A 5-year review (1998-2002) was done at the pulmonary unit of the University College Hospital (UCH), Ibadan, Nigeria to determine the pattern of presentation of tuberculosis (TB) and the prevalence of TB and human Immunodefiency virus (HIV) co-infection. A total of one thousand patients were managed for TB but 777 confirmed as having TB had their case files analysed. There were 418 males and 359 females giving a male to female ratio of 1.16:1.00. Pulmonary tuberculosis (PTB) accounted for 78.6% of the patients seen over the period, followed by tuberculosis meningitis 7.8% then TB spine 6.8% an

During the routine care of patients, pulmonary and critical care specialist may have significant exposure to respiratory infections and potentially infectious body fluids such as blood and pleural fluid. Bronchoscopy and intensive care services often require close contact with patients and the use of sharp devices required for intravascular catheterization. Rendering such care places physicians at risk for acquiring agents such as human immunodeficiency virus (HIV), hepatitis viruses, and Mycobacterium tuberculosis. As of December 1998, 188 cases of occupational transmission of HIV to health c

Polymorphism in SLC11A1 has been implicated in host susceptibility to tuberculosis. We have studied associations between INT4, D543N, and 3'UTR polymorphisms of SLC11A1 and different clinical forms of TB. Analysis used 507 patients with pulmonary TB, 123 with extra pulmonary TB and 513 controls. INT4 and D543N showed allelic association with pulmonary TB (P=0.02 and 0.03 respectively). INT4-D543N-3'UTR haplotypes showed an association with pulmonary TB (P=0.03). No association of SLC11A1 with miliary TB was observed, and a possible association of D543N to the pleural form (P=0.08) was suggeste

OBJECTIVE: The purpose of this study was to compare the inflammatory activity of pulmonary lesions between active tuberculosis and nontuberculous mycobacterial (NTM) infection in acid-fast bacilli smear-positive non-HIV-infected patients using gallium-67 (Ga) scintigraphy.
PATIENTS AND METHODS: Twenty-four patients with culture-proven active pulmonary tuberculosis (PTB) and an additional 17 patients meeting the criteria of pulmonary NTM infection were enrolled into the retrospective study. The Ga uptake of the pulmonary lesion was assessed by visual scoring of uptake grade and semiquantitativ

AIM: To compare the Mycobacterium tuberculosis isolates of adult index cases with multidrug-resistant (MDR) tuberculosis to the isolates obtained from their child contacts.
PATIENTS AND METHODS: A 4-year prospective study in the Western Cape Province of South Africa. We evaluated 149 child contacts of 80 adult MDR pulmonary tuberculosis cases. This report includes those cases where a culture for M. tuberculosis was obtained from both the adult source case and the child contact. Isolates were compared by drug susceptibility pattern and restriction fragment length polymorphism analysis.

Adjunctive corticosteroid therapy is recommended for selected human immunodeficiency virus (HIV)-infected patients with presumed Pneumocystis carinii pneumonia. Because corticosteroids may exacerbate undiagnosed tuberculosis, we evaluated the frequency with which tuberculosis in HIV-infected patients mimics P carinii pneumonia. Over a 12-month period, we identified 105 HIV-infected patients with pleuropulmonary tuberculosis and 84 patients with P carinii pneumonia who were sufficiently hypoxemic to warrant corticosteroid therapy. Of the 105 patients with tuberculosis, acid-fast smears of clini

Genitourinary tuberculosis represents a form of extra-pulmonary tuberculosis that occurs in the kidneys, ureters, seminal vesicles, prostate, testis, vas deferens, and epididymis. Isolated testicular involvement is unusual, and differential diagnosis includes testicular tumor, acute infection, infarction, and granulomatous infection. We report a case of a 36-year-old Ecuadorian man residing in New York, New York, who presented with a painful scrotal mass, weight loss, and purulent discharge from ulcerated lesion in scrotal area 10 years following his immigration to the United States. No other

Chest radiographs, computed tomography and gallium scanning are useful in diagnosing the pulmonary manifestations of acquired immunodeficiency syndrome. Most opportunistic infections in patients with AIDS affect the lung as the primary target organ. Bilateral perihilar or basilar interstitial infiltrates, which may progress to the ground-glass appearance of adult respiratory distress syndrome, are commonly seen in cases of Pneumocystis carinii pneumonia. Unilateral or miliary infiltrates and cavitary lesions may be atypical presentations. Diffuse interstitial infiltrates are also seen in mycob

Sera from patients with active tuberculosis were assayed for IgG antibodies to purified protein derivative (PPD) by an enzyme-linked immunosorbent assay (ELISA) technique. Eighty seven of the 112 patients (78%) with sputum culture-positive, and 57 of 78 patients (73%) with sputum culture-negative, pulmonary tuberculosis had positive serology. Seventeen of 19 patients (89%) with renal tuberculosis (demonstrable lesion on intravenous pyelography) had antibody to PPD, whereas only 3 of 9 patients (33%) with urinary tuberculosis (positive urine culture only, without abnormality on intravenous pyel

Parotid tuberculosis is not widely described in the literature. Even though a rare presentation of a commonly occurring disease, it is still missed as a differential diagnosis and often mistaken for neoplasms, eventually leading to unnecessary surgical resections. Here, we present a case of a young lady with a slowly growing unilateral parotid mass along with matted, non-tender cervical, axillary, and inguinal lymphadenopathy, who was incidentally found to have HIV with a cluster of differentiation 4 (CD4) lymphocyte count of 38 per microliter. Fine needle aspiration revealed acid-fast bacilli

UNLABELLED: Tuberculosis (TB) is an infectious disease caused by the Mycobacterium tuberculosis complex (MTBC). Pulmonary TB is the most common form of presentation, but extrapulmonary tuberculosis (EPTB) contributes significantly to morbidity and mortality. Rarely, patients with EPTB develop a form of ankle or foot arthritis. The diagnosis of TB arthritis is often overlooked because of the insidious onset and the non-specific clinical symptoms. Prognosis is generally poor; early diagnosis and delivery of the most appropriate treatment is critical to avoid functional disability.

Miliary tuberculosis is a lethal form of disseminated tuberculosis (TB), deriving its name from the millet-seed-sized granulomas in multiple organs. As TB still remains a leading cause of morbidity and mortality in India, its disseminated forms need to be diagnosed early to ensure more aggressive treatment at the earliest possible time. However, a considerable number of cases are missed ante-mortem. We discuss the case of a 32-year-old immunocompromised, non-HIV patient with an ante-mortem diagnosis of pulmonary TB. However, multiple organ involvement by Mycobacterium tuberculosis was demonstr

Pulmonary tuberculosis is a serious complication of anthracosilicosis and a promoting factor for death. It is very difficult to distinguish the advanced stage of atypical anthracosilicosis from anthracosilicosis associated with tuberculosis by the chest radiograph when the tubercule bacili is negative in sputum, but it is necessary for physician to treat and administrate. In order to probe into the differential diagnosis of these two diseases author made a comparison analysis between the radiographic appearances and the pathological findings of 21 autopsies cases which were clinically diagnose

A series of 31 cases (33 ears) of tuberculous otitis was reviewed. Classical findings of the disease, such as painless ottorhea and multiple perforations of the tympanic membrane, are not consistent with the clinical findings reported here. Severe conductive hearing loss, abundant pale granulations, and an eroded malleus handle occur consistently and appear to be important clinical features of the disease. In all cases suspected of tuberculosis, granulation tissue from the middle ear or mastoid was submitted for bacteriologic and histologic examination. As a result, tuberculous otitis was diag

Mycobacterium tuberculosis primarily causes pulmonary diseases; however, it can also affect extrapulmonary organs, including the eyes. The choroid, as the intraocular tissue with the richest blood supply, is one of the most susceptible structures to intraocular tuberculosis. Abnormal ocular symptoms that occur during systemic anti-tuberculosis treatment (ATT) can easily be misdiagnosed as drug side effects or treatment failure. Abruptly discontinuing a specific medication could lead to the recurrence of extraocular tuberculosis and irreversible vision loss. This case study focuses on a young f

BACKGROUND: Tuberculosis is a global health issue affecting millions of people worldwide. While pulmonary tuberculosis is common, hepatic tuberculosis is rare and accounts for less than 1% of cases. Diagnosis is challenging owing to nonspecific symptoms and its ability to mimic other hepatic diseases.
CASE PRESENTATION: This case report describes a 62-year-old male healthcare worker, originally from the Philippines and of Filipino ethnicity, who was initially suspected to have cystic echinococcosis, an epidemiologically uncommon zoonotic disease in both Austria and the Philippines. Echinococc

Laryngeal tuberculosis is the most frequent granulomatous disease of the larynx and it is prone to be diagnosed as cancer. COVID-19 pandemic caused considerable disruption in tuberculosis service provisions both in the primary care and hospital settings. This report describes a rare case of life-threatening stridor in a patient who presented with an ulceroproliferative laryngeal mass later confirmed as laryngeal tuberculosis. Urgent tracheostomy was performed. The patient's sputum and the computed tomography of the chest revealed a pulmonary, as well as laryngeal tuberculosis. The patient was

In spite of the high prevalence of tuberculosis worldwide, there are few studies on its psychiatric complications. The mental state of 53 patients with pulmonary tuberculosis seen in a Nigerian chest clinic was examined using the 30-item General Health Questionnaire (GHQ-30), the Present State Examination (PSE), and a clinical evaluation based on the International Classification of Disease, tenth edition (ICD-10). Results were compared with two comparison groups: (1) a group of 20 long-stay orthopedic patients with lower limb fractures; and (2) a group of 20 apparently healthy controls. The so

A 44-year-old man was admitted to the hospital because of acute promyelocytic leukemia. A nodular shadow (2.5 cm in diameter) was seen in the right upper lung field on a chest roentgenogram. During the administration of combination chemotherapy for the leukemia, diffuse granular shadow appeared in all lung fields. Transbronchial lung biopsy findings indicated tuberculosis. The patient was given streptomycin, isoniazid, rifampicin and ethambutol, but the nodular show enlarged and a pleural effusion appeared on the right side. Cytologic examination of the pleural effusion revealed adenocarcinoma
