Relationship of HOMA-IR with chronic kidney disease in diabetic and non-diabetic Chinese populations: findings from the REACTION study.

BACKGROUND: Chronic kidney disease (CKD) affects 8.2% of China's population and is a major global health concern. While insulin resistance (IR) is linked to CKD, the relationship between insulin resistance (HOMA-IR) and CKD risk remains unclear, especially in diabetic and non-diabetic populations. METHODS: This cross-sectional study analyzed data from 32,055 Chinese adults in the REACTION study. Logistic regression and generalized additive models assessed the association between HOMA-IR and CKD risk in diabetic (DM) and non-diabetic (Non-DM) populations, with nonlinear relationships explored

Metagenome-Based Functional Differentiation of Gut Microbiota and Ecological Adaptation Among Geographically Distinct Populations of Przewalski's Gazelle (Procapra przewalskii).

Przewalski's gazelle (Procapra przewalskii) is an endangered ungulate endemic to the Qinghai-Tibet Plateau, with a small population size and exposure to multiple ecological pressures. Its gut microbiota may play a crucial role in host environmental adaptation. To investigate the functional divergence of gut microbial communities, we performed high-throughput metagenomic sequencing on 105 wild fecal samples collected from 10 geographic regions around Qinghai Lake. The results revealed significant regional differentiation in key functional modules related to metabolism, antibiotic resistance mec

Long-term trends in major amputations for the state of Pennsylvania.

INTRODUCTION: Recent reports have suggested that the incidence of major amputations in the United States may be increasing. Despite the rapidly increasing use of closed vascular surgery and major improvements in diabetes management and medications, patients with diabetes mellitus in rural or distressed communities continue to experience a disproportionately high number of major amputations. Identification of the specific areas and populations at risk is urgently needed to address the disparity and rising incidence of a totally preventable devastating health outcome. This population-based study

Exploring the genetic landscape of COVID-19 susceptibility and severity among patients in Türkiye.

BACKGROUND: One of the most challenging factors for clinicians in managing COVID-19 has been differences in the clinical course. To investigate the parameters associated with severe disease in detail, along with examining known risk factors such as advanced age and comorbidities, understanding personal genetic factors is necessary, as the clinical course may change due to differences in the host genome. METHODS: Human genetic variants reported to be associated with severe disease were genotyped in 68 patients in COVID-19 medical wards and 52 in COVID-19 intensive care units at Hacettepe Univer

Exploring demographic, healthcare, and socio-economic factors as predictors of COVID-19 incidence rate: A spatial regression analysis.

This study investigated the relationship between demographic, healthcare, and socio-economic factors, and COVID-19 incidence rate per 100,000 population in Thailand at the province level between January 2020 and March 2022, using a five-phase approach by spatial analysis. OLS models were initially used with significant variables: household, hospital, and industry density, nighttime light index (NTLI). Spatial dependency led to spatial error (SEM) and spatial lag models (SLM), performing better with similar significant variables being applied. SEM explains 58, 65 and, 70 percent in Wave 1, 4 an

Current and future strategies for the treatment of chronic hepatitis C.

Chronic hepatitis C infection is a major cause of liver disease and hepatocellular carcinoma worldwide. While hepatitis C has been treated for decades with some success, the introduction of direct acting antiviral agents has revolutionized the treatment of hepatitis C with finite, highly effective, well-tolerated therapy and there are few populations that cannot be successfully treated now or are complicated to manage. The World Health Organization has released elimination targets in an effort to eliminate viral hepatitis and reduce dramatically the morbidity and mortality caused by both viral

Epidemiology of fracture in the Russian Federation and the development of a FRAX model.

UNLABELLED: The incidence of hip, forearm and humeral fractures was studied in two cities from the Russian Federation. Fracture rates were used to create a FRAX model for Russia and to estimate the current and future burden of fracture. PURPOSE: There is scant information on the epidemiology of fracture in Russia. The aim of the study was to determine the incidence of major fractures in order to populate a new FRAX model and to characterise the burden and future burden of fractures. METHODS: The number of hip, forearm and humeral fractures was determined in two Russian cities with a well-def

Improving Blood Pressure Among African Americans With Hypertension Using a Mobile Health Approach (the MI-BP App): Protocol for a Randomized Controlled Trial.

BACKGROUND: African Americans shoulder significant disparities related to hypertension (HTN), which is a serious public health problem in the city of Detroit, Michigan, where more than 80% of the population is African American. Connectivity through smartphones, use of home blood pressure (BP) monitoring, and newly developed mobile health (mHealth) interventions can facilitate behavioral changes and may improve long-term self-care for chronic conditions, but implementation of a combined approach utilizing these methods has not been tested among African American patients with uncontrolled HTN. Vaping: Respiratory Impacts and Public Health Challenges in the Americas.

The use of electronic nicotine delivery systems (ENDS), also known as electronic-cigarette (e-cig) or vaping, is a public health threat throughout the Americas. Teenagers and young adults have the highest rates of use of these tobacco products. However, vaping has a gateway effect, which has led to dual use of both combustible tobacco and nicotine vaping products, which is concerning given that greater adverse health effects of e-cigarettes are seen in the context of co-use with combustible tobacco. Multiple policy approaches have been used across North and South America, with varying success.

Perspectives on sex and gender inclusion in clinical practice guidelines: a qualitative study.

INTRODUCTION: Clinical practice guidelines (CPGs) provide standardised direction for the diagnosis, management and treatment of medical conditions. However, sex and gender, critical determinants of health, are often inconsistently incorporated within guideline development. This study explored expert opinions on how sex and gender are currently considered in CPGs, the barriers to meaningful integration and practical strategies to strengthen equity-focused guideline development. METHODS: Semi-structured interviews were conducted with 28 participants representing clinical practice, sex and gende

Beyond the Battlefield: Examining Acute Kidney Injury in Yemen's Humanitarian Crisis.

BACKGROUND: Acute kidney injury (AKI) represents a growing yet under-recognized public health threat in Yemen, where ongoing conflict has exacerbated vulnerabilities to infectious diseases and waterborne illnesses. Sepsis and severe volume depletion-secondary to diarrheal diseases such as cholera-are leading contributors to AKI incidence, especially in a region with a collapsing health infrastructure and limited access to dialysis. OBJECTIVE: This review consolidates existing evidence on the burden, causes, and outcomes of infection-related and dehydration-induced AKI in Yemen. It emphasizes

The gut-bone axis in osteoporosis: a multifaceted interaction with implications for bone health.

With the accelerated aging of the population, degenerative orthopedic diseases, particularly osteoporosis, have become a major public health challenge, threatening bone health and affecting the quality of life. Existing anti-osteoporosis regimens remain rather unitary or poorly adhered, which also limits the maintenance of bone health to some extent. Given the increasingly elucidated prominence of gut-related factors in osteoblasts/osteoclasts and bone formation/metabolism/maintenance, focusing on intestinal microecology and then targeting the distal bone tissue via the gut-bone axis have been

Vaccine hesitancy and bone health: Musculoskeletal sequelae of vaccine-preventable diseases.

Vaccine hesitancy threatens global health by diminishing herd immunity and permitting the return of vaccine-preventable diseases (VPDs) including poliomyelitis, measles and coronavirus disease 2019 (COVID-19). These infections carry short-term and long-term musculoskeletal consequences, such as paralysis, limb deformities, septic or reactive arthritis, osteomyelitis and chronic pain, that place an ever increasing burden on orthopaedic services. This narrative review synthesizes clinical and epidemiological evidence linking sub-optimal immunization to orthopaedic morbidity caused by major VPDs.

Detection of Japanese Encephalitis Virus in Culex Mosquitoes in Singapore.

Mosquito-borne flaviviruses are emerging pathogens of an increasing global public health concern because of their rapid increase in geographical range and the impact of climate change. Japanese encephalitis virus (JEV) and West Nile virus (WNV) are of concern because of the risk of reemergence and introduction by migratory birds. In Singapore, human WNV infection has never been reported and human JEV infection is rare. Four sentinel vector surveillance sites were established in Singapore to understand the potential risk posed by these viruses. Surveillance was carried out from August 2011 to D

Moving to Beat Anxiety: Epidemiology and Therapeutic Issues with Physical Activity for Anxiety.

PURPOSE OF REVIEW: The purpose of this paper was to provide a comprehensive narrative review of the relationship between physical activity (PA) and anxiety and the rationale for including it as a treatment option for anxiety disorders. Several gaps in the literature are highlighted alongside recommendations for future research. RECENT FINDINGS: PA in the general population has established efficacy in preventing and managing cardiovascular disease and improving wellbeing. Recent epidemiological data further suggests that people who are more active may be less likely to have anxiety disorders.
