Disparities in anticoagulation therapy for older adults with non-valvular atrial fibrillation: a comparative study of urban and suburban communities.

BACKGROUND: Non-valvular atrial fibrillation (NVAF) carries a severe risk of ischemic stroke, making anticoagulation therapy the cornerstone of prophylaxis. However, real-world data on anticoagulation management at the primary care level remain sparse. This study aims to evaluate the current landscape of anticoagulation therapy among older NVAF patients undergoing community health examinations in Shanghai, China, and to analyze the disparities between urban and suburban populations. METHODS: A stratified cross-sectional study was conducted across four community health centers (CHCs)-two urban

Network oscillatory dynamics accompany cerebral bioenergetic defence in hypoxia.

A network physiology framework investigated how coordinated interactions among multiple organ systems collectively support the preservation of cerebral bioenergetic function and better distinguish adaptive from maladaptive responses to hypoxia. Twelve healthy males were passively exposed to 6 h of normoxia (21% O2) and hypoxia (12% O2) in a randomised, single-blind, crossover design. Venous blood was assayed for oxidative-nitrosative stress (OXNOS, spectroscopy/chemiluminescence) and neurovascular unit (hs-ELISA) biomarkers. Global cerebral delivery of O2 and glucose were determined by duplex

Comparison of Heart Rate- and Race Pace-Based Training Prescriptions on Training Execution Accuracy in Recreational Distance Runners.

PURPOSE: While distance running training zones used to prescribe training through the heart rate (HR)-based approach are delimited by HRs associated ventilatory turn points, those in the race pace (RP)-based method are demarcated by percentages of competition average pace at a specific distance. However, no study has directly compared which method better ensures Training Execution Accuracy (TEA) to prescribed intensity zones. This study aimed to compare TEA, quantified as time spent in prescribed intensity zones, between HR- and RP-based training prescriptions during low-intensity continuous (

The type and amount of care provided by older adult caregivers to a spouse with cancer: Results from a mixed methods study.

INTRODUCTION: Spouses of older adults with cancer provide most long-term informal care, and those receiving treatment for cancer rely on this care, but few studies have examined this in Eastern Europe. Therefore, this study aims to understand the type and amount of care provided by older adult caregivers to their spouses during active cancer treatment in Bulgaria, and how patient and caregiver sociodemographic and health variables influence the care provided. MATERIALS AND METHODS: This study used an exploratory mixed-methods convergent parallel research design comprising a cross-sectional su

Modulation of heart rate and heart rate variability during animal-assisted treatment of patients in a minimally conscious state: A randomized controlled crossover study.

BACKGROUND: Animal-assisted treatment (AATx) is a promising and increasingly used approach in neurorehabilitation, yet its psychophysiological effects remain largely unexplored. Patients in a minimally conscious state (MCS) show severely altered consciousness with minimal but definite behavioral signs of awareness. Because behavioral assessment in this population is limited, psychophysiological measures such as heart rate (HR) and heart rate variability (HRV) may offer valuable insights into autonomic regulation during therapy. AIM: The present study investigated whether AATx influences HR an

Cold stimulation applied concurrently with high-frequency electrical stimulation does not attenuate the development of secondary pinprick hypersensitivity in humans.

Diffuse noxious inhibitory controls (DNIC) reduce spinal nociceptive processing in animal models, and the human analogue, conditioned pain modulation (CPM), is thought to engage descending inhibitory pathways. A recent translational study reported that neither cuff-induced DNIC in animals nor cuff-induced CPM in humans interferes with high-frequency electrical stimulation (HFS)-induced spinal sensitization in animals or secondary pinprick hypersensitivity in humans. In contrast, another human study, using cold-water immersion to induce CPM, observed a substantial reduction in low-frequency sti

Detection of balance in the elderly under the influence of stress (DEPIE): A cross-sectional study protocol.

Age-related changes increase frailty and vulnerability to stress in older adults, and stress has been linked to poorer balance and fall risk. However, the mechanisms by which emotional stress may contribute to falls remain unclear. This study explores whether: 1) emotional stressors lead to neuromuscular changes that affect postural control in older adults; and 2) technology can assist in fall prevention by detecting increased risk. A cross-sectional, laboratory-based observational study is being conducted in a single session comparing 30 young adults (18-39 years) and 30 older adults (≥65 yea

A First Look at Shifts in Community-Entry Home Health Following Medicare Payment Reform.

OBJECTIVES: Medicare-funded home health (HH) provides short-term nursing, physical therapy, and other services to over 3.5 million older adults each year. Currently, half of HH episodes are "community-entry," meaning the patient was referred without an immediately preceding hospitalization. The 2020 implementation of a new payment system-the Patient-Driven Groupings Model (PDGM)-reduced traditional Medicare reimbursement for community-entry HH (CEHH). We investigated shifts in CEHH care delivery following PDGM implementation. DESIGN: Cross-sectional study of national 2019 and 2021 linked HH c

Response to the letter to editor for "concerns regarding the three-time-point sampling approach for diurnal transcriptome analysis in adipose tissue: evidence from fasting-independent time-of-day effects".

We respond to methodological concerns regarding the use of three daytime adipose tissue biopsies to estimate diurnal transcriptomic patterns. We clarify that the Stenvers et al. dataset was used for a technical evaluation of reduced temporal sampling, not as a biological replication of our dietary intervention. Additional analyses in healthy controls and participants with type 2 diabetes showed that rhythmic gene recovery was incomplete, particularly for weak oscillations. However, amplitude and acrophase estimates remained highly concordant among genes classified as rhythmic in both analyses.

Next generation spray-dried on-demand plasma: A multicenter, randomized, dose-escalation, first-in-human safety study.

BACKGROUND: Prehospital plasma transfusion for patients at risk of hemorrhagic shock reduces mortality, but cold-chain and logistical constraints limit product availability. This study reports the first-in-human clinical evaluation of a new spray-dried plasma product, Frontline On-Demand Plasma (ODP). METHODS: Twenty-four healthy volunteers were sequentially enrolled into three dose-escalation cohorts. Autologous apheresis plasma was processed into plasma frozen within 24 h (PF24), with half of each collection manufactured into ODP. Cohort 1 (n = 6) received 200 mL ODP; Cohort 2 (n = 6) recei
