BACKGROUND: Pediatric patients with open forearm fractures who undergo operative irrigation and debridement (I&D) are often treated with implant fixation due to concern for delayed healing, nonunion, or loss of alignment. For an open forearm fracture treated with I&D, it is unknown whether reduction and casting alone without implant fixation affects outcomes in young patients. We aim to compare the results of pediatric open forearm fractures treated with I&D and cast immobilization with no implant fixation (NIF) to those treated with implant fixation (IF).

Features of supraosteal osteosynthesis of the distal metaepiphysis of radial bone in elderly patients, permitting stable fixation of bone fragments in conditions of osteoporosis, were highlighted. Results of treatment of 35 patients, aged 60-78 years were analyzed. The proper plate positioning and conduction of the distal screw line provides the stable fixation, eases early rehabilitation and facilitates good functional results.

It is often the case that screws cannot be firmly anchored in small, marginal joint fragments. This is especially relevant in osteoporotic bone. By redesigning the last plate hole of a one-third tubular plate to function as a hook, these bone fragments can be securely grasped and stabilized.
