Indium-111 leukocyte accumulation in submandibular gland saliva as a cause for false-positive gut uptake in children.

Unexplained false-positive accumulation of In-111 leukocyte in the gastrointestinal tract has been reported previously. In a pediatric population, uptake in the submandibular gland was noted in 19 of 42 children. This is believed to be a normal finding in children that is not seen in adults. In some of these 19 children, unexplained gastrointestinal activity was also noted. Saliva from a patient with false-positive gastrointestinal uptake and positive submandibular activity was positive for In-111. One suggested cause of unexplained gut activity may be secondary to swallowed activity in the sa

Neonatal myotonic dystrophy associated with prenatal ventriculomegaly. A case report.

Myotonic dystrophy is an autosomal dominant neuromuscular disease in which the maternal manifestations may be mild or subclinical. However, the congenital form, transmitted only from affected mothers, is reported to have a neonatal mortality rate as high as 50%. Ultrasonographic features of pregnancies with affected fetuses include hydramnios and decreased fetal movement, breathing motion and swallowing. Several reports have also noted an occasional infant with ventriculomegaly or hydrocephalus. It appears that prenatal ventriculomegaly may also be an occasional ultrasonographic finding herald

[Recurrent multiple paralysis of cranial nerves. Gougerot-Sjögren syndrome].

A 53 year-old woman presented with 6 episodes of recurrent cranial nerve palsies over seven years, each episode lasting several weeks. She had three episodes of diplopia, one of difficulty with swallowing, a left facial paralysis and two episodes of trigeminal sensory neuropathy. Sjögren's syndrome was diagnosed when the patient developed dryness of the eyes and mouth, 7 years after the first episode. The diagnosis was proved by biopsy of the accessory salivary glands, the presence of a hypergammaglobulinaemia and anti-tissue antibodies.

Button-battery ingestion--a hazard of modern living.

Disc batteries are ingested by children with ever-increasing frequency as they become more widely available for a multitude of small electronic devices in the home. Because of their unique properties their ingestion cannot be managed in quite the same expectant way as most other foreign bodies swallowed. Experience with 13 cases is briefly recorded, the literature is reviewed, and based on our findings a plan of management is suggested. The potential hazards of button-battery ingestion should be publicized, and prevention encouraged by parental education, manufacturer's packaging and safe disp

Salvage of a failed colon interposition in the esophagus with a free jejunal graft.

A patient who had sustained lye burns of the esophagus had undergone reconstruction with a colon interposition. Because of necrosis and stricture of the interposed colon, he was unable to manage salivary secretions and was maintained on gastrostomy feedings. The defect from the pharynx to the midmanubrial level was reconstructed with a free jejunal graft by using microvascular surgical techniques. At 18-month follow-up, barium swallow roentgenography showed flow through the jejunal and colonic segments and into the stomach. The patient gained 15.9 kg postoperatively and was able to consume a n

[Accidental fatal formaldehyde poisoning].

A previously healthy man accidentally swallowed 20 to 50 ml of Formalin (25%). Relevant clinical findings after the accident were: severe metabolic acidosis, severe disseminated intravascular coagulation and renal failure. He died 7 hours after formaldehyde ingestion-presumably from toxic pulmonary oedema. Relevant post-mortem findings were: massive pulmonary damage (toxic oedema), leather-like thickening of the gastric wall and multiple subendocardial haemorrhages. The treatment of formaldehyde poisoning is briefly discussed.

Rehabilitation of patients with postoperative deficits cranial nerves VIII through XII.

Increasingly aggressive surgical attack upon disease in and about the base of the skull will continue to produce various combinations of deficits of the 8th through 12th cranial nerves. Although isolated paralyses of these nerves usually can be managed with little difficulty, various combinations of paralyses can result in loss of airway, voice, or swallowing functions that are challenging to the head and neck surgeon. A thorough understanding of the disordered physiologic conditions that these lesions can produce, coupled with a logical medical and surgical approach to their management, may r

Infravestibular horizontal partial laryngectomy. A new surgical method.

We have developed a new functional surgical technique for cancers of the larynx specially those situated glottically and subglottically which are considered remedial to a total laryngectomy. Specifically an infravestibular horizontal partial laryngectomy can be performed to remove neoplasms together with the glottis and subglottis. The larynx then may be reconstructed by suturing the bands to the ascended trachea and the conserved upper half of the tyroid cartilage. The results have been encouraging. Swallowing, phonation and respiration by the natural vias has been obtained in 8 of the 12 pat

Treatment of carcinoma of the base of the tongue and the larynx.

Cancer of the base of the tongue and the larynx is usually treated primarily with irradiation to 6,000 rads. Persistence of tumor after irradiation is common and requires a logical, thoughtful approach designed to augment all tissues removed. Careful preoperative planning and the use of the lateral approach to the base of the tongue, in conjunction with pharyngostome and laryngostome formation allows wide resection of the cancer. Preservation of the function of speech and swallowing, and preservation of the mandibular contour are important, and correlate closely with carotid artery protection.

A patient with respiratory obstruction in glandular fever.

Simcock, A. D., and Prout, B. J. (1974).Thorax, 29, 145-146. A patient with respiratory obstruction in glandular fever. Glandular fever is a common disease, and one of the most consistent features is enlargement of tonsillar and nasopharyngeal lymph nodes. This is usually of little serious consequence and, if difficulty in swallowing or breathing occurs, responds rapidly to treatment. We report a case of glandular fever where lymph node enlargement caused respiratory obstruction which required urgent tracheostomy. Some other uncommon features are discussed.

Translation and Validation of the TOR-BSST Into Turkish for Stroke Patients.

PURPOSE: The most common cause of death in Turkey is attributed to vascular diseases, including stroke. Dysphagia stands out as one of the prevalent and life-threatening complications that often follow a stroke. Within the Turkish context, the availability of validated bedside screening tests for assessing dysphagia remains limited. The primary objective of this study was to undertake the translation of the TOR-BSST into Turkish and subsequently validate this newly translated Turkish version. METHOD: Once the translation of the TOR-BSST into Turkish was accomplished, 60 stroke patients were c

Adrenal Suppression From Mucosal Absorption of Dexamethasone Mouthwash.

We present an unusual cause of reversible secondary adrenal insufficiency in a breast cancer patient receiving immunotherapy: systemic absorption of a dexamethasone mouthwash used to "swish and spit," without being swallowed, which resulted in elevated serum dexamethasone concentrations. Most such patients are presumed to have permanent adrenal insufficiency - well recognized as a complication of immunotherapy - and committed to life-long exogenous glucocorticoid treatment. Recognition of the potential for such oral mucosal steroids to be systemically absorbed, which might be more common than

Laryngotracheal Clefts: History, Evaluation, and Modern-Day Management.

There has been considerable growth in our knowledge of laryngeal tracheoesophageal clefts or laryngeal clefts, an anomalous communication between the airway and the esophagus. The most recent classifications separate laryngeal clefts into 4 major groups differentiated by the depth of the cleft. Presentation includes respiratory and swallowing difficulties and work up includes evaluation in and out of the operating room. Minor grade clefts may be managed in a variety of ways while higher grade clefts will require surgical intervention. A multidisciplinary team is key in all aspects of diagnosis

Pyloric stenosis following hydrochloric acid-based bathroom cleaner ingestion in a toddler.

Accidental ingestion of corrosive household substances can lead to serious injuries in young children. While damage to the oesophagus is most common, injuries affecting the stomach, especially the pylorus, are rare and can be easily missed. We describe a toddler who developed persistent vomiting after swallowing a hydrochloric acid-based bathroom cleaner. Investigations revealed complete blockage at the gastric outlet due to scarring. She was successfully treated with surgery. This case highlights the importance of early recognition and timely intervention, especially when the child continues

[Acute airway obstruction from a retropharyngeal hemorrhage].

A case is reported of a man in his 70s that presented to the emergency department due to difficulty swallowing after a fall. He was found to have a large retropharyngeal hematoma, which led to complete airway obstruction about an hour after the injury. As oral endotracheal intubation was impossible due to the bleeding, an emergency cricothyrotomy was performed in an ambulance by an emergency medicine trainee. The patient was then transferred to Landspitali for further treatment, but was discharged home in good health 19 days later. Retropharyngeal bleeding following trauma is rare. A pre-hospi

Generation of an ITPR1+/- and isogenic control induced pluripotent stem cell line for SCA15/16 model development.

Spinocerebellar ataxia type 15/16 (SCA15/16) is a rare neurodegenerative disorder caused by heterozygous deletions of ITPR1, leading to haploinsufficiency of the encoded endoplasmic reticulum membrane calcium channel. Patients present with progressive gait disturbances, abnormal eye movements, difficulties with speech and swallowing, and tremors associated with atrophy of the cerebellum. Using CRISPR/Cas9 technology, we generated ITPR1+/- and isogenic control induced pluripotent stem cell (iPSC) lines from PGP1 iPSCs for SCA15/16 model development. The clones were genotyped, karyotyped, and as

Conduit Selection for Reconstruction After Esophagectomy for Esophageal Cancer.

The reconstruction of the esophagus after esophagectomy presents many technical and management challenges to surgeons. An effective gastrointestinal conduit that replaces the resected esophagus must have adequate length to reach the upper thoracic space or the neck, have robust vascular perfusion, and provide sufficient function for an adequate swallowing mechanism. The stomach is currently the preferred conduit for esophageal reconstruction after esophagectomy. However, there are circumstances, where the stomach cannot be utilized as a conduit. In these cases, an alternative conduit must be c

[Exploration of acupuncture for myasthenia gravis based on the theory of yiqi zhouliu].

Based on HUANG Yuanyu's theory of yiqi zhouliu (constant circulatory flowing of qi) it is explored that the pathogenesis of myasthenia gravis refers to qi deficiency in the middle jiao and the disarrangement of zhouliu. It is summarized that the approach to acupuncture diagnosis and treatment should focus on replenishing qi of the middle jiao based on yangming and tonifying the functions of zang organs to eliminate pathogens, assisted with regulating shen (spirit) and opening the orifices so as to promote the recovery of motor and non-motor functions of the four limbs, eyelids, speech articula
