Operative treatment of 2-part surgical neck fractures of the proximal humerus (AO 11-A3) in the elderly: Cement augmented locking plate Philos™ vs. proximal humerus nail MultiLoc®.

BACKGROUND: Proximal humeral fractures are with an incidence of 4-5 % the third most common fractures in the elderly. In 20 % of humeral fractures there is an indication for surgical treatment according to the modified Neer-Criteria. A secondary varus dislocation of the head fragment and cutting-out are the most common complications of angle stable locking plates in AO11-A3 fractures of the elderly. One possibility to increase the stability of the screw-bone-interface is the cement augmentation of the screw tips. A second is the use of a multiplanar angle stablentramedullary nail that might pr

Unilateral lateral mass-facet fractures with rotational instability: new classification and a review of 39 cases treated conservatively and with single segment anterior fusion.

OBJECTIVE: This study examined the clinical and radiologic results of cervical spine injuries associated with a unilateral lateral mass-facet fracture (ULMFF) in an attempt to clarify the fracture pattern and treatment strategies using single-level anterior fusion. METHODS: From July 2003 to June 2006, adult patients, who had sustained ULMFFs of the middle cervical spine, were reviewed retrospectively. The fractures were classified into six subtypes using roentgenogram and computed tomography imaging with three-dimensional analysis. Initially, 15 patients without severe translation and kyphos

Elastic nails for fibular fracture in adult tibiofibular fractures.

Treatment of adult tibiofibular fractures, especially severely comminuted fractures, is technically challenging due to the lack of reduction … an duration of fracture healing in the radiographs was 4.1 months (range, 3-8 months). No recurrent fracture dislocation and breakage of implant were observed. At the last follow-up visit, the lower-extremity alignment was excellent. Two degrees of varus deformity was found in 3 cases, and 2 degrees of valgus deformity was observed in 2 cases, but there were no seriou …

The Cat Mandible (II): Manipulation of the Jaw, with a New Prosthesis Proposal, to Avoid Iatrogenic Complications.

The cat mandible is relatively small, and its manipulation implies the use of fixing methods and different repair techniques according to its small size to keep its biomechanical functionality intact. Attempts to fix dislocations of the temporomandibular joint should be primarily performed by non-invasive techniques (repositioning the bones and immobilisation), although when this is not possible, a surgical method should be used. Regarding mandibular fractures, these are usually concurrent with other traumatic injuries that, if serious, should be treated first. A non-invasive approach should a

Aulus Cornelius Celsus and the Head Injuries.

Aulus Cornelius Celsus lived in Rome during the reigns of Augustus and Tiberius. He developed the scientific Latin, and his work De Medicina was the first medical textbook to be written in Latin, and subsequently the first medical treatise to be printed. He perhaps was a simple medical writer rather than a real physician, but his contributions to medical knowledge are outstanding. He described the tetrad of inflammation (dolor, rubor, calor, tumor), coined the term cancer, and reported several revolutionary surgical techniques. His eighth book, almost entirely dedicated to dislocations and fra

[Treatment of pelvic avulsion fractures in children and adolescents].

INTRODUCTION: Paediatric avulsion fractures of the pelvis and hip joint are rare injuries most commonly associated with a rapid muscle contraction of the respected apophysis, which is the weakest component of the growing skeleton. The chosen therapy option can be either conservative or surgical. METHODS: We present a cohort of 69 patients treated for an apophyseal avulsion fracture of the anterior superior / inferior iliac spine, the lesser and greater trochanters iliac crest and the ischial tubercle. We compared the treatment outcomes, treatment time, need for immobilisation, and complicatio

Post-traumatic heterotopic ossification about the elbow.

To evaluate the effect of surgical timing on the formation of heterotopic ossification about the elbow, 71 consecutive patients with elbow trauma requiring operative management were evaluated. Fourteen patients were excluded because they suffered from head injury, burns, or severe open injuries requiring surgery on two or more occasions. Sixteen patients were lost to follow-up, leaving a group of 41 patients. The average age of patients was 35 years. The fractures involved the olecranon in 19, distal humerus in 12, and radial head/neck in 10 patients. Six of these fractures were accompanied by

Glenoid rim lesions associated with recurrent anterior dislocation of the shoulder.

Twenty-five shoulders with recurrent instability and associated anterior glenoid rim lesions were reviewed to 1) develop a classification system of the lesions, 2) evaluate radiographic techniques in detecting the lesions, and 3) analyze the outcome of surgery. Lesions were classified into three types: Type I, a displaced avulsion fracture with attached capsule; Type II, a medially displaced fragment malunited to the glenoid rim; and Type III, erosion of the glenoid rim with less than 25% (Type IIIA) or greater than 25% (Type IIIB) deficiency. Lesions were detected by plain radiographs (19 sho

Arthroscopic Reduction and Internal Fixation of an Osseous Bankart Lesion.

UNLABELLED: Operative treatment of anterior glenohumeral instability is challenging, particularly with the presence of an anterior glenoid rim fracture, also called an "osseous Bankart lesion." Successful reduction and fixation of the lesion has been shown to greatly reduce the risk of recurrent dislocations while achieving osseous union and normalization of glenoid anatomy1. DESCRIPTION: The current surgical video article outlines a technique for an osseous Bankart repair in a patient with a displaced fracture as well as substantial pain and instability. First, the amount of bone loss is mea

Cervical and thoracic spine injury in pediatric motor vehicle crash passengers.

OBJECTIVE: Motor vehicle occupants aged 8 to 12 years are in transition, in terms of both restraint use (booster seat or vehicle belt) and anatomical development. Rear-seated occupants in this age group are more likely to be inappropriately restrained than other age groups, increasing their vulnerability to spinal injury. The skeletal anatomy of an 8- to 12-year-old child is also in developmental transition, resulting in spinal injury patterns that are unique to this age group. The objective of this study is to identify the upper spine injuries commonly experienced in the 8- to 12-year-old age

A case of tracheal injury with cricoid fracture due to blunt laryngeal trauma.

BACKGROUND: Laryngeal injuries, especially cricoid cartilage fractures, are rare but may present with airway emergencies, and appropriate airway management is vital. CASE PRESENTATION: A 24-year-old man admitted to the emergency room with isolated blunt cervical trauma after a motorcycle accident. The patient presented dysphonia, stridor, hemoptysis, cervical subcutaneous emphysema, and increasing respiratory distress. Oral intubation was initially successful, however, worsening subcutaneous emphysema and loss of effective ventilation suggested airway disruption. Emergency cricothyrotomy was

Minimally invasive stabilization of the anterior pelvic ring in fragility fractures using a submuscularly implanted internal fixator - a retrospective case series of 34 geriatric patients.

PURPOSE: Various surgical techniques for osteosynthesis in fragility fractures of the pelvis (FFP) are described. Since 2012, a submuscularly placed internal fixator has been used to stabilize the anterior pelvic ring. Indications for this procedure are a dislocation in the anterior pelvic ring and/or severe pain with associated immobility. This technique potentially has several advantages compared to subcutaneous procedures, including improved patient comfort, less irritation, and enhanced biomechanical stability due to the deeper rod placement. METHODS: Digital files of patients aged ≥ 65 y

Risk factors for cephalic necrosis after plate and screw osteosynthesis of 3- and 4-part proximal humerus fractures: prospective cohort study of 121 patients.

PURPOSE: Avascular necrosis (AVN) of the humeral head is a relatively frequent complication after proximal humerus fractures (PHF), often leading to poor outcomes and reoperation. This study investigates both non-modifiable (fracture type, trauma energy, age, sex, Charlson comorbidity index) and modifiable (surgical access, bone graft use, reduction quality) risk factors for post-operative AVN in Neer 3-4-part PHFs. METHODS: Patients with Neer 3-4-part PHFs treated using angular stable plates and followed for at least 6 months were included. Clinical and radiographic elements were evaluated.
