[Exploration of surgical approaches of complex tibial plateau fractures].

OBJECTIVE: To evaluate the surgical treatment and its results of complex tibial plateau fractures. METHODS: From May 2003 to November 2004, 24 cases of complex tibial plateau fractures were treated by two-incision approaches. There were 16 males (including 1 case of bilateral), and 8 females, with mean age of 40.6 years (21-65 years), eleven fractures occurred in left extremity and 14 in the right. No patients suffered open fractures. Fracture was caused by fall from height in 10 cases, by traffic accident in 12 cases and by another causes in 2 cases. According to Schatzker classification, ty

A Rare Adolescent T-Y Distal Humerus Fracture with Capitellar Shear Component Managed Using a Triangular Cannulated Screw Construct and Herbert Screw Fixation: A Rare Case Report.

INTRODUCTION: T-condylar (T-Y) distal humerus fractures are rare intra-articular injuries in adolescents (<2% of pediatric elbow fractures). The coexistence of a capitellar coronal shear fragment is exceptionally uncommon and can be easily missed on radiographs. We report successful management of this rare injury using computed tomography (CT)-guided planning and stable screw fixation. CASE REPORT: A 12-year-old boy presented after a fall with a displaced T-condylar distal humerus fracture. CT demonstrated an associated capitellar coronal shear fragment. Open reduction through a posterior par

Pipkin type III fractures: a narrative review of literature and comparative opinion on ORIF vs. arthroplasty.

Pipkin Type III is extremely rare and is associated with worse prognosis and several complications. There is still no consensus on the management of injuries and whether these fractures should be treated surgically with ORIF or arthroplasty. Pipkin type III involves a combination of ipsilateral femoral head and neck fractures. Physioanatomical complexity of this fracture is often challenging, with a poorer prognosis than any other subgroups. The most common complications after management of Pipkin Type III are avascular necrosis of the femoral head and post-traumatic osteoarthritis. The treatm

Optimizing Treatment Strategies for Distal Radius Fractures in Osteoporosis: A Comparative Review.

Osteoporosis is a common yet underdiagnosed condition that increases the risk of fractures, contributing to substantial morbidity, mortality, and healthcare costs. Distal radius fractures (DRFs) are some of the most common fractures associated with osteoporosis and often precede more severe fractures. Managing DRFs in patients with osteoporosis can be a challenge due to altered bone quality, which can affect healing and surgical fixation. This review examines both operative and nonoperative management strategies for DRFs in osteoporotic patients, emphasizing the importance of individualized tr

A Neglected Fracture of Acetabulum Treated With Total Hip Replacement.

Total hip replacement (THR) for osteoarthritis or inflammatory arthritis yields better outcomes than THR for patients with neglected acetabular fractures. The inferior clinical results mostly arise from an unforeseen bone deficit, making the treatment more time-consuming and complex for instances requiring acetabular restoration and bone grafting. There is a lack of research on the clinical results of THR in cases where acetabular fractures have been overlooked. A 55-year-old male patient presented with a malunited anterior column of the acetabulum, non-union of the posterior column with protr

Application of 3D Printing in the Surgical Planning of Trimalleolar Fracture and Doctor-Patient Communication.

To evaluate the effect of 3D printing in treating trimalleolar fractures and its roles in physician-patient communication, thirty patients with trimalleolar fractures were randomly divided into the 3D printing assisted-design operation group (Group A) and the no-3D printing assisted-design group (Group B). In Group A, 3D printing was used by the surgeons to produce a prototype of the actual fracture to guide the surgical treatment. All patients underwent open reduction and internal fixation. A questionnaire was designed for doctors and patients to verify the verisimilitude and effectiveness of

A Rare Case of Morel-Lavallee Syndrome Complicating an Anterior Dislocation of Hip Joint.

INTRODUCTION: Non-or partial weight bearing is frequently the standard treatment after peri-articular lower extremity fractures. Displaced talar neck fractures are severe injuries compromising vascularity of the corpus and consequently are at risk for non-union and avascular necrosis, the main reason to restrict weight bearing for up to three months according to most literature. CASE PRESENTATION: We report a case of a 31-year old male with a high impact car accident. His pelvic ring and Hawkins II talar fracture were treated by open reduction and internal fixation. Rehabilitation was based o

Nonunion of the proximal humerus. A review of 25 cases.

Records of 25 patients with nonunion of the proximal humerus were reviewed retrospectively. The initial fractures included 19 two-part surgical neck fractures and six three-part fractures. Fourteen fractures were treated nonoperatively and 11 surgically. Nine of 11 of the initial internal fixations were unsatisfactory. At the time of fracture 16 patients had one or more significant medical illnesses. Nonunion of the proximal humerus was associated with considerable morbidity. Patients complained of pain, stiffness, and disability in association with shoulder dysfunction. Four treatment groups

Functional outcome after calcaneus fractures: a comparison between polytrauma patients and isolated fractures.

BACKGROUND: Displaced intra-articular calcaneal fractures (DIACFs) can have long-term consequences in terms of foot pain and disability. This injury frequently occurs in the context of polytrauma patients and its influence in long-term functional prognosis is well known. The purpose of this study is to compare the etiology, severity, and functional outcome of the operated DIACFs between polytrauma patients and isolated cases. METHODS: Eighty-six patients with operated displaced intra-articular calcaneal fractures through open reduction and internal fixation (ORIF) were managed at our institut

The use of circular external fixators in the management of lower limb trauma in Dublin: a single surgeon's 20-year experience.

INTRODUCTION: It has been estimated that approximately 520,000 injury presentations are made to Irish accident and emergency departments each year. Fractures account for 20 % of these injuries. Circular external fixators (frames) have been shown to be a safe and effective method of treatment for long bone fractures where internal fixation is impossible or in-advisable. We present the outcomes of all frames applied at our institution for stabilisation of acute fractures over a 20-year period. METHODS AND METHODS: We retrospectively reviewed a prospectively compiled database of all frames appli
