The Role of Machine Perfusion for Liver Transplantation in Children.

Pediatric liver transplantation (PLT) is the only lifesaving procedure for children with advanced liver disease and tumors. Despite significant progress and overall success in PLT, the field continues to face challenges due to procedural complexity across a heterogeneous age group, especially in infants and small children. Despite the use of living donor grafts, waitlist mortality remains disproportionately high in these younger patients. The unique challenges in PLT-including limited donor availability and higher risk of complications-underscore the need for improved strategies to evaluate an

Socioeconomic Differences in Navigating Access to Lung Transplant.

IMPORTANCE: Inequitable access to transplant in the US is well recognized, yet the nature and extent of upstream disparities in care prior to transplant are unknown. OBJECTIVE: To understand patterns of referral for lung transplant by race, ethnicity, and neighborhood-level socioeconomic status. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included adults aged 18 to 80 years with obstructive and restrictive lung disease from a single large-volume transplant center in Cleveland, Ohio, who were diagnosed between January 1, 2006, and May 11, 2023. EXPOSURES: Neighborhood

Management of Pulmonary Granulomatous Venulitis by Lung Transplantation.

Granulomatous venulitis is an exceedingly rare condition characterized by granulomatous inflammation and obliteration of small pulmonary veins, leading to the development of pulmonary venoocclusive disease. The prognosis is poor, with most patients surviving less than 1 year after diagnosis. We present the first case, to our knowledge, of successful management of this disease by lung transplantation.

Kidney Transplant: More than Immunological Problems.

Kidney transplantation (KT) represents a pivotal intervention for patients with chronic kidney disease (CKD), significantly improving survival and quality of life. However, KT recipients face an array of non-immunological complications, collectively amplifying cardiovascular (CV) and metabolic risks. This review explores the intersection of cardio-metabolic syndrome and KT, emphasizing the recently introduced cardiovascular-kidney-metabolic (CKM) syndrome. CKM syndrome integrates metabolic risk factors, CKD, and CV disease, with KT recipients uniquely predisposed due to immunosuppressive thera

Long-Term Care of the Liver Transplant Recipient.

Reflecting continued enhancements of surgical techniques and immunosuppression, liver transplant recipients are experiencing increased longevity. Once patients have successfully navigated the perioperative and early posttransplant period, their survival depends on management of immunosuppression to minimize systemic side effects that will contribute to the development of medical comorbidities while avoiding rejection. This includes mitigating cardiovascular risk factors and other components of the metabolic syndrome. The concept of transplant survivorship emphasizes a holistic approach to impr

Efficacy and safety of belatacept in heart transplant recipients.

BACKGROUND: Contemporary immunosuppressive regimens following heart transplantation (HT) are associated with nephrotoxicity. Additionally, some patients develop rejection despite standard immunosuppression, requiring augmentation. Belatacept (BTC) has demonstrated mortality and graft survival benefits while preserving renal function in kidney transplant recipients, but data regarding HT remain limited. This study aimed to evaluate the utilization, safety, and efficacy of BTC-based therapy in HT recipients, comparing outcomes with patients managed under a standard immunosuppression protocol. M

Allogeneic hematopoietic cell transplantation for autoinflammatory disorders.

The understanding of autoinflammatory disorders, which are caused by the dysregulated activation of the innate immune system, has improved with the discovery of new diseases and the expansion of phenotypes. Inflammation can be controlled using immunosuppressive drugs, biological agents, and molecular-targeted therapies. However, some cases remain refractory to treatment, and certain patients experience side effects associated with the long-term use of corticosteroids. Recently, allogeneic hematopoietic cell transplantation (HCT) was reported to improve symptoms in refractory cases. Based on th

Monotherapy Immunosuppression in Pediatric Heart Transplant Recipients.

BACKGROUND: Although dual-drug immunosuppression is standard for rejection prophylaxis in pediatric heart transplant (HT), single-drug immunosuppression (monotherapy) may be appropriate for certain patients. We describe the experience with monotherapy in select patients at a single institution. METHODS: Retrospective review of HT patients at a pediatric transplant center who underwent HT between 1/2001-12/2020 and were treated with calcineurin-inhibitor monotherapy for > 1 month. Patients were transitioned to monotherapy due to concerns for over-immune suppression, including recurrent infecti

Role of immunosuppression in liver transplantation. Advantages and disadvantages.

Cirrhosis of the liver is a major cause of death worldwide, with a recent increase in cases secondary to metabolic dysfunction-associated steatotic liver disease and alcohol use. Liver transplantation is currently the only definitive treatment but requires immunosuppression for preventing graft rejection. Calcineurin inhibitors are the cornerstone of immunosuppression in transplanted patients. However, their use conditions metabolic and renal complications, such as metabolic syndrome, post-transplant diabetes, and chronic kidney disease. Minimizing or discontinuing immunosuppression has been s

Diversity, equity, inclusion, and belonging in organ transplantation.

Diversity, equity, inclusion, and belonging (DEIB) have implications for transplant access and outcomes. Inequities in transplantation have been identified over the years for minoritized groups based on race, ethnicity, sex, sexual orientation, gender identity, disability status, and other sociocultural identities. While DEIB initiatives have demonstrated success in improving transplant outcomes for some minoritized groups, many gaps still exist, and additional work is needed. Concerns about these practices have also been raised, and they may create barriers to achieving DEIB goals. This under

Cryopreservation Strategies to Improve Access to Organ Transplantation.

Organ transplantation is the only definitive cure for end-stage organ failure. The primary obstacle is the scarcity of viable donor organs, resulting in many patients dying on the transplant waitlist. The last decade has brought a range of transformative technologies to address this shortage: machine perfusion has risen as an option to greatly increase the human donor organ utilization, while xenotransplantation further promises to fundamentally alter the calculus in supply constraints. Given this increasing availability, the logistics of allocation rises as the next key technological barrier.

The Allures and the Alarms of the Hair Transplant Tourism Industry.

BACKGROUND: Hair transplant tourism has surged globally, with Turkey, especially Istanbul, becoming the epicenter, generating about US$1 billion annually. This growth is fueled by substantially lower costs and appealing all-inclusive packages, leading patients to prioritize perceived value despite associated risks abroad. OBJECTIVES: This review critically examines the multifaceted international hair transplant industry, using Turkey as a case study. We aim to analyze economic drivers and the perception that high surgical volume equates to expertise, against significant patient safety concern

Cognitive impairment assessments in kidney transplantation: A review.

Mild cognitive impairment (CI) is not an absolute contraindication for kidney transplantation (KT). However, clinical assessment has not been standardized, and several practice challenges remain. We synthesized existing evidence on the effect of CI on adult kidney transplant recipients (KTRs) and KT candidates. Of the 1333 titles and abstracts screened, seven studies were eligible; all were observational. Our synthesis included 1035 KTRs and 4659 patients being evaluated for KT. Studies that used the Montreal Cognitive Assessment (38-55 %) reported a higher CI prevalence than those that used t

Hemophagocytic Lymphohistiocytosis in a Pediatric Lung Transplant Recipient.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare complication of solid organ transplantation and is a syndrome of systemic hyperinflammation secondary to dysregulation of the inflammatory response, primarily involving lymphocytes and macrophages. It is often fatal and therefore early recognition and treatment are crucial. Among 11 adult cases of HLH in post-lung transplant cases found in the literature, only one patient survived. CASE: We report the first known pediatric case of HLH following lung transplantation. The patient, a previously healthy adolescent, developed end-stage
