Electrical impedance tomography in neonatal respiratory diseases: a clinical review of applications and evidence.

Neonatal respiratory diseases remain a leading cause of morbidity and mortality in intensive care settings, with management complicated by the limitations of current monitoring tools. Chest radiography provides only static snapshots and cannot capture the dynamic evolution of lung aeration, while lung ultrasound, though portable, remains operator-dependent and incapable of quantifying regional ventilation. Electrical Impedance Tomography (EIT) has emerged as a noninvasive, radiation free imaging modality that addresses these gaps by enabling real time, continuous visualization of regional lung

Three Years of Bidirectional, Home-Based Opioid Route Management in Advanced Cervical Cancer with Coexisting Causes of Route Failure: A Case Report.

Oral administration is preferred for cancer pain, and when it fails the switch to a parenteral or transdermal route is usually made once and in one direction. We describe a woman with recurrent cervical cancer managed at home for approximately three years in whom several causes of route failure coexisted-short bowel syndrome with a stoma (impairing enteral absorption of sustained-release opioids), profound cachexia (impairing subcutaneous absorption), and recurrent indwelling-device infections (threatening vascular access)-so that the opioid route had to be changed repeatedly and in both direc

Rewiring cell death and evading repair: Evolution of temozolomide and emerging strategies against resistant glioblastoma.

Glioblastoma (GBM) is the most common and aggressive primary malignant brain tumor, characterized by a universally dismal prognosis. For over two decades, the alkylating agent temozolomide (TMZ) has remained the cornerstone of the standard-of-care "Stupp protocol" largely owing to its unique pH-dependent activation and favorable blood-brain barrier penetration. However, the clinical efficacy of TMZ is profoundly compromised by complex intrinsic and acquired resistance mechanisms. These are primarily driven by O6-methylguanine-DNA-methyltransferase (MGMT) overexpression, mismatch repair (MMR) d

[Bottlenecks and breakthroughs in gastric cancer diagnosis and treatment: Towards a new era of precision and intelligent integration].

Gastric cancer constitutes a significant global health burden, and its clinical management is undergoing a critical transition from a traditional experience-driven paradigm toward the deep integration of precision medicine and artificial intelligence (AI). At present, the main bottleneck has shifted from a lack of therapeutic options to insufficient capacity for precise clinical decision-making. In recent years, diagnostic approaches have seen marked advances through the application of AI-augmented endoscopy, radiomics, molecular subtyping, and liquid biopsy, reflecting progress in both precis

From Pandemic Response to Kill the Clipboard: Patient-Controlled Sharing of Health Data Using International Patient Summary (IPS) and QR codes.

BACKGROUND: Despite advances in interoperability, patient-supplied medical histories commonly use paper-based forms. Dissatisfaction with this inefficiency has led to the "Kill the Clipboard" initiative from the Centers for Medicare & Medicaid Services to advance digital alternatives. During the coronavirus disease 2019 (COVID-19) pandemic, patient-controlled vaccination record sharing demonstrated that Quick Response (QR) codes and digital data standards provide an effective option. Building on this, several nations are using three standards to securely view and exchange medical data: Fast He

Diabetic Ketoacidosis Unveiling Severe Hypertriglyceridemia and Acute Mild Pancreatitis: A Rare Initial Manifestation of Type 2 Diabetes Mellitus.

BACKGROUND Diabetic ketoacidosis (DKA) in combination with hypertriglyceridemia-induced acute mild pancreatitis is a rare but life-threatening clinical triad that occurs in a small subset of patients with DKA. It is an exceptionally uncommon initial presentation of type 2 diabetes mellitus in adults. This case highlights the importance of early recognition and multidisciplinary intervention in such complex clinical scenarios. CASE REPORT A 35-year-old man with no known history of diabetes mellitus presented with 3 days of epigastric abdominal pain. Initial laboratory tests confirmed DKA and se

Food-based enteral formulas in pediatric clinical practice: an expert opinion.

INTRODUCTION: Standard enteral formulas are widely used in pediatric practice but may be associated with gastrointestinal intolerance, limited food diversity, and caregiver dissatisfaction. Growing interest in whole-food feeding has led to increased use of blenderized tube feeds and food-based enteral formulas (FBEFs), although definitions and implementation strategies remain heterogeneous. AREAS COVERED: This expert opinion review summarizes evidence and practice-based insights from an international, multidisciplinary panel of physicians and dieticians on the use of FBEF in pediatric clinica

Supplemental Security Income for Children, Adolescents, and Young Adults With Disabilities and the Role of the Pediatrician: Policy Statement.

The Supplemental Security Income (SSI) program remains an important source of financial support for low-income families of children, adolescents, and young adults with disabilities. In most states, SSI eligibility also qualifies children for the state Medicaid program, providing access to health care services. The Social Security Administration (SSA), which administers the SSI program, considers a child disabled under SSI if there is a medically determinable physical or mental impairment or combination of impairments that results in marked and severe functional limitations. The impairment(s) m

A Quercetin Nanocarrier-Loaded Dual Network Injectable Hydrogel for Mesenchymal Stem Cells (MSCs) Delivery Targeting Osteoarthritis.

Osteoarthritis (OA) is a progressive, chronic disorder of the musculoskeletal system affecting more than 500 million individuals globally. Current treatment strategies primarily provide palliative care, with limited potential to alter disease progression or reverse tissue deterioration. Stem cell transplantation holds promising results. But poor cell retention and survival due to ROS and inflammation in OA lead to subpar therapeutic outcomes. In this study, we developed a dual-network gelatin methacrylate (GelMA) and κ-carrageenan-based injectable hydrogel loaded with antioxidant quercetin-PLG

Predictive fetal medicine and the ownership of prenatal data: legal, ethical, and professional challenges.

Advances in artificial intelligence and multi-omic analysis are transforming fetal medicine from a diagnostic discipline into a predictive one. Yet the legal, deontological, and ethical frameworks that govern prenatal and fetal data have not evolved accordingly. Current regulations protect the mother as a patient but do not recognize the fetus-or the future child-as a legal data subject. As a result, information generated before birth remains confined within maternal medical records, creating uncertainty about who may later access or reuse it. This paper examines the emerging ethical and legal

Genetic Testing in Inherited Retinal Disease: Current Strategies and Future Directions.

Inherited retinal diseases (IRDs) are a major cause of visual impairment worldwide, marked by extensive genetic and phenotypic heterogeneity. Recent estimates from the U.S. suggest a prevalence of nearly 1 in 1000 individuals, reflecting both disease burden and improved diagnostic recognition. This review traces the shift from linkage analysis and Sanger sequencing to high-throughput next-generation sequencing, including panel-based, whole-exome, and whole-genome sequencing. Phenotype-driven testing strategies and standardized variant interpretation frameworks, such as the American College of

Targeting BCMA in Plasmablastic Lymphoma With Teclistamab: A Case Study of Three Patients.

Plasmablastic lymphoma (PBL) is a rare subtype of non-Hodgkin lymphoma (NHL) with limited standard therapeutic options and poor outcomes. PBL typically lacks expression of the B-cell antigens CD20 or CD19 and, accordingly, has not benefited from the incorporation of B-cell-targeted antibodies. However, recent reports demonstrate that B-cell maturation antigen (BCMA) is expressed in most patients with PBL. BCMA is an established therapeutic target in multiple myeloma. Herein, we describe the clinical course of 3 patients with PBL found to express BCMA by immunohistochemistry who were treated wi

[From Goal-Directed Therapy to Personalized Hemodynamic Management].

The article describes the transition from traditional goal-directed therapy (GDT) to personalized hemodynamic management in the perioperative setting. The primary objective of hemodynamic optimization is to ensure adequate tissue oxygenation, determined by arterial oxygen content and cardiac output. The latter is mainly influenced by preload, afterload, and myocardial contractility. Static preload parameters such as central venous pressure have limited predictive value for fluid responsiveness, whereas dynamic indices (SVV, PPV) may provide superior guidance under specific conditions.Large ran

Hallmarks of Weaning From Mechanical Ventilation: A 3S Framework for Ventilator Liberation-A Narrative Review.

Safe and long-lasting separation from invasive mechanical ventilation is a critical aspect of care provision in the ICU, yet it remains a complex and heterogeneously applied process. Current weaning strategies often rely on fragmented assessments, personal judgment, and poorly integrated decision milestones that individually and collectively may contribute to delayed attempts, weaning, and extubation failure. In this How I Do It article, we propose a comprehensive framework based on 7 hallmarks, organized into 3 sequential and interrelated phases (3S framework): screening, separating, and secu
