Targeted Therapies for Eosinophilic Gastrointestinal Disorders.

The growing recognition of eosinophilic gastrointestinal disorders has revealed the limitations of current treatment (mainly based on dietary modification and corticosteroids), and include refractoriness, high recurrence rates, and the need for long-term therapy. Research efforts, mainly in eosinophilic esophagitis (EoE), have unveiled essential pathophysiological mechanisms leading to these disorders, which bear some similarities to those of atopic manifestations and are shared by eosinophilic gastroenteritis (EGE) and eosinophilic colitis (EC). Novel targeted therapies, some imported from br

Prevention of allergy with diverse and healthy microbiota: an update.

PURPOSE OF REVIEW: Microbiota consist of symbiotic microscopic neighbors that interact on and within our bodies in diverse and incompletely understood ways throughout our lifetime. Though various associations with allergic disease have been described, clear effective therapeutic interventions to prevent allergy have been elusive. RECENT FINDINGS: The human microbiome is influenced by multiple factors, including: mode of infant delivery (vaginal vs. cesarean section), breastfeeding, diet, presence of siblings and pets, exposure to antibiotics and other medications (particularly antacids), life

Contact Allergy to Alkyl Glucosides: A Review-Part 1.

This is Part 1 of a 2-part narrative review of contact allergy and allergic contact dermatitis (ACD) to alkyl glucosides. It discusses the nature of alkyl glucosides (mainly decyl, lauryl, cetearyl, and coco-glucoside), their properties and applications, results of patch testing in routine testing and in groups of selected patients, and presents all reported case series and case reports of ACD to decyl and lauryl glucoside. The alkyl glucosides are nonionic surfactants that are produced by the reaction of glucose with fatty alcohols and that are increasingly being used in cosmetics. This class

Safety of the Selective JAK1 Inhibitor Oclacitinib in Dogs.

Apoquel(oclacitinib maleate) as a film-coated tablet, a selective Janus kinase (JAK)1 inhibitor, was approved by the United States Food and Drug Administration (FDA) in 2013 for the control of pruritus associated with allergic dermatitis and control of atopic dermatitis in dogs at least 12 months of age. The goal of this review is to describe the safety of oclacitinib in dogs based on data from investigational laboratory and field studies, independent directed studies, and an extensive postmarketing pharmacovigilance (PV) surveillance program. The safety of oclacitinib has been extensively eva

Anti-inflammatory Effect of Cannabidiol and Palmitoylethanolamide Containing Topical Formulation on Skin in a 12- O -Tetradecanoylphorbol-13-Acetate-Induced Dermatitis Model in Mice.

BACKGROUND: Chronic inflammatory skin disorders, such as atopic dermatitis, have significant disease burden worldwide. Although efficacious, the adverse effect profile of topical corticosteroids limits long-term use. As an alternative, cannabinoids have been shown to have anti-inflammatory therapeutic effects. OBJECTIVE: The aim of this study was to assess the effects of a topical cannabinoid product using dermatitis mouse model. METHODS: Thirty-five mice were randomized into treatment groups. 12- O -tetradecanoylphorbol-13-acetate was used as an irritant on 1 ear with the contralateral ear

Extensive eczema herpeticum in a previously well child.

BACKGROUND: Eczema herpeticum, also known as Kaposi varicelliform eruption, is a potentially life-threatening disseminated cutaneous viral infection. In the majority of cases, this condition develops as a complication in patients with atopic dermatitis. However, it may arise in a wide spectrum of pre-existing skin conditions, including psoriasis, seborrheic dermatitis, contact dermatitis, cutaneous T cell lymphoma, pemphigus vulgaris, and others. CASE PRESENTATION: We present the case of a 2-year-old boy who was brought to the emergency department because of a high-grade fever and rash. The f

Patch testing in children with hand eczema. A 5-year multicentre study in Spain.

BACKGROUND: Hand eczema is common in children, but affected children are seldom patch tested. Relatively few studies have assessed patch testing in the paediatric population, and none has specifically evaluated its use in hand eczema in children. OBJECTIVES: The aim of this study was to determine the prevalence of contact allergy in children with hand eczema, and to identify the most frequent allergens and their relevance. MATERIAL AND METHODS: We performed a 5-year retrospective study of children (aged 0-15 years) with hand eczema tested with the Spanish baseline series at the Dermatology D

Upadacitinib Therapy in Adolescent Severe Alopecia Areata: A Case Series and Narrative Review.

PURPOSE: Alopecia areata (AA) is a common, immune-mediated, non-scarring form of hair loss. Janus kinase inhibitors provide considerable insight into the treatment of severe AA. However, the efficacy and safety of upadacitinib treatment of adolescents and pediatric patients with severe AA is unclear, especially in those without concomitant atopic diseases. PATIENTS AND METHODS: Adolescents with severe AA receiving upadacitinib were recruited from the Dermatology Outpatient department. This is a retrospective case series. Clinical characteristics, hair regeneration, and adverse events were ana

The age of eczema onset and multiple food allergies.

BACKGROUND: Atopic dermatitis (AD) is the most common pediatric atopic disease and a risk factor for food allergy (FA) development. Children with AD are prone to developing allergic comorbidities. Its relationship with FA phenotype and timing of onset remains unclear. OBJECTIVE: To evaluate associations between AD presence and onset timing with FA-related outcomes and atopic comorbidities. METHODS: FORWARD is a longitudinal, multicenter study enrolling children younger than 12 years with physician-diagnosed FA between 2017 and 2024. Written informed consent was obtained. Data were collected

The Overlooked Burden of Atopic Comorbidities in Eosinophilic Esophagitis: Insights from a Real-Life Comprehensive Multidisciplinary Evaluation.

Background/Objectives: Eosinophilic esophagitis (EoE) is a T2-mediated disease characterized by dysphagia and food impaction. It is often associated with other atopic disorders and is considered a late manifestation of the "atopic march". In clinical practice, allergic comorbidities are frequently underdiagnosed and primarily based on self-reporting, potentially underestimating the true burden of T2-related pathology. To address this, a multidisciplinary task force was established at our tertiary center to systematically evaluate newly diagnosed patients with EoE. Methods: This cross-sectional

Part I. The role of Staphylococcus aureus in the pathophysiology of dermatologic disease.

In this first part of a two-part continuing medical education series, we examine the impact of Staphylococcus aureus (S. aureus) resistance and its pathogenic mechanisms, such as toxins, virulence factors, superantigens, quorum sensing, and biofilm formation. Next, we explore how S. aureus infection contributes to delayed wound healing, impetigo, and staphylococcal scalded skin syndrome. Additionally, S. aureus plays a crucial role in the pathophysiology of dermatologic diseases that goes beyond infection, including atopic dermatitis, cutaneous T-cell lymphoma, epidermolysis bullosa, and other

Role of Cyclosporine (CsA) in Immuno-dermatological Conditions.

Cyclosporine (CsA) is a calcineurin inhibitor that acts selectively on T cells. It has been used in dermatology since 1997 for its US Food and Drug Administration indication of psoriasis and off-label for various other inflammatory skin conditions, including atopic dermatitis, alopecia areata, urticaria, lichen planus and many others in pediatric, adults as well as in pregnant women. However, clinicians' preferences for management differ, which may have a bearing on the treatment selection. Hence, the purpose of this review is to outline the role of CsA in various skin conditions through conse

Placebo effects on itch: a meta-analysis of clinical trials of patients with dermatological conditions.

Although placebo contributes to the effects of treatment for various symptoms and conditions, its effect on itch has rarely been investigated. In this meta-analysis, the magnitude of the placebo effect on itch was systematically investigated in clinical trials including patients with chronic itch due to atopic dermatitis, psoriasis, or chronic idiopathic urticaria. From searches in four databases, 34 articles were included in the quantitative analyses. Placebo treatment significantly decreased itch (1.3 out of 10, 95% confidence interval 1.02-1.61) compared with baseline itch (effect size 0.55
