[Techniques of local anesthesia in the area of the head].

In the head and neck region, many procedures can be performed under local anesthesia. The anesthetic agent can be applied onto the surface of the skin or mucosa (surface anesthesia), it can be infiltrated into the tissue to be treated, or it can be used to block the regional sensory nerve (conduction anesthesia). In this paper, the most important techniques for local anesthesia of the face are presented as a short clinical guide.

Systemic herpetic infection diagnosed by retinal biopsy.

We describe a patient with systemic lupus erythematosus who developed myelopathy, optic neuropathy, retinopathy, and aseptic meningitis unresponsive to immunosuppressive and antimicrobial therapy. Although cultures and brain biopsy were unrevealing, a Herpes virus was suggested by retinal biopsy. The patient recovered dramatically after institution of acyclovir therapy and optic-nerve-sheath decompression. This case demonstrates that herpetic retinitis can occur in an iatrogenically suppressed host and underscores the value of tissue diagnosis.

[Revision of the parotid--assessing the risk and indications status for re-parotidectomy].

Recurrences of benign and malignant tumors and inflammatory complications are the main indications for revision parotidectomy. Many of these operations could be avoided if the primary surgical procedure had been adequate. Revision parotidectomy is difficult to perform when the facial nerve is embedded in scar tissue. Indications for and risks of revision parotidectomy are demonstrated for typical situations such as recurrent pleomorphic adenomas and malignancies, postoperative fistulas, and recurrences of previously operated chronic parotitis.

Radiation-induced cerebral lesions in childhood.

Post-irradiation cerebral pathologies may appear in various forms from localized radiation necrosis to a plurifocal type or from local to diffuse vasculopathies. Contrary to the current prevalent opinion, these lesions are not rare in children since young nerve tissue is particularly sensitive to ionizing radiation. Given the seriousness of some of these lesions, the authors recommend careful evaluation of the risk involved in relation to the real necessity of administering irradiation therapy in childhood.

New perspectives on polycystic ovary syndrome: hypothalamic-sympathetic-adipose tissue interaction.

Polycystic Ovary Syndrome (PCOS) is a prevalent endocrine and metabolic disorder among women of reproductive age. Although the traditional mechanistic framework has focused on dysregulation of the hypothalamic-pituitary-ovarian axis (HPOA), emerging evidence suggests that PCOS-associated metabolic disturbances predominantly originate from dysregulation of the hypothalamic-sympathetic nerve-adipose tissue axis (HSF axis). This article systematically reviews the mechanisms by which hypothalamic nuclei (including the arcuate nucleus and paraventricular nucleus) regulate adipose tissue metabolism

Use of intercostal nerve block for chest wall pain in a patient with CLOVES syndrome.

Congenital lipomatous overgrowth, vascular malformations, epidermal nevi and scoliosis/skeletal/spinal anomalies (CLOVES) syndrome is an extremely rare overgrowth syndrome characterized by complex vascular malformations. Management requires an interdisciplinary approach including debulking operations for tissue overgrowth, embolization therapy for vascular malformations and management of chronic pain due to congenital and recurrent vascular overgrowth and from scar tissue from surgical interventions. Here, we present a 35-year-old female with complex medical history due to CLOVES syndrome, wit

Rare Case of a Very Large Angioleiomyoma of the Dorsum Foot.

Angioleiomyomas are relatively rare benign smooth muscle soft tissue tumors which often occur on the extremities. They are rarely diagnosed preoperatively as clinical and radiological examination is often nonspecific and inconclusive. An 80-year-old male presented with a 10-year history of a progressively growing and symptomatic lesion on his right dorsal foot within the first intermetatarsal space. The preoperative diagnosis was suspected to be a neurogenic schwannoma arising from the deep peroneal nerve. Simple excision and histopathology confirmed a diagnosis of angioleiomyoma with nil recu

Reviewing Schwannoma-Hemangioma Composite Tumors With Their Tumorigenetic Molecular Pathways and Associated Syndromic Manifestations.

Schwannomas are common peripheral nerve sheath tumors. Cavernous hemangiomas are vascular tumors that can affect any organ system. The coexistence of cavernous hemangioma with peripheral nervous system neoplasms is a rare occurrence. So far, 37 cases have been documented, and they have been divided into two categories: conjoined association (neoplasms discovered within the tumor tissue) and discrete association (neoplasms discovered outside the tumor tissue, thus placing neoplasms and tumors in close proximity but in different locations). Schwannomas and neurofibromas are the most prevalent tu

A rare case of the recurrent surgery for cribriform-morular variant of papillary thyroid carcinoma.

INTRODUCTION: The cribriform-morular variant of papillary thyroid carcinoma (CMV-PTC) is an uncommon subtype of PTC, and it is associated with familial adenomatous polyposis (FAP). PRESENTATION OF CASE: We describe a 32-year-old female who presented recurrent CMV-PTC with FAP. Surgery performed after the recurrent disease resulted in left recurrent laryngeal nerve palsy due to formation of strong scar after initial operation. The histopathological examination revealed the recurrent CMV-PTC without thyroid capsular invasion in remnant thyroid tissue of berry ligament. In addition, the immunohi

Investigation of MicroRNA Expression in Experimental Glaucoma.

MicroRNAs are small, endogenous noncoding RNAs that modulate post-transcriptional gene expression. Recent evidence suggests that they may have a potential role in the regulation of the complex biological responses that develop in response to elevated intraocular pressure. However, contemporary microRNA assay techniques (e.g., microarrays and next-generation sequencing) typically require large amounts of RNA template that are often times difficult to obtain from glaucomatous tissue. We describe in detail an experimental protocol utilizing targeted pre-amplification and low-density polymerase ch

Biportal Endoscopic Spinal Surgery for Recurrent Lumbar Disc Herniations.

The major problems of revision surgery for recurrent lumbar disc herniation (LDH) include limited visualization due to adhesion of scar tissue, restricted handling of neural structures in insufficient visual field, and consequent higher risk of a dura tear and nerve root injury. Therefore, clear differentiation of neural structures from scar tissue and adhesiolysis performed while preserving stability of the remnant facet joint would lower the risk of complications and unnecessary fusion surgery. Biportal endoscopic spine surgery has several merits including sufficient magnification with panor

Arthrogryposis: diagnosis and therapeutic planning for patients seeking orthodontic treatment or orthognathic surgery.

Arthrogryposis is the name given to a group of musculoskeletal disorders characterized by multiple joint contractures through the body that are present at birth. There are many causes for congenital limitations of the range of motion of a joint. However, the most common form of arthrogryposis, present in 40% of cases, is a condition called amyoplasia. In many cases, abnormal nerve, muscle, and connective tissue development is involved. Hands, wrists, elbows, shoulders, hips, feet, knees, back, and jaws are affected. Because of the complexity of tissue alterations and implications in normal fac

Solitary neurofibroma of the anal canal: report of two cases.

Neurofibromas are benign nerve sheath tumors commonly found in patients afflicted with neurofibromatosis-1. In the absence of multiple neurofibromas or a diagnosis of neurofibromatosis-1, neurofibromas are referred to as solitary and have been reported to involve the skin, subcutaneous tissue, deep soft tissue, and viscera of almost all areas of the body. A neurofibroma involving the anal canal is a rare entity, with only one report in the literature. We present two cases in which large masses involving the anal canal of elderly females were locally resected and proved to be neurofibromas by h

Decellularization of fish tissues for tissue engineering and regenerative medicine applications.

Decellularization is the process of obtaining acellular tissues with low immunogenic cellular components from animals or plants while maximizing the retention of the native extracellular matrix structure, mechanical integrity and bioactivity. The decellularized tissue obtained through the tissue decellularization technique retains the structure and bioactive components of its native tissue; it not only exhibits comparatively strong mechanical properties, low immunogenicity and good biocompatibility but also stimulates in situ neovascularization at the implantation site and regulates the polari

Pericytes and Diabetic Microangiopathies: Tissue Resident Mesenchymal Stem Cells with High Plasticity and Regenerative Capacity.

Pericytes (PCs), a heterogeneous population of perivascular supporting cells, are critical regulators of vascular stability, angiogenesis, and blood-tissue barrier integrity. Increasing evidence highlights their active role in the pathophysiology of diabetic microangiopathies, including those affecting the retina, kidney, brain, heart, and peripheral nerves. In diabetes, hyperglycemia-induced PC dysfunction is a major contributor to vascular degeneration, impaired tissue repair, and disease progression across multiple organs. Pericytes also share many characteristics with mesenchymal stem cell

Traumatic and trauma-related amputations: part I: general principles and lower-extremity amputations.

Deliberate attention to the management of soft tissue is imperative when performing an amputation. Identification and proper management of the nerves accompanied by the performance of a stable myodesis and ensuring robust soft-tissue coverage are measures that will improve patient outcomes. Limb length should be preserved when practicable; however, length preservation at the expense of creating a nonhealing or painful residual limb with poor soft-tissue coverage is contraindicated. While a large proportion of individuals with a trauma-related amputation remain severely disabled, a chronically

24 hour changes in catecholamine content of rat thyroid and submaxillary glands.

The occurrence of diurnal rhythmicity in tissue norepinephrine (NE), epinephrine (E) and dopamine (DA) levels of rat thyroid and submaxillary glands was assessed in animals killed at eight time intervals during a 24 hour period. In both tissues significant peak values of NE content were found at the third hour of dark onset (at 23:00 h). Additionally a minimum of NE content of thyroid gland was apparent in rats killed at the fifth hour of photophase (at 11:00 h). As in the case of NE, peak values of E content were found during the scotophase in both tissues examined. In contrast, DA levels exh

Poly(ε-caprolactone) nerve conduit and local delivery of vegf and fgf2 genes stimulate neuroregeneration.

We studied regeneration of rat sciatic nerve while overcoming of a 5-mm diastasis with the aid of nanostructured conduit made of biocompatible and biodegradable poly(ε-caprolactone) and filled with fibrin hydrogel matrix. Implantation of the conduit into the nerve in combination with local delivery of the expression plasmid carrying genes encoding vascular endothelial growth factor (vegf) and fibroblast growth factor 2 (fgf2) leads to an increase in number of myelinated fibers and S-100(+) cells in the peripheral nerve stump and improved recovery of the nerve function. Trigeminal ganglion neuronal activity and glial fibrillary acidic protein immunoreactivity after inferior alveolar nerve crush in the adult rat.

Nerve injury to the mandibular division of the trigeminal nerve has been shown to cause satellite cell reactions that extend beyond the mandibular division of the trigeminal ganglion into the maxillary and ophthalmic divisions. The goal of this study was to determine whether any physiological abnormalities correlated with this dispersal of satellite cell reaction. We investigated the electrophysiological and satellite cell glial fibrillary acidic protein immunoreactivity (GFAP-IR) changes that occur within the trigeminal ganglion 3, 10 and 59 days after a crush injury of the inferior alveolar
