Blau syndrome-the skin as a warning sign.

Blau syndrome is an autosomal dominant chronic inflammatory disease, which may begin with skin manifestations in the first months of life, alerting physicians to the diagnosis. This case reports a patient diagnosed jointly by pediatric dermatology and rheumatology consultants at two years of age.

Etanercept-associated episcleritis: a pediatric case report of a paradoxical adverse reaction and review of the literature.

Scleritis is an inflammation of the episcleral and scleral tissues, characterized by injection in both superficial and deep episcleral vesse … r, has the potential to cause paradoxical adverse reactions including ocular inflammations, such as uveitis, scleritis, and ocular myositis. Etanercept differs in its mechanism of action from other TNF-α inhibitors as it acts as a decoy receptor, and this may partly explain the more frequently reported etanercept-associated ocular inflammation. Etanercept …

Review for Diagnostics of the Year: Inflammatory Choroidal Neovascularization - Imaging Update.

Inflammatory choroidal neovascularization (iCNV) is a rare complication of uveitis but is a major cause of vision compromise in affected patients. Fluorescein angiography (FA) has been the gold standard for diagnosis. However, it is an invasive modality and when used alone, it might be difficult to distinguish iCNV from inflammatory lesions. Optical coherence tomography (OCT) is a noninvasive and rapid imaging modality that can provide additional features to diagnose iCNV. OCT angiography (OCTA) uses intrinsic motion contrast to visualize flow and is useful to distinguish iCNV from inflammator…

Ocular manifestations in IgA nephropathy.

Immunoglobulin A nephropathy (IgAN) is a rare but important systemic disease with or without ocular manifestations. We describe 4 cases of IgAN presenting with scleritis and review the various ocular manifestations in patients with IgAN. We found 55 cases with ocular manifestations in patients with prior or newly-diagnosed IgAN described in 38 publications. The most common ocular manifestations of IgAN were episcleritis (23.6%), scleritis (16.4%), hypertensive retinopathy or retinal vasculopathy (20.0%), and uveitis (14.5%). The median age at presentation was 36.5 years, with 54.5% female pati…

[A case of hydrocephalus during the course of sporadic Blau syndrome].

Blau syndrome (BS) is a rare granulomatous inflammatory disease presenting in early childhood as dermatitis, arthritis, and uveitis. Here, we describe a case of hydrocephalus in a patient with sporadic BS. A 36-year-old female, with mutations in the NOD2 gene on chromosome 16, who had been diagnosed with BS at the age of 19 years, had visual impairment and required support when walking for a long time. She was admitted to our hospital due to deterioration in her walking ability and an inability to stand by herself. We diagnosed an obstructive hydrocephalus based on head MRI. The aqueductal ste…

Dual drug delivery from hydrophobic and hydrophilic intraocular lenses: in-vitro and in-vivo studies.

Posterior capsule opacification (PCO) still remains the most frequent long term complication after cataract surgery, while endophthalmitis i … tter to the posterior capsular bag, while hydrophilic IOLs are advised in the case of patients with uveitis, glaucoma or diabetes. The IOLs were loaded with MXF + KTL and sterilized by high hydrostatic pressure. Both IOLs reduced the tendency for adhesion of LECs. In vivo tests were done to compare the concentration of the drugs in the aqueous humor obtai …

Sympathetic Ophthalmia - a Contribution to Immunology, Clinic and Current Imaging.

BACKGROUND: Sympathetic ophthalmia (SO) is a rare inflammation of an operated or injured eye that spreads to the fellow eye. It is typically a bilateral granulomatous panuveitis. The traumatized eye is referred to as inciting eye and the fellow eye as sympathizing eye. The pathophysiology of the disease is not entirely understood, but there is strong evidence of an autoimmune genesis. PATIENTS/MATERIAL AND METHODS: A selective literature search on epidemiology, immunology, clinical features and risk factors of SO was carried out. In addition, our own experience using multimodal imaging for th…

Ebola virus disease and the eye.

PURPOSE OF REVIEW: The world has witnessed the largest Ebola virus disease (EVD) epidemic in West Africa with an unprecedented number of infected individuals and survivors. Many systemic and ocular complications have been reported in survivors. Herein, we review the ophthalmic complications associated with Ebolavirus. RECENT FINDINGS: EVD causes ocular disease during acute infection and during convalescence. Acute patients manifest with conjunctivitis, subconjunctival hemorrhages, and acute vision loss of unclear cause. The most common complication during recovery is uveitis with live virus i…

The pediatric red eye.

There is a broad differential for the pediatric red eye, which may range from benign conditions to vision- and/or life-threatening conditions. This article presents a systematic differential, red flags for referral, and treatment options.

Ocular problems in early stages of multiple sclerosis.

PURPOSE: In its early course, multiple sclerosis (MS) manifests in a variety of symptoms so that ophthalmologists should know that the ocular problems are often the first sign of the disease and may predict additional demyelinating events. METHOD: The author have reviewed the data in the recent literature about the ocular problems in the early stages of MS. RESULTS: The initial signs of MS are: optic neuritis (acute, unilateral), blurred vision, diplopia or oscillopsia (illusory visual motion) due to oculomotor disorder, chronic bilateral uveitis, facial palsy (similar to an idiopathic Bell'…

Extraintestinal manifestations of Crohn's disease.

In each case of extraintestinal manifestations of Crohn's disease, active disease, if present, should be treated to induce remission, which may positively influence the course of most concomitant extraintestinal manifestations. For some extraintestinal manifestations, however, a specific treatment should be introduced. This latter part of disease management will be discussed in this chapter, in particular for pyoderma gangrenosum, uveitis, spondylarthropathy - axial arthropathy - and primary sclerosing cholangitis, which have also been described in quiescent Crohn's disease. Few new drugs for …

Rifabutin for treating pulmonary tuberculosis.

BACKGROUND: Rifamycins are an essential component of modern short-course regimens for treating tuberculosis. Rifabutin has favourable pharmacokinetic and pharmacodynamic properties and is less prone to drug-drug interactions than rifampicin. It could contribute to shortening of therapy or simplify treatment in HIV-positive people who also need antiretroviral drugs. OBJECTIVES: To compare combination drug regimens containing rifabutin with those containing rifampicin for treating pulmonary tuberculosis SEARCH STRATEGY: We searched Cochrane Infectious Diseases Group Specialized Register (Janua
