Transitions in hearing impairment and psychological distress in older adults.

OBJECTIVE: Although previous research has demonstrated an association between hearing impairment (HI) and psychological distress, less work has explored the effect of transitions in hearing status on distress over time, or the explanatory pathways that may explain this association. METHOD: We use 2 waves of data from the Canadian National Population Health Survey to examine the effect of HI transitions on changes in distress during 6 years, and to test the potential mediating effects of psychosocial factors and chronic stressors. RESULTS: Experiencing a decline in hearing was associated with

What is recovery in adolescent anorexia nervosa?

OBJECTIVE: The principal aim of this study is to describe the types of problems faced in defining recovery from anorexia nervosa (AN) as well as to illustrate the magnitude that various definitions have on recovery rates for AN. METHOD: Comparative rates of recovery from AN using a range of definitions (percent ideal body weight, psychological recovery, and combinations of these variables) were calculated using long-term outcome data from a study of adolescents treated for AN. In addition, a Kaplan-Meier survival analysis was used to model recovery over the long-term follow-up period. RESULT

Evaluation and differential diagnosis of dyspareunia.

Dyspareunia is genital pain associated with sexual intercourse. Although this condition has historically been defined by psychologic theories, the current treatment approach favors an integrated pain model. Identification of the initiating and promulgating factors is essential to reaching a successful diagnosis. The differential diagnoses include vaginismus, inadequate lubrication, atrophy and vulvodynia (vulvar vestibulitis). Less common etiologies are endometriosis, pelvic congestion, adhesions or infections, and adnexal pathology. Urethral disorders, cystitis and interstitial cystitis may a

Methodological considerations in estimating speed of cognitive operations.

Individuals infected with Human Immunodeficiency Virus (HIV) and having cognitive impairment have been described as having slow mentation. Data supporting this proposition come from a variety of sources, including Sternberg's (1966) item recognition memory task. The procedure nominally provides an index of speed of mental operations, independent from input/output demands. However, since the original use of this procedure in the 1960s, advances in cognitive psychology have revealed many of its limitations. The purpose of the present study was to examine the psychometric characteristics of this

Does cholesterol screening result in negative labeling effects? Results of the Massachusetts Model Systems for Blood Cholesterol Screening Project.

Several previous studies that looked at the effects of labeling individuals as hypertensive found increases in psychosocial distress, diminished feelings of well-being, or absenteeism. Other studies found no such effects. Thus far, similar studies relating to labeling for high blood cholesterol levels have not been published. The Massachusetts Model Systems for Blood Cholesterol Screening Project investigated whether labeling effects occurred as a result of the community-based screening, education, and referral programs it conducted in Worcester and Lowell. Nine questions concerning perception

Mental health-related quality of life mediates the association between psychosocial factors and medication adherence in rheumatoid arthritis.

OBJECTIVE: Medication adherence remains a major challenge in the long-term management of rheumatoid arthritis (RA). While clinical determinants of adherence have been widely studied, the psychological mechanisms through which individual differences influence adherence behavior are less well understood. The present study aimed to examine a theoretically informed model linking personality traits, coping-related humor styles, mental health-related quality of life, and medication adherence in patients with RA. METHODS: A cross-sectional study was conducted in a sample of adults diagnosed with RA.

The International Index of Erectile Function-6 as a tool for detecting subclinical erectile dysfunction: insights from latent variable modeling and item response theory.

BACKGROUND: In the diagnosis of erectile dysfunction (ED), the 5 + 1 erectile function domain of the International Index of Erectile Function (IIEF-6) is the reference screening tool, but its conventional sum-score scoring may mask subtle functional impairments in high-functioning men because of ceiling effects. AIM: To operationalize the detection of subclinical erectile dysfunction (SED) using latent variable modeling applied to the IIEF-6 and to validate SED as a distinct phenotype independent of ejaculatory and psychological confounders. METHODS: This cross-sectional analysis used the we

Integrating the Health Belief Model and the Theory of Planned Behavior to Decode Willingness to Pay for Cancer Screening: Evidence From Community-Based Cross-Sectional Surveys in China.

BACKGROUND: Willingness to pay for cancer screening has been widely adopted to examine barriers to screening. This study integrated the Health Belief Model and the Theory of Planned Behavior to explore the correlational pathways linked to WTP-CS among Chinese community-dwelling residents. METHODS: A multistage stratified survey was carried out in a cancer early screening demonstration zone. Questionnaires assessed demographic, psychosocial, HBM, TPB variables, and WTP-CS. Hierarchical logistic regression identified correlates; bivariate correlation analyses described pairwise associations; pa

HIV, violence, blame and shame: pathways of risk to internalized HIV stigma among South African adolescents living with HIV.

INTRODUCTION: Internalized HIV stigma is a key risk factor for negative outcomes amongst adolescents living with HIV (ALHIV), including non-adherence to anti-retroviral treatment, loss-to-follow-up and morbidity. This study tested a theoretical model of multi-level risk pathways to internalized HIV stigma among South African ALHIV. METHODS: From 2013 to 2015, a survey using total population sampling of ALHIV who had ever initiated anti-retroviral treatment (ART) in 53 public health facilities in the Eastern Cape, South Africa was conducted. Community-tracing ensured inclusion of ALHIV who wer

The temporal representation of experience in subjective mood.

Humans refer to their mood state regularly in day-to-day as well as clinical interactions. Theoretical accounts suggest that when reporting on our mood we integrate over the history of our experiences; yet, the temporal structure of this integration remains unexamined. Here, we use a computational approach to quantitatively answer this question and show that early events exert a stronger influence on reported mood (a primacy weighting) compared to recent events. We show that a Primacy model accounts better for mood reports compared to a range of alternative temporal representations across rand

Disconnection's renaissance takes shape: Formal incorporation in group-level lesion studies.

Group-level voxelwise statistical analyses of lesion-deficit relationships have been used to implicate brain structures critical for specific aspects of human cognition and behavior. Current approaches fail to account for the role of fiber tract disruptions in causing deficit, and confound cortical damage with damage to fibers of passage. Here, we develop a framework, Generalized Lesion-Symptom Mapping (GLSM), to integrate fiber tract information from DTI-based tractographic atlases in tractwise and voxelwise lesion-deficit analyses. First, we used the geniculo-calcarine system as a validation

The differential effectiveness of psychosocial and biogenetic causal explanations in reducing negative attitudes toward "mental illness".

Given the apparent failure of the "mental illness is an illness like any other" approach to reducing negative stereotypes of people with mental health problems, the differential effects of biogenetic and psychosocial explanations of psychiatric symptoms were evaluated. Attitude measures were administered to young New Zealand adults before and after a video portraying a young man with psychotic symptoms followed by either biogenetic or psychosocial explanations. Consistent with previous studies, the "medical model" approach significantly increased perceptions of dangerousness and unpredictabili

Characterization of trichotillomania. A phenomenological model with clinical relevance to obsessive-compulsive spectrum disorders.

Multiple approaches to characterization of TTM have been developed, including categoric definitions and dimensional considerations. When TTM is viewed in the context of other disorders with common comorbidities and overlapping similar phenomenologies, such as OCD, body dysmorphic disorder, skin picking, TS, and olfactory reference syndrome, clinical approaches to assessment and differential diagnosis are more complex. This article presents a general overview of TTM included as a background for a heuristic clinical framework for assessing obsessive-compulsive spectrum disorders. A comprehensive

Progress and controversies in neuropsychology of memory.

In the first part of this paper we discuss some of the most important areas of progress and of controversy in the field of memory functions and of memory impairment. Three main issues are taken separately into account: (1) the fractionation of memory into a number of sub-systems and the relationships existing among these subsystems; (2) the main theories of amnesia and the mechanisms underlying memory disorders; (3) the main anatomoclinical forms of amnesia and the brain structures subserving various components of the amnesic syndrome. In the second part we shift from the basic research to the

Understanding Solo Dining as a Multifaceted Behavioural Response: Social Anxiety, Loneliness and Well-Being.

This study examines the psychological mechanisms underlying solo dining in the 'loneliness economy'. Using structural equation modelling (SEM), it explores relationships among social anxiety, loneliness, solo dining intentions and dependency orientation as a moderator. Because solo dining may reflect both voluntary solitude and avoidance-based solitude, the present model primarily tests the avoidance-linked pathway associated with social anxiety and loneliness. Findings show that loneliness is significantly associated with solo dining behaviour, especially among individuals with high dependenc

Why Don't You Just Eat? Neuroscience and the Enigma of Eating Disorders.

Eating disorders are severe psychiatric illnesses that are associated with high mortality. Research has identified environmental, psychological, and biological risk factors that could contribute to the psychopathology of eating disorders. Nevertheless, the patterns of self-starvation, binge eating, and purging behaviors are difficult to reconcile with the typical mechanisms that regulate appetite, hunger, and satiety. Here, the authors present a neuroscience and human brain imaging-based model to help explain the detrimental and often persistent behavioral patterns seen in individuals with eat

Mind-Body Interactions in Anxiety and Somatic Symptoms.

Anxiety and somatic symptoms have a high prevalence in the general population. A mechanistic understanding of how different factors contribute to the development and maintenance of these symptoms, which are highly associated with anxiety disorders, is crucial to optimize treatments. In this article, we review recent literature on this topic and present a redefined model of mind-body interaction in anxiety and somatic symptoms, with an emphasis on both bottom-up and top-down processes. Consideration is given to the role played in this interaction by predisposing physiological and psychological

The consequences of tinnitus and tinnitus severity on cognition: A review of the behavioural evidence.

People with tinnitus report anecdotal difficulties in mental concentration and psychological treatments for tinnitus advise on concentration difficulties and how to manage them. Yet the literature lacks any coherent discussion about what precise theoretical cognitive constructs might be mediating reported concentration problems. This review addresses this gap by describing and critically appraising the behavioural evidence for the effects of tinnitus on cognitive performance (namely working memory and attention). Empirical evidence is somewhat limited, but there is some support that tinnitus i

Dental disease and health behaviour: the development of an interactional model.

This paper describes the development of an interactional model in which both the clinical and the socio-psychological aspects of dental disease and its prevention are considered. Clinical and social data were collected from two separate samples, and the interrelationships between antecedent, motivational and preventive health behaviour variables and their influences on each other and on dental health outcomes were measured by regression analyses. A complex pattern of significant interrelationships was revealed, in which direct and indirect influences on dental health and behaviour operated at

Predictors of burden in spouse caregivers of individuals with Alzheimer's disease.

This study used the following model of distress: Distress = [Exposure to Stress + Vulnerability]/[Psychological and Social Resources]. The constructs in the model were operationalized as (a) distress in response to caregiver experiences (burden); (b) exposure to stress (care recipient functional impairment in activities of daily living [ADLs]); (c) vulnerability (caregiver health problems, anger, and anxiety); and (d) resources (coping, outlook on life, and social supports). Long-term burden (15-18 months after entry) was predicted by several baseline variables: burden, care recipient ADLs, vu
