Population exposure-response model of 131I in patients with benign thyroid disease.

PURPOSE: The study aimed to explore the relationship of different exposure measures with 131I therapy response in patients with benign thyroid disease, estimate the variability in the response, investigate possible covariates, and discuss dosing implications of the results. METHODS: A population exposure-response analysis was performed using nonlinear mixed-effects modelling. Data from 95 adult patients with benign thyroid disease were analysed. Evaluated exposure parameters were: administered radioactivity dose (Aa) [MBq], total absorbed dose (ABD) [Gy], maximum of absorbed dose-rate (MXR) [

Artificial intelligence-enabled retinal vasculometry for prediction of circulatory mortality, myocardial infarction and stroke.

AIMS: We examine whether inclusion of artificial intelligence (AI)-enabled retinal vasculometry (RV) improves existing risk algorithms for incident stroke, myocardial infarction (MI) and circulatory mortality. METHODS: AI-enabled retinal vessel image analysis processed images from 88 052 UK Biobank (UKB) participants (aged 40-69 years at image capture) and 7411 European Prospective Investigation into Cancer (EPIC)-Norfolk participants (aged 48-92). Retinal arteriolar and venular width, tortuosity and area were extracted. Prediction models were developed in UKB using multivariable Cox proporti

Accounting for individualized competing mortality risks in estimating postmenopausal breast cancer risk.

PURPOSE: Accurate risk assessment is necessary for decision-making around breast cancer prevention. We aimed to develop a breast cancer prediction model for postmenopausal women that would take into account their individualized competing risk of non-breast cancer death. METHODS: We included 73,066 women who completed the 2004 Nurses' Health Study (NHS) questionnaire (all ≥57 years) and followed participants until May 2014. We considered 17 breast cancer risk factors (health behaviors, demographics, family history, reproductive factors) and 7 risk factors for non-breast cancer death (comorbidi

Letter to the editor on: "Clinical and logistical determinants of operative timing, surgical approach, and admission in pediatric supracondylar humerus fractures".

This letter to the editor examines recent work by Nelson et al. regarding the clinical and logistical determinants of operative timing, surgical approach, and hospital admission in 242 pediatric supracondylar humerus fractures. While acknowledging the study's insights into resource allocation, the author raises several methodological and statistical concerns. These include potential selection bias from differential missing data on weekends, the exclusion of nonoperatively managed patients which limits triage applicability, and the recoding of surgical approaches. Furthermore, the critique high

Explainable AI Elucidates Musculoskeletal Biomechanics: A Case Study Using Wrist Surgeries.

As datasets increase in size and complexity, biomechanists have turned to artificial intelligence (AI) to aid their analyses. This paper explores how explainable AI (XAI) can enhance the interpretability of biomechanics data derived from musculoskeletal simulations. We use machine learning to classify the simulated lateral pinch data as belonging to models with healthy or one of two types of surgically altered wrists. This simulation-based classification task is analogous to using biomechanical movement and force data to clinically diagnose a pathological state. The XAI describes which musculo

Analysis of peripheral arterial disease (PAD) patients by laser speckle measurement techniques.

Diabetic foot is a well-known problem among patients suffering from peripheral arterial diseases (PAD). This article presents an optical sensor for contactless measurement of the anatomical site based on laser speckle techniques. The sensor illuminates the inspected tissue and analyzes the captured back-reflected light from the time-changing speckle patterns. An occlusion test was implemented to provide a statistical parameter to differentiate between a low perfused and a healthy foot. A clinical study of 15 subjects was conducted. The video was analyzed by two methods: dynamic laser speckle (

NMR-based metabolomic profiling can differentiate follicular lymphoma from benign lymph node tissues and may be predictive of outcome.

Follicular lymphoma (FL) is a cancer of B-cells, representing the second most common type of non-Hodgkin lymphoma and typically diagnosed at advanced stage in older adults. In contrast to the wide range of available molecular genetic data, limited data relating the metabolomic features of follicular lymphoma are known. Metabolomics is a promising analytical approach employing metabolites (molecules < 1 kDa in size) as potential biomarkers in cancer research. In this pilot study, we performed proton nuclear magnetic resonance spectroscopy (1H-NMR) on 29 cases of FL and 11 control patient specim

Dynamic prediction by landmarking in competing risks.

We propose an extension of the landmark model for ordinary survival data as a new approach to the problem of dynamic prediction in competing risks with time-dependent covariates. We fix a set of landmark time points tLM within the follow-up interval. For each of these landmark time points tLM , we create a landmark data set by selecting individuals at risk at tLM ; we fix the value of the time-dependent covariate in each landmark data set at tLM . We assume Cox proportional hazard models for the cause-specific hazards and consider smoothing the (possibly) time-dependent effect of the covariate

Non-invasive markers of liver fibrosis in HCV mono-infected and in HIV/HCV co-infected subjects.

Non-invasive markers of liver fibrosis have been recently developed as a possible alternative to liver biopsy. The clinical management of hepatic diseases is dependent on the extent of liver fibrosis. Liver biopsy remains the gold standard but severe complications are found in about 0.5% of cases. Studies involving sequential liver biopsies are impractical, costly, and risky. Therefore non-invasive markers of liver fibrosis could be useful. These drawbacks justify an intensive research on non-invasive alternatives. Several serum markers are either directly involved in fibrosis remodelling or a

Cognitive distortion and depression in chronic pain: association with diagnosed disorders.

Previous studies have demonstrated that cognitive distortion is associated with increased levels of self-reported depression among chronic pain patients, suggesting that cognitive models of depression might be useful in this context. However, reliance on self-reports of depression hampers generalization of these results to clinically significant depressive disorders. To address this problem, we examined the association between depression diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders, Third Edition, Revised (DSM-III-R; American Psychiatric Association, 1987) (

Mind the gaps: overlooking inaccessible regions confounds statistical testing in genome analysis.

BACKGROUND: The current versions of reference genome assemblies still contain gaps represented by stretches of Ns. Since high throughput sequencing reads cannot be mapped to those gap regions, the regions are depleted of experimental data. Moreover, several technology platforms assay a targeted portion of the genomic sequence, meaning that regions from the unassayed portion of the genomic sequence cannot be detected in those experiments. We here refer to all such regions as inaccessible regions, and hypothesize that ignoring these regions in the null model may increase false findings in statis

Personalized 5-Year Prostate Cancer Risk Prediction Model in Korea Based on Nationwide Representative Data.

Prostate cancer is the fourth most common cause of cancer in men in Korea, and there has been a rapid increase in cases. In the present study, we constructed a risk prediction model for prostate cancer using representative data from Korea. Participants who completed health examinations in 2009, based on the Korean National Health Insurance database, were eligible for the present study. The crude and adjusted risks were explored with backward selection using the Cox proportional hazards model to identify possible risk variables. Risk scores were assigned based on the adjusted hazard ratios, and

External validation of ADO, DOSE, COTE and CODEX at predicting death in primary care patients with COPD using standard and machine learning approaches.

BACKGROUND: Several models for predicting the risk of death in people with chronic obstructive pulmonary disease (COPD) exist but have not undergone large scale validation in primary care. The objective of this study was to externally validate these models using statistical and machine learning approaches. METHODS: We used a primary care COPD cohort identified using data from the UK Clinical Practice Research Datalink. Age-standardised mortality rates were calculated for the population by gender and discrimination of ADO (age, dyspnoea, airflow obstruction), COTE (COPD-specific comorbidity te

A Unified Approach of Meta-Analysis: Application to an Antecedent Biomarker Study in Alzheimer's Disease.

This article provides a unified methodology of meta-analysis that synthesizes medical evidence by using both available individual patient data (IPD) and published summary statistics within the framework of likelihood principle. Most up-to-date scientific evidence on medicine is crucial information not only to consumers but also to decision makers, and can only be obtained when existing evidence from the literature and the most recent individual patient data are optimally synthesized. We propose a general linear mixed effects model to conduct meta-analyses when individual patient data are only

Predicting unplanned acute care transfer from an inpatient rehabilitation facility across rehabilitation populations.

Risk factors for acute care transfers (ACTs) from inpatient rehabilitation facilities (IRFs) vary by setting and patient population. Prior risk prediction models designed to apply to all IRF patients in an IRF have achieved poor to acceptable predictive value, while better performance (C-statistic > 0.8) was attained by models isolated to specific impairment groups. Our aim was to determine if a general risk prediction model for ACT attained sufficient predictive utility when applied across all patients requiring IRF stay. We retrospectively reviewed adults admitted to an academic, rural IRF b

[Proinflammatory cytokine content in the saliva of children suffering from chronic tonsillitis].

INTRODUCTION: We have examined 92 children aged between 6 and 15, suffering from chronic tonsillitis (CT). Tumor necrosis factor (TNF-α) and interleukin 1β and 6 (IL-1β and IL-6) contents have been defined in saliva. The control set comprised 17 healthy children. Cytokine content was defined with the enzyme multiplied immunoassay sets (Vektor Best Ltd., Russia) by enzyme-linked immunosorbent assay. The statistic analysis and data processing were carried out with statistic analysis programs (version 3.2, R Foundation for Statistical Computing, Vienna, Austria). RESULTS: The content of cytokine

Comparison of the efficacy of pentazocine and tramadol for reducing emergence agitation in children after general anesthesia: a retrospective cohort study.

BACKGROUND: Emergence agitation (EA) is a common postoperative complication in pediatric anesthesia that can lead to serious safety events and increased healthcare burden. Both pentazocine and tramadol are frequently used for postoperative analgesia in children, but their comparative effectiveness in preventing EA remains unknown. This study aimed to compare the efficacy of a single intravenous dose of tramadol versus pentazocine for reducing EA in children undergoing tonsillectomy and adenoidectomy. METHODS: This single-center retrospective cohort study included 240 children aged 2-12 years

Lipid abnormalities in chronic viral hepatitis: associations and machine learning-enhanced prediction.

BACKGROUND: Alteration in serum lipids is a well-recognized metabolic accompaniment of chronic hepatitis B (HBV) and hepatitis C (HCV) viral infections, and may reflect a combination of disease severity and metabolic dysregulation. Despite this established association, lipid profile monitoring is not standard practice in these patients, and lipid abnormalities may remain unrecognized until they influence long-term cardiometabolic outcomes or complicate overall risk stratification. This gap in routine practice provided the rationale for the present study. AIMS: This study aimed to evaluate whet

Predictability of individualized dosage regimens of carbamazepine and valproate mono- and combination therapy.

WHAT IS KNOWN AND OBJECTIVE: Many investigators agree that appropriate rational utilization of therapeutic drug monitoring (TDM) with Bayesian feedback dosage adjustment facilitates epilepsy treatment with carbamazepine (CBZ) and/or valproate (VPA) by increasing the seizure control and safety, as well as by reducing treatment costs. In previous works we have developed and used in clinical practice population pharmacokinetic (PK) models of different dosage forms for VPA and post-induction CBZ behaviour, as well as for combined therapy with CBZ plus another 'old' antiepileptic drug (AED). Logistic regression in estimates of femoral neck fracture by fall.

UNLABELLED: The latest methods in estimating the probability (absolute risk) of osteoporotic fractures include several logistic regression models, based on qualitative risk factors plus bone mineral density (BMD), and the probability estimate of fracture in the future. The Slovak logistic regression model, in contrast to other models, is created from quantitative variables of the proximal femur (in International System of Units) and estimates the probability of fracture by fall. OBJECTIVES: The first objective of this study was to order selected independent variables according to the intensit
