Fanconi anaemia (FA) is a rare autosomal recessive inherited disease, which consists of bone marrow failure, skeletal deformities and multi organ nvolvement. Here, we report an emergency surgery on a FA patient with mucormycosis infection of paranasal sinuses. Since there are a few reports about FA, there is no general consensus on anaesthetic considerations so we deal with recommendations that are based on the pathology of FA. Possible difficult air way is the main concern, and low oxygen concentration and avoidance of nitrous oxide are among recommendations.

This paper presents a clinical case report of a golden eagle (Aquila chrysaetos) with foreign bodies (stones) in its proventriculus. The case deals with the identification, management and removal of foreign objects identified in the gastrointestinal tract. A surgical removal by proventriculotomy under general anaesthesia was attempted. The surgery and the recovery were uneventful, and the follow-up after six months revealed no complications. To the best of our knowledge, there are no other reports of successful foreign body removal by proventriculotomy in the golden eagle.

This article is the third of an annual series reviewing the research highlights of the year pertaining to the subspecialty of perioperative echocardiography for the Journal of Cardiothoracic and Vascular Anesthesia. The authors thank the editor-in-chief, Dr. Kaplan, and the editorial board for the opportunity to continue this series. In most cases, these will be research articles targeted at the perioperative echocardiography diagnosis and treatment of patients after cardiothoracic surgery; but in some cases, these articles will target the use of perioperative echocardiography in general.

Von Willebrand disease (VWD) is an inherited bleeding disorder with abnormal primary haemostasis due to defects in, or decreased concentration of the glycoprotein von Willebrand factor. In Denmark, the estimated prevalence of VWD is 1% corresponding to approximately 50,000 patients, but only a few hundred have been diagnosed, mostly due to prolonged bleeding after a trauma or during surgery. Thus, VWD is underdiagnosed in the general population. Improved anamnestic screening for bleeding disorders such as VWD in certain high-risk groups can facilitate institution of prophylactic treatment.

Four thousand cardiac implantable electronic devices (CIED) are implanted yearly in Croatia with constant increase. General anesthesia and surgery carry some specific risk for the patients with implanted CIEDs. Since most of the surgical procedures are performed in institutions without reprogramming devices available, or in the periods when they are unavailable, these guidelines aim to standardize the protocol for perioperative management of these patients. With this protocol, most of the procedures can be performed easily and, more importantly, safely in the majority of surgical patients.

Before making a treatment decision for patients presenting with a urethral stricture, following anamnesis and general examinations, appropriate diagnostics are necessary. This includes at least uroflowmetry and cystourethrography and the use of ultrasound and cystoscopy may be of additional help. The curative treatment of a urethral stricture is always an operation. Besides endourological procedures, open surgery for urethral reconstruction is also performed. This article aims to give an overview about the necessary diagnostic measures and the pros and cons of the different operative therapies

The diagnosis and treatment of kidney cancer continues to evolve with advances in imaging and surgical approaches. The use of nephron sparing surgery (NSS) has become the operation of choice for treating small renal masses. Yet, technical difficulty and a variety of approaches have left debate for best method in the overweight population. This review summarizes the current knowledge in the open, laparoscopic, and robotic approaches to identify key risk factors, general assessments, complication rates, and the influence of body habitus for each approach.

Important differences still exist in the management of patients who receive TAVR in Europe and in the USA. Provisional participation of cardiovascular surgeons and imaging cardiologists, use of TEE, general anesthesia with endotracheal intubation, and length of stay are some of the major differences which may define the future of TAVR in the USA. TAVR procedure will most likely be performed without CT surgery presence and with limited echocardiographic imaging guidance in USA in the near future as it is already being done in Europe.

A 13-year-old girl with right adrenal pheochromocytoma pretreated with Nifedipine, underwent surgical removal of the tumor. General anesthesia with Propofol infusion and pethidine analgesia was performed. To control the blood pressure (BP), Nitroglycerine infusions were administered successfully. Following the ligation of the adrenal veins, patient was hypotensive and inotropic support was required. This case report showed that in the absence of alpha blockers and short acting drugs the surgery could be done with a very good follow up and Nitroglycerine as antihypertensive.

The incidence of chronic obstructive pulmonary disease has increased in the last decade. The anesthetic management of these patients in upper abdomen surgery is a challenge to the anesthesiologist, since general anesthesia is associated with a high possibility of severe pulmonary complications. The search for a suitable alternative has been a subject of study for years. The case is presented of a patient with chronic obstructive pulmonary disease, who required an urgent cholecystectomy. The treatment of the case and brief review of the literature is presented.

A key figure in the development of anaesthesia in Russia was the surgeon Nikolay Ivanovich Pirogov (1810-1881). He experimented with ether and chloroform and organised the general introduction of anaesthesia in Russia for patients undergoing surgery. He was the first to perform systematic research into anaesthesia-related morbidity and mortality. More specifically, he was one of the first to administer ether anaesthesia on the battlefield, where the principles of military medicine that he established remained virtually unchanged until the outbreak of the Second World War.

Negative pressure pulmonary edema (NPPE) is defined as fluid transudation into the pulmonary interstitium which occurs as a result of elevated negative intrathoracic pressure caused by the upper respiratory tract obstruction and strong inspiratory effort. NPPE is usually seen during emergence from general anesthesia in the early post-operative period especially after upper respiratory tract surgery. We present a case of a 37-year-old male patient who underwent septoplasty operation and developed NPPE which could not diagnosed and progressed to acute subendocardial myocardial infarction.

OBJECTIVE: We present a very rare case of penetrating injury into the orbit by a needlefish. The patient underwent extirpation twice at another hospital.
METHODS: We performed foreign body removal from a right subbrow incision under general anesthesia.
RESULTS: The foreign body was successfully removed and the patient's diplopia recovered gradually after surgery.
CONCLUSIONS: Treatment was similar to that used for penetrating injury. As there is a risk of secondary infection, it is important to completely remove the fish body, followed by vigorous irrigation and debridement.

Osteogenesis imperfecta is a rare, genetically inherited syndrome involving connective tissue. It results in extremely fragile bones and disorders of other organs and body systems. Children with osteogenesis imperfecta are susceptible to bone fractures and often require surgery and anaesthesia. We describe a series of 14 general anaesthetics in three patients suffering from this disease. In one of these cases, perioperative hyperthermia was observed. Anaesthetic management of osteogenesis imperfecta and a possible relationship between this syndrome and malignant and non-malignant hyperthermia

Alveolar soft-part sarcoma (ASPS) is a rare tumor. Cure is based solely on radical surgery. The general prognosis is poor. The tongue is an unusual site in adults, but not in children. Tumor removal can cause a severe impact on quality of life, even if reconstruction is possible. ASPS is a highly vascularized tumor and antiangiogenic therapy may have a role. We describe the use of the antiangiogenic combination bevacizumab and celecoxib in the preoperative management of a patient with an ASPS of the tongue.

This report documents the sudden onset of aortic regurgitation (AR) by an exceptional cause. A 68-year-old woman suddenly experienced general fatigue, and AR was diagnosed. One year later, we performed aortic valve replacement. At surgery, three aortic cusps with a larger noncoronary cusp had prolapsed along with a free-floating fibrous band that had previously anchored the cusp to the aortic wall. Its rupture had induced the sudden onset of AR. There was no sign of infectious endocarditis. We performed successful aortic valve replacement.

Simultaneous extramedullary plasmacytoma of the nasopharynx and larynx is exceedingly rare; to the best of our knowledge, only 1 other case has been previously reported. We report a new case, which occurred in a 46-year-old woman. She was treated with a combination of surgery, radiotherapy, and chemotherapy. At 2 years of follow-up, no evidence of recurrence or progression to multiple myeloma was noted. We discuss the clinical features and treatment of plasma cell neoplasms in general and their three variants in particular.

A 68-year-old man with a history of paroxysmal supraventricular tachycardia developed wide QRS complex tachycardia one minute after induction of general anesthesia. As the wide QRS complex tachycardia appeared to be ventricular and pulsation of the radial artery could not be felt, intravenous antiarrhythmic drugs were given. Surgery was postponed. Postoperative electrophysiological study revealed ectopic atrial tachycardia. Catheter abration was performed. Regular P waves were noticed by precise review of the intraoperative electrocardiogram. The intraoperative tachycardia was thought to be su

Patients with a known difficult airway for intubation who present with intestinal obstruction are at an increased risk for receiving general anesthesia. It may be necessary to perform an awake fiberoptic intubation, or possibly a tracheostomy if an awake intubation cannot be performed. In some cases, an awake tracheostomy may not be possible due to the anatomy. We report a case in which a patient with extreme fixed neck flexion deformity in whom a tracheostomy would not have been possible, presented for emergency abdominal surgery.
