Video-assisted repair of a ruptured right hemidiaphragm.

Video-assisted thoracic surgery is emerging as a viable approach to increasingly complex intrathoracic therapeutic procedures. We present a case of delayed diagnosis of a ruptured right diaphragm caused by a closed trauma in a young woman. The diaphragm was repaired successfully using a video-assisted procedure. Limited postoperative chest pain and muscular impairment allowed early physical therapy that resulted in an excellent outcome. Video-assisted thoracic surgery is suggested as a new treatment option for traumatic diaphragmatic disorders.

Laparoscopic diagnosis and excision of Meckel's diverticulum.

Meckel's diverticulum is one of the primary concerns in the differential diagnosis of the pediatric patient with massive, acute gastrointestinal bleeding, intussusception, or abdominal pain of uncertain cause. The hospital course of two children with Meckel's diverticulum, successfully treated by laparoscopic excision, is presented, along with details of the operative procedure. Both patients recovered from the procedure without incident and were discharged at 24 and 48 hours after surgery. The authors believe a laparoscopic approach is safe and effective in the diagnosis and treatment of Meck

Invasiveness of medical procedures and state anxiety in women.

The authors studied state anxiety in women awaiting three different medical examinations: abdominal ultrasonography, mammography, and hysterosalpingography. These procedures differ in degree of intrusiveness and cause, respectively, low, intermediate, and high levels of pain and discomfort. The evidence supported the hypothesis that the degree of anxiety will correspond to the level of invasiveness. In addition, the study examined the impact of demographic variables on degree of anxiety. The results suggest that to plan appropriate strategies for stress reduction, the medical team treating wom

Lord's procedure--the best operation for hydrocele?

A prospective study was carried out to compare Lord's hydrocele operation with traditional surgical procedures. The groups of patients investigated were comparable in terms of age, period of observation, number of previous aspirations and size of hydrocele. The incidence of isolated hydrocele of the spermatic cord, as well as epididymal cysts, was lower in the patients who underwent Lord's procedure (7.2 versus 15.8%), as was the percentage of patients reporting post-operative pain for more than 3 days (4.3 versus 15.8%, p less than 0.05).

[Primary closure of split thickness skin donor sites using surgical foil].

The advantages of using an operation foil for wound dressing when split thickness skin grafts have been taken are presented. For some years we have used an air and steam permeable plastic foil for wound dressing in cases where split thickness skin grafts have been taken. The advantages of this procedure in our opinion are: It minimizes postoperative pain. It allows inspection of the wound whenever it appears desirable because of the transparency of the foil. It reduces the cost of wound dressing and also nursing procedures.

Allograft reconstruction of distal biceps ruptures: surgical technique and analysis of outcomes in chronic tears and failed repairs.

BACKGROUND: Distal biceps tendon tears are relatively uncommon injuries that may result in pain and dysfunction. When these injuries are addressed acutely, healing rates, functional recovery, and patient outcomes are excellent. Despite generally good outcomes, failures do occur. Chronic tears of the distal biceps or those requiring revision surgery pose a unique challenge, as the tendons are often retracted and atrophic, requiring allograft reconstruction to bridge the gap in the repair construct. This manuscript aims to describe a safe, reproducible surgical technique and examine the clinical

From microvascular decompression to radiosurgery: a network meta-analysis of interventions for medication-refractory trigeminal neuralgia.

INTRODUCTION: Trigeminal neuralgia (TN) is a chronic, debilitating neuropathy characterized by sudden, severe facial pain. Given the variety of available procedural options, we performed a systematic review and network meta-analysis to compare the relative efficacy and safety of interventions for medication-refractory TN, primarily based on observational evidence. METHODS: We conducted a systematic review and network meta-analysis registered in PROSPERO (CRD420251163324). PubMed, Embase, and CENTRAL were searched from inception to July 2025. A frequentist random-effects network meta-analysis w

Delayed radicular pain following two large volume epidural blood patches for post-lumbar puncture headache: a case report.

INTRODUCTION: Postdural puncture headache (PDPH) is a known complication of diagnostic lumbar puncture. Multiple factors including needle si … might require an epidural blood patch (EBP). Though complications are rare, cases of immediate post-procedural pain and subdural epidural hematoma have been reported. Here we present a case of PDPH treated with sequential EBPs that resulted in delayed radicular pain. [Appendectomy: open versus laparoscopic versus single port : Evidence for choice of surgical procedure].

The treatment of choice in acute appendicitis is still the surgical removal of an inflamed vermiform appendix. There is still some disagreement regarding the optimal access route, i.e. conventional open or minimally invasive. The best available evidence is used to answer the question of the current optimal choice of procedure. For laparoscopic appendectomy there are evidence-based benefits in terms of access trauma, postoperative pain, wound infection rates and convalescence. For the alternative minimally invasive procedure single port appendectomy, mini-laparoscopic appendectomy or NOTES appe

Cement embolization of a segmental artery after percutaneous vertebroplasty: a potentially catastrophic vascular complication.

Serious complications related to percutaneous vertebral augmentation procedures, vertebroplasty and kyphoplasty, are rare and most often result from local cement leakage or venous embolization. We describe an adult patient who underwent multi-level, thoracic percutaneous vertebral augmentation procedures for painful osteoporotic compression fractures. The patient's percutaneous vertebroplasty performed at the T9 level was complicated by the asymptomatic, direct embolization of the right T9 segmental artery with penetration of cement into the radicular artery beneath the pedicle. We review the

[Transposition of the jejunum and selective proximal vagotomy following duodenopancreatectomy].

We report the use of a new surgical procedure following pancreaticoduodenectomy in nine patients with chronic relapsing pancreatitis and its complications. The modification of the Whipple reconstruction following resection of the head of the pancreas consists in selective proximal vagotomy, transposition of a jejunal loop for bile duct, and pancreatic anastomosis. The longest follow up period has been 18 months. In agreement with reports on other procedures for resection of the head of pancreas, the patients who had been operated on suffered few symptoms. With one exception they had no pain at

Virtual reality in percutaneous coronary intervention: from coronary imaging and procedural planning to patient care.

Percutaneous coronary intervention (PCI) is one of the most frequently performed invasive procedures in medicine, yet it remains technically demanding, carries a long operator learning curve, exposes both patients and staff to ionising radiation, and is a recognised source of patient anxiety. Virtual reality (VR), which represent immersive, computer-generated environments experienced through a head-mounted display, has been proposed as a potential tool across the PCI pathway. This narrative review synthesises literature from the last five years on four domains in which VR has been applied to P

Comparison of ligation-assisted versus snare traction-assisted endoscopic full-thickness resection for small (≤ 1.5 cm) gastric submucosal tumors originating from the muscularis propria: a propensity score-matched analysis.

BACKGROUND AND AIMS: Endoscopic full-thickness resection (EFTR) is a promising yet technically demanding procedure for gastric submucosal tumors originating from the muscularis propria (SMT-MPs). Traction and ligation are commonly employed to facilitate resection. This study compared ligation-assisted EFTR (EFTR-L) with intraluminal snare traction-assisted EFTR (EFTR-S) for small (≤ 1.5 cm) gastric SMT-MPs. METHODS: We retrospectively analyzed a cohort of 179 consecutive patients who underwent EFTR-L or EFTR-S for small gastric SMT-MPs at a single tertiary center between January 2018 and Marc

Mid-term Patient-Reported Outcomes Following Isolated Gastrocnemius Recession for Recalcitrant Plantar Fasciitis.

BACKGROUND: Most patients with plantar fasciitis experience symptom relief with nonoperative management. However, a subset remains refractory to conservative treatments and may benefit from surgical intervention. Prior studies support the use of isolated gastrocnemius recession, demonstrating favorable short-term outcomes and low complication rates. Herein we assessed the outcomes following an isolated gastrocnemius recession for recalcitrant plantar fasciitis extended to midterm outcomes (36-89 months). METHODS: Eighty-eight patients who underwent isolated gastrocnemius recession for recalci

Comparative efficacy and safety of remimazolam versus propofol in bronchoscopic procedures: a GRADE-assessed systematic review and meta-analysis.

BACKGROUND: Bronchoscopy is a widely performed diagnostic and therapeutic intervention in pulmonary medicine. Sedatives are recommended with it to improve patient comfort and procedural outcomes. Propofol, the traditional sedative, carries risks of hemodynamic and respiratory adverse events, particularly in elderly or comorbid patients. Remimazolam, an ultra-short-acting benzodiazepine, is being explored as a potential alternative with a better safety profile. OBJECTIVE: To systematically evaluate the efficacy and safety of remimazolam versus propofol for bronchoscopic procedures. STUDY DESIGN

Robotic versus laparoscopic inguinal hernia repair: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Robotic inguinal hernia repair has experienced rapid adoption despite limited high-quality comparative evidence. Previous meta-analyses have mainly focused on non-randomized series, and the only synthesis restricted to RCTs included only three trials and short-term outcomes. This systematic review and meta-analysis synthesizes all available RCT evidence across clinical, patient-reported, and surgeon-centered endpoints. METHODS: A systematic search of MEDLINE, Embase, Scopus, Web of Science, and Cochrane CENTRAL was performed. RCTs comparing robotic versus laparoscopic inguinal her

Appendiceal-source Klebsiella pneumoniae iliac pseudoaneurysm with rupture risk: a three-case series highlighting post-EVAR reassessment.

BACKGROUND: Infective native aorto-iliac aneurysms and pseudoaneurysms are uncommon but life-threatening vascular infections. In rupture-prone presentations, emergency endovascular aneurysm repair (EVAR) may be required to reduce the immediate risk of hemorrhage, but technical exclusion of the aneurysm does not establish infection control. CASE PRESENTATION: We report three patients with rupture or impending rupture of infective native aorto-iliac aneurysms who underwent emergency EVAR followed by post-procedural reassessment for residual infection. Case 1 was a 66-year-old woman with type 2
