Recent progress and challenges in the discovery of new neuraminidase inhibitors.

INTRODUCTION: At present, the key public health concern is to define the way in which the next influenza pandemic should be controlled. While influenza vaccines are available, their effectiveness could be significantly reduced if new strains differ significantly from those of the vaccines. Therefore, antiviral drugs play an important role in the prevention and management of influenza. The influenza neuraminidase (NA), a surface-glycoprotein enzyme involved in releasing the virus from the host cell, has been considered as an essential therapeutic target for treatment and prophylaxis of influenz

Antiviral prescriptions to U.S. ambulatory care visits with a diagnosis of influenza before and after high level of adamantane resistance 2005-06 season.

BACKGROUND: Rapid emergence of influenza A viruses resistance to anti-influenza drugs has been observed in the past five years. Our objective was to compare antiviral prescription patterns of ambulatory care providers to patients with a diagnosis of influenza before and after the 2005-2006 influenza season, which was temporally concordant with the emergence of adamantane resistance. We also determined providers' adherence to Centers for Disease Control and Prevention (CDC) 2006 interim treatment guidelines for influenza after the dissemination of guidelines. METHODOLOGY/PRINCIPAL FINDINGS: We

Epidemiology of malaria in the forest-savanna transitional zone of Ghana.

BACKGROUND: Information on the epidemiology of malaria is essential for designing and interpreting results of clinical trials of drugs, vaccines and other interventions. As a background to the establishment of a site for anti-malarial drugs and vaccine trials, the epidemiology of malaria in a rural site in central Ghana was investigated. METHODS: Active surveillance of clinical malaria was carried out in a cohort of children below five years of age (n = 335) and the prevalence of malaria was estimated in a cohort of subjects of all ages (n = 1484) over a 12-month period. Participants were sam

The effect of hypoxia in development.

There is increasing evidence that the oxygen supply to the human embryo in the first trimester is tightly controlled, suggesting that too much oxygen may interfere with development. The use of hypoxia probes in mammalian embryos during the organogenic period indicates that the embryo is normally in a state of partial hypoxia, and this may be essential to control cardiovascular development, perhaps under the control of hypoxia-inducible factor (HIF). A consequence of this state of partial hypoxia is that disturbances in the oxygen supply can more easily lead to a damaging degree of hypoxia. Exp

I1-imidazoline-receptor agonists in the treatment of hypertension: an appraisal of clinical experience.

Although essential hypertension is usually defined as a hemodynamic disorder, it is expressed differently among individuals and varies during progression of the disease state. Therefore, various types of treatment can be envisioned. The use of selective I1-imidazoline-receptor agonists to modulate I1-imidazoline receptors involved in the central regulation of blood pressure has led to the introduction of a novel class of centrally acting antihypertensive drugs. Moxonidine, a representative molecule of this class, dissociates between a 10% alpha 2-adrenoceptor-agonist action linked with side ef

Pathology of allergic diseases.

An allergic disease may develop in any organ or system. The respective etiological factors include foreign proteins, infectious agents such as various microbes, viruses, moulds, parasites, chemical compounds often in the form of drugs usually designated as haptens, polysaccharides, benign and malignant neoplasms. Of the factors operating in the causal pathogenesis of such diseases the most important one is an exaggerated formation of antibodies, which appears to be uncontrolled and occurring irrespective of the demands of the organism. The essential morphological features in allergic inflammat

Magnesium and potassium. Inter-relationships in cardiac disorders.

Magnesium and potassium are the 2 major intracellular cations. The intracellular concentrations of these 2 ions appear to be closely correlated, but the existence of a relationship between the plasma concentrations of these ions has been controversial. A major function of potassium is to maintain the excitability of nerve and muscle tissue. Alterations in either the intracellular or extracellular potassium concentration affect membrane excitability by alterations in the resting membrane potential. The critical factor is not the actual potassium concentration in either compartment but rather th

Improvement in postharvest preservation of guava using edible coating composed of natural components-a synergistic effect.

The patient with status epilepticus has continuous or rapidly repeating seizures. Generalised convulsive status epilepticus (GCSE) is the most common form of the disorder and is a life-threatening condition that requires prompt medical management. Status epilepticus that does not respond to first-line benzodiazepines (lorazepam or diazepam) or to second-line antiepileptic drugs (phenytoin/fosphenytoin, phenobarbital or valproate) is usually considered refractory and requires more aggressive treatment. The optimal treatment of refractory GCSE has not been defined, but patients should be treated

Prevention and management of postpartum hemorrhage and hypertensive disorders of pregnancy: results from a 31-country rapid assessment.

BACKGROUND: Postpartum hemorrhage (PPH) and Hypertensive Disorders of Pregnancy (HDP) remain leading causes of maternal death. This multi-country survey asked key informants to assess uptake of evidence-based practices; determine their country's implementation of updated WHO global guidelines; and describe the private sector's role in PPH and HDP management. To our knowledge, this is among the few multi-country assessments to examine national implementation of updated WHO recommendations for PPH and HDP alongside private-sector engagement across three major regions-sub-Saharan Africa, South an

Essential thrombocythemia: 2024 update on diagnosis, risk stratification, and management.

OVERVIEW: Essential thrombocythemia is a Janus kinase 2 (JAK2) mutation-prevalent myeloproliferative neoplasm characterized by clonal thrombocytosis; clinical course is often indolent but might be interrupted by thrombotic or hemorrhagic complications, microcirculatory symptoms (e.g., headaches, lightheadedness, and acral paresthesias), and, less frequently, by disease transformation into myelofibrosis (MF) or acute myeloid leukemia. DIAGNOSIS: In addition to thrombocytosis (platelets ≥450 × 109 /L), formal diagnosis requires the exclusion of other myeloid neoplasms, including prefibrotic MF,

Targeting Blood-Brain Barrier Damage with Nanotechnology in the Fight Against Neurodegenerative Diseases.

The global health burden is attributed to neurodegenerative diseases, of which Alzheimer's disease and Parkinson's disease represent the major NDs. Changes in the blood-brain barrier system are considered an important mechanism in the pathogenesis of neurodegeneration, a process increasingly recognised. This review critically evaluates the recent advancement in nanotechnology that aims at targeting and recovering BBB disruption in neurodegenerative diseases. Nanoparticles, including polymeric, liposomal, micellar, metallic, and carbon-based systems, have the potential to cross the BBB. These m

A universal cell-based method for high-throughput analysis of viroporin function and inhibition.

Viroporins are transmembrane proteins encoded mainly by RNA viruses that oligomerize to form pores essential for multiple stages of the viral life cycle. Although they represent attractive antiviral drug targets, progress has been limited in part by the absence of high-throughput approaches to assess viroporin activity and inhibition. Here, we present a novel cell-based, fluorescence-driven method (VipoFluX) that enables high-throughput discovery of viroporins and their characterization, including inhibition in eukaryotic cells. We confirmed the robustness of the method by reproducing datasets

Emerging technologies and strategies in epilepsy surgery: toward personalised medicine.

Epilepsy surgery, the treatment of choice for drug-resistant focal epilepsy, is evolving rapidly. This progress is driven by a growing interest in the network theory of epilepsy, advances in data-driven models, and a focus on personalised treatment approaches. As a result, treatment options have expanded to include minimally invasive procedures, neurostimulation devices, and network-based interventions. Predicting surgical outcomes-such as seizure freedom and neuropsychological effects-remains challenging but is improving through advances in computational technology and molecular research, pav

EML4-ALK in Non-small Cell Lung Cancer: Molecular Mechanisms and Targeted Therapies.

Lung cancer is a malignancy characterized by high global incidence and mortality rates, with approximately 85% of cases classified as non-small cell lung cancer (NSCLC). Advances in molecular biology and targeted therapeutic agents have ushered NSCLC into the era of precision medicine. The echinoderm microtubule-associated protein-like 4 (EML4) and anaplastic lymphoma kinase (ALK) fusion gene represents a pivotal target for personalized treatment in NSCLC. Although ALK inhibitors exhibit significant efficacy against EML4-ALK-positive tumors, addressing drug resistance remains a major challenge

UniPoseScore: A Unified Graphormer3D-Based Framework for Protein-Ligand Binding Pose Scoring and Refinement.

Accurate prediction of protein-ligand binding poses is essential for structure-based drug discovery. In recent years, computational approaches, particularly molecular docking, have generated large numbers of candidate ligand poses. However, existing scoring functions remain limited in their ability to distinguish near-native poses from incorrect docking decoys. In this work, we propose UniPoseScore, a Graphormer3D-based framework for ligand pose scoring and refinement. The model simultaneously predicts pose RMSD and atomic displacement vectors to guide ligand pose refinement. Across four bench

Automated processing pipeline for functional imaging of human cardiac organoids using optical coherence tomography.

Structural and functional assessment of human cardiac organoids (hCOs) is essential for pharmaceutical drug screening and disease modeling. Existing evaluation approaches have provided valuable insights, but many rely on invasive sensors that may influence contractility, or on 2D imaging approaches that are constrained by geometric assumptions and restrict field of view. To address these challenges, we present a non-invasive imaging approach using 4D optical coherence tomography (4D-OCT) to reconstruct a synchronized, volumetric video of beating hCOs by denoising, segmenting, and synchronizing

Targeting gut-derived NETosis: A paradigm shift in understanding metformin's therapeutic action.

Metformin remains the most widely prescribed drug for diabetes management, yet recent studies have explored additional benefits, whose mechanisms are not completely understood. Recent research has highlighted the central role of the intestine in mediating metformin's therapeutic effects, involving interactions with the gut microbiota, intestinal epithelial cells, and the immune system. Among its various properties, metformin also exhibits immunomodulatory activity, drawing growing attention to its impact on neutrophil function. In particular, the excessive formation of neutrophil extracellular

Pulmonary Pharmacokinetics/Pharmacodynamics of Commonly Used Antibiotics in the Treatment of Community-Acquired Pneumonia.

Community-acquired pneumonia (CAP) remains a major global health burden, with antibacterial therapy as the primary treatment strategy. In clinical practice, plasma drug concentrations are often used to guide therapy, yet they may not reliably reflect effective pulmonary exposure, which is particularly critical for older adults, patients with comorbidities, and critically ill populations. In recent years, targeted pulmonary pharmacokinetics/pharmacodynamics (PK/PD) studies have gained increasing attention and have become an important reference for guiding precision antimicrobial therapy in clin
