A Discussion of a Case of Paradoxical Ipsilateral Hemiparesis in a Patient Diagnosed with Pterional Meningioma.

Background: Although various theories have been developed to explain ipsilateral hemiparesis, the causes behind this clinical phenomenon are still poorly understood. The main pathophysiological hypotheses include the anatomical variations in decussation of the corticospinal tract, the theory of diaschisis, the Kernohan-Woltman notch phenomenon, and Ectors' syndrome. The current article aims to report the case of a 43-year-old woman diagnosed with ipsilateral hemiparesis following a right pterional meningioma, later treated by surgery. The different theories behind this paradoxical clinical phe

A Systematic Review with a Meta-Analysis of the Morphological Variants of the Corpus Callosum: Related Neurocognitive Clinical Implications.

BACKGROUND: Corpus callosum agenesis (CCA) occurs in approximately 1 in every 4000 births and is identified in 3-5% of individuals evaluated through neuroimaging for neurodevelopmental disorders. The combined prevalence of CCA and hypoplasia is estimated to range from 1.8 to 10 in every 10,000 births. METHODS: The online databases Medline, Scopus, Web of Science, Google Scholar, Cumulative Index to Nursing and Allied Health Literature, and Latin American and Caribbean Literature in Health Sciences were searched until May 2025. Two authors independently conducted the search, selected the studi

Unraveling Delayed Puberty: A Rare Case of Congenital Hypogonadotropic Hypogonadism Masked by Celiac Disease and Plummer-Vinson Syndrome.

Congenital hypogonadotropic hypogonadism (CHH) is a rare disorder that results in delayed puberty and infertility due to impaired secretion of gonadotropin-releasing hormone (GnRH). Here, we present a case of a 25-year-old male with a known history of Plummer-Vinson syndrome and celiac disease, who presented with the chief complaints of easy fatigability, shortness of breath on exertion, difficulty in swallowing, and no secondary sexual characteristics. Physical examination revealed a pale, thin, and lean male appearing significantly younger than his chronological age with notable findings of

The Important Role of the Right Dorsolateral Prefrontal Cortex in Conflict Adaptation: A Combined Voxel-Based Morphometry and Continuous Theta Burst Stimulation Study.

Humans can flexibly adjust their executive control to resolve conflicts. Conflict adaptation and conflict resolution are crucial aspects of conflict processing. Functional neuroimaging studies have associated the dorsolateral prefrontal cortex (DLPFC) with conflict processing, but its causal role remains somewhat controversial. Moreover, the neuroanatomical basis of conflict processing has not been thoroughly examined. In this study, the Stroop task, a well-established measure of conflict, was employed to investigate (1) the neuroanatomical basis of conflict resolution and conflict adaptation …

Physical Exercise and Mechanism Related to Alzheimer's Disease: Is Gut-Brain Axis Involved?

BACKGROUND: Alzheimer's disease is a progressive neurodegenerative disease characterized by structural changes in the brain, including hippocampal atrophy, cortical thinning, amyloid plaques, and tau tangles. Due to the aging of the global population, the burden of Alzheimer's disease is expected to increase, making the exploration of non-pharmacological interventions, such as physical exercise, an urgent priority. RESULTS: There is emerging evidence that regular physical exercise may mitigate the structural and functional declines associated with Alzheimer's disease. The underlying mechanism

Classification of Alzheimer's disease: application of a transfer learning deep Q-network method.

Early diagnosis is crucial to slowing the progression of Alzheimer's disease (AD), so it is urgent to find an effective diagnostic method for AD. This study intended to investigate whether the transfer learning approach of deep Q-network (DQN) could effectively distinguish AD patients using local metrics of resting-state functional magnetic resonance imaging (rs-fMRI) as features. This study included 1310 subjects from the Consortium for Reliability and Reproducibility (CoRR) and 50 subjects from the Alzheimer's Disease Neuroimaging Initiative (ADNI) GO/2. The amplitude of low-frequency fluctu

AI-based differential diagnosis of dementia etiologies on multimodal data.

Differential diagnosis of dementia remains a challenge in neurology due to symptom overlap across etiologies, yet it is crucial for formulating early, personalized management strategies. Here, we present an artificial intelligence (AI) model that harnesses a broad array of data, including demographics, individual and family medical history, medication use, neuropsychological assessments, functional evaluations and multimodal neuroimaging, to identify the etiologies contributing to dementia in individuals. The study, drawing on 51,269 participants across 9 independent, geographically diverse da

Autoimmune encephalitis in a resource-limited public health setting: a case series analysis.

BACKGROUND: Autoimmune encephalitis (AE) consists of a group of acquired diseases that affect the central nervous system. A myriad of phenotypes may be present at the onset. Due to the heterogeneity of clinical presentations, it is difficult to achieve uniformity for the diagnostic and therapeutic processes and follow-up strategies. OBJECTIVE: To describe a series of patients diagnosed with AE in a resource-limited public hospital in southern Brazil and to analyze therapeutics and outcomes. METHODS: We retrospectively reviewed the electronic medical records of patients diagnosed with AE at t

Neurobiological research on N,N-dimethyltryptamine (DMT) and its potentiation by monoamine oxidase (MAO) inhibition: from ayahuasca to synthetic combinations of DMT and MAO inhibitors.

The potent hallucinogen N,N-dimethyltryptamine (DMT) has garnered significant interest in recent years due to its profound effects on consciousness and its therapeutic psychopotential. DMT is an integral (but not exclusive) psychoactive alkaloid in the Amazonian plant-based brew ayahuasca, in which admixture of several β-carboline monoamine oxidase A (MAO-A) inhibitors potentiate the activity of oral DMT, while possibly contributing in other respects to the complex psychopharmacology of ayahuasca. Irrespective of the route of administration, DMT alters perception, mood, and cognition, presumab

Exploring causal association between functional/structural connectivity and major depression disorder: A bidirectional Mendelian randomization study.

OBJECTIVES: Prior observational studies have suggested a correlation between major depressive disorder (MDD) and communication imbalances within the resting-state brain network (RSN), but the causal relationship remains unclear. This research uses Mendelian randomization (MR) analysis to explore the potential causal effects between functional connectivity (FC), structural connectivity (SC) and MDD. METHODS: Two-sample bidirectional MR analysis was employed in this study. Inverse variance weighted (IVW) was used to explore the causal relationship between the FC/SC and MDD, with various methods

Reduced resting-state functional connectivity of default mode network subsystems in patients with obsessive-compulsive disorder.

OBJECTIVES: Neuroimaging studies have reported extensive resting-state functional connectivity (rsFC) abnormalities in the default mode network (DMN) in patients with obsessive-compulsive disorder (OCD), but findings are inconsistent. DMN can be divided into three subsystems: core, dorsal medial prefrontal cortex (dMPFC), and medial temporal lobe (MTL). This study aimed to explore abnormalities in rsFC strength within and between DMN subsystems in OCD patients, and their relationship with clinical symptoms. METHODS: This study recruited 39 OCD patients and 45 healthy controls (HCs). OCD sympt

Functional Magnetic Resonance Imaging-Specific Alternations in the Default Mode Network in Obsessive-Compulsive Disorder: A Voxel-Based Meta-Analysis.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a common and debilitating mental disorder. Neuroimaging studies have highlighted that a dysfunctional default mode network (DMN) plays a key role in the pathophysiological mechanisms of OCD. However, findings of impaired DMN regions in OCD have been inconsistent. We used meta-analysis to identify functional magnetic resonance imaging (fMRI)-specific abnormalities of the DMN in OCD. METHODS: PubMed, Web of Science, and Embase were searched to screen resting-state fMRI studies of the amplitude of low-frequency fluctuation/fractional amplitude o

Rapid eye movement sleep behavior disorder and its relation to Parkinson's disease: The potential of graph measures as brain biomarkers to identify the underlying physiopathology of the disorder.

Rapid eye movement behavior disorder (RBD) is a parasomnia characterized by the loss of skeletal muscle atonia during the rapid eye movement (REM) sleep phase. On the other hand, idiopathic RDB (iRBD) is considered the prelude of the various α-synucleinopathies, including Parkinson's disease (PD), dementia with Lewy bodies and multiple system atrophy. Consequently, over 40% of patients eventually develop PD. Recent neuroimaging studies utilizing structural magnetic resonance imaging (s-MRI), diffusion-weighted imaging (DWI), and functional magnetic resonance imaging (fMRI) with graph theoretic

Investigating predictors of progression from mild cognitive impairment to Alzheimer's disease based on different time intervals.

BACKGROUND: Mild cognitive impairment (MCI) is the early stage of AD, and about 10-12% of MCI patients will progress to AD every year. At present, there are no effective markers for the early diagnosis of whether MCI patients will progress to AD. This study aimed to develop machine learning-based models for predicting the progression from MCI to AD within 3 years, to assist in screening and prevention of high-risk populations. METHODS: Data were collected from the Alzheimer's Disease Neuroimaging Initiative, a representative sample of cognitive impairment population. Machine learning models w

Neural underpinnings of response inhibition in substance use disorders: weak meta-analytic evidence for a widely used construct.

RATIONALE: Substance use disorders (SUDs) rank among the most severely debilitating psychiatric conditions. Among others, decreased response inhibition capacities could make it more difficult for patients to abstain from drug use and maintain abstinence. However, meta-analyses on the neural basis of response inhibition in SUDs yielded conflicting results. OBJECTIVE: In this study, we revisited the neuroimaging research field and summarized the existing fMRI literature on overt response inhibition (Go/NoGo and stop-signal paradigms) across different SUDs. METHODS: We performed a systematic li

The Role of Magnetic Transcranial Stimulation in the Diagnosis and Post-Surgical Follow-Up of Cervical Spondylotic Myelopathy.

Degenerative cervical myelopathy (DCM) consists of spinal cord damage due to its compression through the cervical spine. The leading cause is degenerative. The diagnosis is clinical, and the therapeutic approach is usually surgical. Confirmation of the diagnostic suspicion is done by magnetic resonance imaging (MRI); however, this test lacks functional information of the spinal cord, the abnormality of which may precede involvement in neuroimaging. Neurophysiological examination using somatosensory evoked potentials (SSEPs) and transcranial magnetic stimulation (TMS) allows for an evaluation o
