer, local data are scarce. Therefore, in this study, we aimed to evaluate the characteristics of adenomatous colon polyps in the Saudi population. METHODS: We retrospectively reviewed the electronic databases of all patients who underwent colonoscopy for any indication between January 2015 and December 2019 at a tertiary care hospital. This study included adult patients who were found to have colorectal poly

A case of strongly suspected Lynch syndrome with colorectal neuroendocrine carcinoma.

BACKGROUND: Neuroendocrine cell carcinomas (NEC) of the colon and rectum are uncommon, representing ~ 0.1% of all colorectal carcinomas. They are associated with a much worse prognosis compared to adenocarcinoma of the colon and rectum, as death occurs in approximately half of all patients within 1 year. Lynch syndrome (LS) is the most common cause of inherited colorectal cancer, accounting for 2-4% of newly diagnosed colorectal cancer cases. This case is extremely rare which was strongly suspected LS as the background, and NEC as the histological type of colorectal cancer. CASE PRESENTATION:

Detection of Crohn disease lesions of the small and large bowel in pediatric patients: diagnostic value of MR enterography versus reference examinations.

OBJECTIVE: The purpose of this article is to prospectively determine the accuracy of MR enterography in detecting Crohn disease lesions from the jejunum to the anorectal region in pediatric patients, in comparison with main reference investigations. SUBJECTS AND METHODS: Fifty consecutive children with known Crohn disease underwent MR enterography with oral contrast agent and gadolinium-chelate intravenous injection. Two radiologists detected and localized lesions by dividing the bowel into nine segments (450 analyzed segments in 50 patients). Ileocolonoscopy, barium studies, intestinal ultra

A nationwide Danish cohort study challenging the categorisation into right-sided and left-sided colon cancer.

OBJECTIVES: The categorisation of colon cancer (CC) into right-sided (RCC) and left-sided (LCC) disease may not capture more subtle variances in aetiology and prognosis. In a nationwide study, we investigated differences in clinical characteristics and survival of RCC versus LCC and of the complete range of CC subsites. DESIGN: Prospective nationwide cohort study. SETTING: The database of the Danish Colorectal Cancer Group (DCCG). PARTICIPANTS: 23 487 CC patients. OUTCOME MEASURES: Overall survival (Kaplan-Meier plots) and mortality (HR from Cox proportional hazards regression analysis) ac

Primary resection and side-to-end anastomosis next to an end-colostomy in the management of acute malignant obstruction of the left bowel: an alternative in selected patients.

BACKGROUND: Patients presenting with an acute obstructing carcinoma of the left bowel are a surgical challenge. Under more difficult circumstances with gross distension of the proximal colon many surgeons will decide to defer anastomosis. Hartmann's procedure still represents a valid treatment option. We describe our experience with primary resection and side-to-end anastomosis next to an end-colostomy in the management of acute malignant obstruction of the left bowel. METHODS: The surgical procedure involves resection of the tumour and primary stapled side-to-end anastomosis next to a protec

The use of endoscopic ultrasound-guided fine-needle aspiration for investigation of submucosal and extrinsic masses of the colon and rectum.

BACKGROUND AND STUDY AIM: Ensdoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) has been reported as a useful technique for histological diagnosis of submucosal or extrinsic gastrointestinal and pancreatic lesions. The aim of this study was to evaluate the use of EUS-FNA for the diagnosis of lesions either within or adjacent to the wall of the colon and rectum. PATIENTS AND METHODS: A total of 22 patients with a lesion within the wall of, or adjacent to, the colon or rectum underwent EUS-FNA. They were divided into two groups: patients who had previously had a malignancy (the "previo

Right- and left-sided colorectal cancers display distinct expression profiles and the anatomical stratification allows a high accuracy prediction of lymph node metastasis.

Accurate preoperative prediction of lymph node metastasis and degree of tumor invasion would facilitate an appropriate decision of the extent of surgical resection of cancers to reduce unnecessary complication or to minimize the risk of recurrence in patients. We analyzed gene expression profiles characteristic of the invasiveness of colorectal carcinoma in a total of 89 cases, using a cDNA array and pattern classification algorithms. We set binary classes for a panel of clinicopathologic parameters, each of which was divided at different levels for categories (discrete) or values (continuous)

Relationship between patterns of bleeding and Hemoccult sensitivity in patients with colorectal cancers or adenomas.

Although Hemoccult (Smith Kline Diagnostics, Inc., Sunnyvale, Calif.) screening for colorectal cancer is based on detection of the peroxidase activity of blood, little is known about the relationship between patterns of bleeding from cancers or adenomas and sensitivity of the test slides. 51Cr-labeled blood loss was measured for a mean of 9.1 days in 46 patients with colorectal cancer and 28 with adenomas. Duplicate sets of Hemoccult II slides were prepared by the patients and mailed for testing with and without preliminary rehydration. Mean blood loss was significantly related to the site of

Human large intestinal epithelium: light microscopy, histochemistry, and ultrastructure.

Despite numerous reports of morphologic characteristics of premalignant and malignant large intestinal epithelium, the literature lacks comprehensive reports of the morphologic features of the epithelium of the normal large intestine, except of the rectum. Large intestinal epithelium from 41 persons was obtained, and samples from the ascending, transverse, descending, and rectosigmoid areas were studied by light microscopy, histochemical techniques, and transmission and scanning electron microscopy. The morphologic features and histochemical reactions of the various segments of the large intes

An Unusual First Presentation of Colon Cancer: Pulmonary Lymphangitic Carcinomatosis.

Pulmonary lymphangitic carcinomatosis (PLC) is a rare manifestation of metastatic disease that presents with nonspecific respiratory symptoms. We describe a young male who presented with progressive dyspnea and cough and was initially managed for pneumonia. Chest imaging demonstrated diffuse interstitial abnormalities with mediastinal lymphadenopathy and pleural effusions, prompting evaluation for interstitial lung disease (ILD). Extensive infectious and rheumatologic testing was unrevealing. Cytologic analysis of pleural fluid revealed adenocarcinoma, and subsequent imaging identified a prima

An unusual cause of gastrointestinal bleeding: adult mixed-type Henoch-Schönlein purpura.

A 25-year-old man presented with 3-day history of abdominal pain, vomiting, diarrhea, and bloody stools. The contrast-enhanced CT examination of the abdomen detected thickening and edema of intestinal canal wall. The complete colonoscopy showed hyperemia, dropsy and erosion in the sigmoid colon and rectum. The biopsies revealed obvious bleeding points in mucosa. Then, wireless capsule endoscopy (OMOM ® JS-ME-I) was carried out and showed multiple lesions in the entire small intestine with diffused hyperemia, dropsy and erosion, even multiple and large ulcers. Subsequently, symmetrical scattere

Retrospective analysis of IMA ligation level effects on nodal dissection and anastomosis in colorectal cancer.

This study compared the outcomes of high versus low inferior mesenteric artery (IMA) ligation in 380 patients undergoing surgery for rectal or sigmoid colon cancer, with intraoperative indocyanine green (ICG) fluorescence angiography used to assess anastomotic perfusion. The high ligation group demonstrated significantly greater lymph node yield (13 vs.12 p < 0.001) and was associated with more advanced T/N stages, while operative time was longer (218 ± 45 vs. 189 ± 38 min, p < 0.001). ICG navigation enabled objective blood supply evaluation, resulting in no ischemia-related anastomotic leaks

Laparoscopic repair for internal hernia associated with colostomy: a case report.

A 63-year-old woman was admitted with abdominal pain two months after laparoscopic abdominoperineal resection for rectal cancer. Computed tomography revealed dilated small intestine had passed through a defect between the lifted sigmoid colon and abdominal wall. She was diagnosed with small bowel obstruction without strangulation due to internal hernia and managed nonoperatively based on her wish. Recurrence of intestinal obstruction occurred for which curative surgery was performed laparoscopically. The herniated intestine was restored to the normal position, and the hernia orifice was closed

A patient reads the medical literature.

A 45-year-old male with no significant past medical history presented with a lump in his right groin. Outpatient computed tomography imaging ordered by his primary care physician demonstrated moderate right inguinal hernia containing nonobstructed distal descending/proximal sigmoid colon. Surgical repair of the hernia was recommended and the patient was referred for consultation with a general surgeon. The patient then conducted a search of current medical literature and, upon reflection, refused to let the consulted surgeon operate upon him. After identifying a significantly more experienced
