PURPOSE OF REVIEW: Acute internal carotid artery occlusion (ICAO) is associated with high rates of morbidity and mortality, whereas chronic ICAO can present insidiously with recurrent strokes. In this review, we discuss the medical and surgical management approaches among patients with acute, subacute, and chronic ICAO.
RECENT FINDINGS: We reviewed the recent literature regarding clinical presentation of acute and chronic cases of ICAO, and discuss the current data, accepted guidelines, and prospects. Surgical, endovascular, or a combination (hybrid) revascularization has been shown to be eff

A 62-year-old woman with a history of partially treated Graves disease and hypertension presented with approximately 3 weeks of worsening fatigue, lower extremity myalgias, and shortness of breath. Her medical history included a thyroid radiofrequency ablation several years earlier. Following the ablation, she was found to have some residual thyroid activity, negating the need for therapy. She was lost to follow-up after months of normal thyroid-stimulating hormone values. On this presentation, the patient was noted to be in atrial fibrillation with a rapid ventricular rate, and although she p

Patients with medically refractory headache disorders are a rare and challenging-to-treat group. The introduction of peripheral neurostimulation (PNS) has offered a new avenue of treatment for patients who are appropriate surgical candidates. The utility of PNS for headache management is actively debated. Preliminary reports suggested that 60-80% of patients with chronic headache who have failed maximum medical therapy respond to PNS. However, complications rates for PNS are high. Recent publication of 2 large randomized clinical trials with conflicting results has underscored the need for fur

UNLABELLED: Rhinophyma, a progressive, disfiguring disease of the nose, is considered the final stage of acne rosacea. In its early stages, rhinophyma can be managed with medical treatment using isotretinoin or oral antibiotics (metronidazole). However, severe cases usually are refractory to medical approaches. Surgical therapies to treat these severe refractory cases have been described. This report describes a simple, safe, efficient, and cost-effective approach to the treatment of severe rhinophyma using a scalpel and the electroscalpel, instruments readily available in every operating room

Pediatric renovascular hypertension is typically managed with revascularization, angioplasty, or radical nephrectomy. We describe the case of a 13-year-old boy with medically refractory renovascular hypertension who presented to our institution after a failed arterial bypass. Subsequent angiography and renin sampling of the segmental renal veins suggested that the lower pole of the kidney was affected exclusively by the relative hypoperfusion. We proceeded with an open partial nephrectomy in order to excise the affected region of the kidney, while preserving maximum renal function. The patient

Patients with myasthenia gravis undergo lifelong treatment with anticholinesterase agents. While the heart is usually unaffected by this disease, clinicians should bear in mind the potential adverse interaction between cardiac function and the underlying myasthenic disease or its specific medications. In the present article we report, to the best of our knowledge for the first time in the literature, a case of vasospastic acute myocardial infarction due to iatrogenic hypercholinergic crisis secondary to anticholinesterase therapy in an elderly female with myasthenia gravis. This clinical vigne

BACKGROUND: Immune thrombocytopenic purpura (ITP) is primarily characterized by immune-mediated destruction of platelets in circulation. Major treatment options range from careful observation, steroids, immunosuppressive medications, immunoglobulins to splenectomy. Interestingly and rarely, ITP has also been reported after solid organ transplantation in patients receiving immunosuppressive medications. While the incidence of new onset ITP after solid organ transplant is comparatively well documented, new onset ITP after renal transplant has only been reported in two patients. Both these patien

BACKGROUND: Fecal incontinence is a common, socially debilitating disorder. Initial management involves dietary manipulation with bulking agents or antidiarrheal medications and pelvic floor biofeedback. For patients failing these modalities, traditional surgical approaches are morbid and of variable efficacy. Sacral nerve neuromodulation (Interstim, sacral nerve stimulation) was approved in May 2011 for management of medically refractory fecal incontinence. This report summarizes our experience with this treatment modality.
METHODS: A prospectively maintained database from a colorectal speci

PURPOSE: First-line catheter ablation of ventricular tachycardia/ventricular fibrillation (VT/VF) in patients with ischemic cardiomyopathy (ICM) has been associated with improved outcomes; however, most benefit seems to be in patients with moderately depressed left ventricular ejection fraction (LVEF). Herein, outcomes were stratified based on LVEF.
METHODS: A meta-analysis of randomized controlled trials (RCTs) evaluating first-line ablation versus medical therapy in patients with VT and ICM was performed. Risk estimates and 95% confidence intervals (CI) were measured.
RESULTS: Four RCTs wi

BACKGROUND: Isolated spontaneous coronary artery dissection (SCAD) of the first septal perforator (S1) is rare.
CASE SUMMARY: A 48-year-old man presented with chest pain after physical exertion. Coronary angiography demonstrated type 1 SCAD confined to S1. Cardiac magnetic resonance (CMR) confirmed a transmural septal infarction. The patient was managed conservatively with antiplatelet and beta-blocker therapy and experienced an uncomplicated recovery.
DISCUSSION: A limited number of cases of isolated S1 SCAD have been previously reported, predominantly affecting women and all managed conser

Lupus nephritis (LN) stands out as one of the most critical complications of systemic lupus erythematosus (SLE), affecting almost 60% of the patient population. Even though more therapies have been made available for LN in the past decade, clinical outcomes remain less than ideal: nearly 10% to 30% of LN cases still advance to end-stage kidney disease (ESKD), still making the management of LN a clinical challenge. Therefore, the primary aim of treatment of LN is simple: to halt the progression toward chronic kidney disease (CKD) and prevent renal failure. In this review, we briefly describe th

BACKGROUND: Fungal endocarditis in neonates, often caused by Candida albicans, is a rare but severe condition linked to invasive medical interventions. Preterm infants are particularly vulnerable due to their immature immune systems. Diagnosis is challenging due to nonspecific symptoms, and management typically requires both antifungal therapy and surgery.
CASE REPORT: A 43-day-old preterm female infant, born at 34 weeks with intrauterine growth restriction, presented with respiratory distress. She had been treated for suspected sepsis and found to have large tricuspid vegetations. Blood cult

Spontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndrome and is often associated with fibromuscular dysplasia (FMD). We describe the case of a 55-year-old woman with a prior history of SCAD who presented with acute chest pain. Angiography revealed a new dissection in the diagonal branch of the left anterior descending artery with preserved flow, but her condition worsened with recurrent pain and ischemic electrocardiographic changes, ultimately requiring intra-aortic balloon pump support. She stabilized and was discharged on medical therapy, with plans for furth

Secondary mitral regurgitation (SMR) is frequent among patients with heart failure (HF) with reduced ejection fraction (HFrEF), and it is strongly associated with increased mortality, frequent hospitalisations, and poor quality of life. The mechanisms underlying SMR are multifactorial. While guideline-directed medical therapy and cardiac resynchronisation therapy remain the cornerstone of HFrEF management, many patients with significant SMR continue to experience significant symptoms and adverse outcomes. Managing SMR within the context of HF necessitates a multifaceted approach. Transcatheter

The detection of germline pathogenic variants (gPVs) in BRCA1/2 and other breast cancer (BC) genes is rising exponentially thanks to the advent of multi-gene panel testing. This promising technology, coupled with the availability of specific therapies for BC BRCA-related, has increased the number of patients eligible for genetic testing. Implementing multi-gene panel testing for hereditary BC screening holds promise to maximise benefits for patients at hereditary risk of BC. These benefits range from prevention programs to antineoplastic-targeted therapies. However, the clinical management of

As the role of neurologists in managing patients with rheumatic diseases expands, collaboration between rheumatologists and neurologists becomes increasingly vital. This literature review provides an overview of the central nervous system (CNS) manifestations of major autoimmune rheumatic disorders, which may include parenchymal brain and meningeal disease (stroke, meningoencephalitis, meningitis), myelopathies, psychosis, chorea, seizure disorders, and various forms of cephalea. Novel findings linking specific autoimmune markers to CNS damage reveal a direct, previously underestimated link be
