No reduction of radiation dose following the introduction of dose-area product measurement in endoscopic retrograde cholangiopancreatography.

BACKGROUND: During radiological examinations such as endoscopic retrograde cholangiopancreatography (ERCP), it is recommended to record the dose-area product (DAP) to reduce the patient's and the physician's exposure to ionizing radiation. However, the effect of DAP measurement on the total radiation dose in ERCP procedures has never been assessed. OBJECTIVES: This study evaluated radiation dose use in ERCP procedures following the introduction of DAP measurement in the endoscopy unit. Also, patient-related, endoscopist-related and procedure-related factors associated with high DAP values wer

Bactericidal/permeability-increasing protein (rBPI21) and fluoroquinolone mitigate radiation-induced bone marrow aplasia and death.

Identification of safe, effective treatments to mitigate toxicity after extensive radiation exposure has proven challenging. Only a limited number of candidate approaches have emerged, and the U.S. Food and Drug Administration has yet to approve any agent for a mass-casualty radiation disaster. Because patients undergoing hematopoietic stem cell transplantation undergo radiation treatment that produces toxicities similar to radiation-disaster exposure, we studied patients early after such treatment to identify new approaches to this problem. Patients rapidly developed endotoxemia and reduced p

Treatment of chronic radiation proctitis with cryoablation.

BACKGROUND: Chronic radiation proctitis (CRP) is a common problem in patients receiving pelvic radiation. Current therapies have the potential for deep tissue injury with ulcerations, perforation, and fistula formation. Cryospray ablation therapy offers superficial ablation of mucosa and is a potential method to endoscopically treat CRP safely and effectively. OBJECTIVE: To determine tolerability of and response to cryotherapy in patients with radiation proctitis. DESIGN: Prospective case-series pilot study. SETTING: Baylor College of Medicine, Michael E. The Emerging Role of Visible Light in Melanocyte Biology and Skin Pigmentary Disorders: Friend or Foe?

Electromagnetic radiation, notably visible light (VL), has complicated effects on human skin, particularly pigmentation, which have been largely overlooked. In this review, we discuss the photobiological mechanisms, pathological effects, clinical applications and therapeutic strategies of VL at varying wavelengths on melanocyte biology and skin pigmentary disorders. Different VL wavelengths may impose positive or negative effects, depending on their interactions with specific chromophores, photoaging, ROS production, circadian rhythm and other photon-mediated reactions. Further in vivo and in

Genomic adjusted radiation dose stratifies radiotherapy dosing based on tumor-specific sensitivity in HPV+ oropharyngeal cancer.

Uniform radiation therapy (RT) de-escalation in HPV+ oropharyngeal squamous cell carcinoma (OPSCC) has underperformed in clinical trials, likely due to underlying genomic heterogeneity. In this issue of the JCI, Ho et al. evaluated genomic adjusted radiation dose (GARD), which integrates tumor gene expression with RT dose to estimate biological effect. In 191 locoregionally advanced HPV+ OPSCC patients treated with definitive RT with or without chemotherapy, GARD values varied widely, despite uniform dose delivery, and independently predicted overall survival. These data support a genomically

Osteometry and autoradiography of the long bones of skeletons of women exposed to internal radiation at 13--19 years of age.

Lengths of long bones of skeletons were examined in 25 women first exposed to large skeletal doses of radiation (alpha particles from radium) at the age of 13--19 years. Mean lengths did not differ significantly between two subgroups based on age at first exposure to radiation (i.e., 13--16 vs. 17--19 years). Autoradiographs of femora of some women who ingested radium at 13-15 years of age showed evidence for bone growth when blood levels of radium were low (i.e., after ingestion of radium). These findings indicate no detectable effect of large skeletal doses of radiation on growth in adolesce

Estimation of the carcinogenic risk of radiotherapy of benign diseases from shoulder to heel.

BACKGROUND AND PURPOSE: To estimate risk on fatal tumour induction in patients by radiotherapy of benign diseases at various body sites, including heterotopic ossification, omarthritis, gonarthrosis, heel spurs and hidradenitis suppurativa. MATERIAL AND METHODS: The carcinogenic risk is estimated by applying the effective dose concept from the ICRP with the average risk factor of 10% per Sv for high dose and high dose rate. Although, the concept of effective dose for the present study has limitations, its use is considered acceptable for a fairly rough risk estimate. The organ doses are calcu

A reinforcement learning approach for finding optimal policy of adaptive radiation therapy considering uncertain tumor biological response.

Recent studies have shown that a tumor's biological response to radiation varies over time and has a dynamic nature. Dynamic biological features of tumor cells underscore the importance of using fractionation and adapting the treatment plan to tumor volume changes in radiation therapy treatment. Adaptive radiation therapy (ART) is an iterative process to adjust the dose of radiation in response to potential changes during the treatment. One of the key challenges in ART is how to determine the optimal timing of adaptations corresponding to tumor response to radiation. This paper aims to develop

Survival impact on triple-modal strategy comprising hyperthermia, external radiation, and chemotherapy for unresectable locally advanced (UR-LA) pancreatic ductal adenocarcinoma.

BACKGROUND: Present treatment strategy for unresectable locally advanced (UR-LA) pancreatic ductal adenocarcinoma (PDAC) patients is controversial. Hence, a triple-modal therapy, which is a multidisciplinary strategy, was designed for patients with UR-LA PDAC by adding hyperthermia to conventional chemoradiotherapy at our institution. In this study we aimed to evaluate the effectiveness of this strategy. METHODS: Data of 21 UR-LA PDAC patients who underwent the triple-modal treatment were retrospectively analyzed for evaluating the safety and oncological effect of the treatment. The treatment

Argon plasma coagulation for the treatment of hemorrhagic radiation proctitis.

BACKGROUND AND STUDY AIMS: Chronic radiation proctitis is a complication of radiotherapy for malignant pelvic diseases. Rectal bleeding caused by radiation proctitis is difficult to manage. Argon plasma coagulation (APC) is an electrocoagulation technique that appears to be an effective and low-cost alternative to the use of lasers in gastrointestinal endoscopy. The aim of this study was to evaluate the efficacy of APC, as well as patients' tolerance of the procedure, in the treatment of bleeding radiation-induced proctitis. PATIENTS AND METHODS: The charts of 16 patients with chronic radiati

Expanding the Treatment Window for the FDA-Approved Radiation Countermeasure Nplate.

INTRODUCTION: Recent shifts in the geopolitical landscape, including the threat of nuclear conflict and the proliferation of nuclear materials, have heightened concerns about radiological emergencies. Acute radiation syndrome (ARS) is a major health consequence of such events, but effective medical countermeasures (MCMs) remain limited. Although several compounds have been approved as mitigators to treat ARS, there is currently no FDA-approved drug that can be used as a prophylaxis-administered before exposure to prevent or lessen the effects of radiation. One key mitigator is Nplate (romiplos

Curative intent therapy of stage I-III non-small cell lung cancer: a patient-centered precision approach to assess, measure, and interpret benefits and harms.

BACKGROUND: The number of people diagnosed with stage I-III non-small cell lung cancer (NSCLC) is increasing, in part due to greater implementation of lung cancer screening and earlier detection. Definitive surgery, radiation, or chemoradiation are increasingly utilized along with adjunctive therapies that include chemotherapy, radiation, immune checkpoint inhibitors (ICIs), and receptor tyrosine kinase inhibitors (rTKIs). However, remedial and adverse effects exist for each modality that must be accounted for in individual treatment plans with curative intent. The objective of this study was

Secondary supratentorial anaplastic astrocytoma following treatment of medulloblastoma.

The development of secondary tumours is a rare but well known late effect of radiation therapy of lesions in the central nervous system. Most radiation-induced tumours are of mesenchymal origin, but on rare occasions gliomas can occur. We describe a patient in whom a supratentorial anaplastic astrocytoma developed 15 years after surgery and radiotherapy for a childhood posterior fossa medulloblastoma. A concise review of the pertinent literature is given.
