Cardiac remodelling in novel cluster-based metabolic (dysfunction)-associated fatty liver disease: a cross-sectional study in China.

OBJECTIVES: To identify novel, data-driven phenotypic clusters of metabolic dysfunction-associated fatty liver disease (MAFLD) and investigate their associations with cardiac remodelling. DESIGN: Cross-sectional study. SETTING: Secondary care; a single-centre study in China. PARTICIPANTS: A total of 3233 participants diagnosed with MAFLD were included in the study. The diagnosis was conducted in accordance with the established criteria for MAFLD. The exclusion criteria encompassed a history of significant cardiovascular or hepatic diseases, as well as excessive alcohol consumption. PRIMARY

Reduced cardiac extraction of norepinephrine and epinephrine in patients with heart failure--correlation with left ventricular function.

To assess whether the impairment of neuronal norepinephrine (NE) uptake is involved in the increased NE release observed in the failing heart, we examined the cardiac extractions of NE and epinephrine (E) and their correlation with left ventricular function in 16 patients with anterior transmural old myocardial infarction (OMI) and 18 patients with dilated cardiomyopathy (DCM). The plasma NE and E levels were both increased in OMI and DCM, particularly in the coronary sinus, as compared with those in 16 control subjects (Control). The cardiac NE and E extractions were significantly reduced in

Comparison between ivabradine and low-dose digoxin in the therapy of diastolic heart failure with preserved left ventricular systolic function.

Multicenter trials have demonstrated that in patients with sinus rhythm ivabradine is effective in the therapy of ischemic heart disease and of impaired left ventricular systolic function. Ivabradine is ineffective in atrial fibrillation. Many patients with symptomatic heart failure have diastolic dysfunction with preserved left ventricular systolic function, and many have asymptomatic paroxysmal atrial fibrillation. Ivabradine is not indicated in these conditions, but it happens that it is erroneously used. Digoxin is now considered an outdated and potentially dangerous drug and while effecti

Carvedilol versus flecainide treatment in idiopathic ventricular extrasystoles and tachycardias (CARFLECT IV) trial: Rationale and design.

INTRODUCTION AND OBJECTIVES: Idiopathic symptomatic non-sustained ventricular arrhythmias (INVA), including premature ventricular contractions and non-sustained ventricular tachycardias, are generally considered benign. Symptoms and left ventricular dysfunction are indications for treatment with antiarrhythmic drugs. The SCN5A gene encodes part of the voltage-dependent sodium channel type V alpha, which is the primary target of flecainide. The H558R polymorphism of this gene has been associated with a better response to flecainide in atrial fibrillation. The aim of this study is to describe th

Diastolic dysfunction in normotensive men with well-controlled type 2 diabetes: importance of maneuvers in echocardiographic screening for preclinical diabetic cardiomyopathy.

OBJECTIVE: Because a pseudonormal pattern of ventricular filling has never been considered in studies that reported a prevalence of left ventricular diastolic dysfunction (LVDD) between 20 and 40%, our aim was to more completely evaluate the prevalence of LVDD in subjects with diabetes. RESEARCH DESIGN AND METHODS: We studied 46 men with type 2 diabetes who were aged 38-67 years; without evidence of diabetic complications, hypertension, coronary artery disease, congestive heart failure, or thyroid or overt renal disease; and with a maximal treadmill exercise test showing no ischemia. LVDD was

Prediction of early myocardial damage in obstructive sleep apnea patients using combined logistic regression and QUEST decision tree models.

Obstructive sleep apnea (OSA) is linked to cardiovascular complications, including myocardial dysfunction, yet early detection remains difficult. This retrospective study aimed to develop a combined logistic regression and QUEST decision tree model to predict early myocardial dysfunction in OSA patients. Echocardiography left ventricular global longitudinal strain (LVGLS) and right ventricular free wall longitudinal strain (RVFWLS) were used to assess myocardial function in OSA patients. Predictive models were constructed using clinical parameters. External validation involved 100 OSA patients

Dilated cardiomyopathy: causes, mechanisms, and current and future treatment approaches.

Dilated cardiomyopathy is conventionally defined as the presence of left ventricular or biventricular dilatation or systolic dysfunction in the absence of abnormal loading conditions (eg, primary valve disease) or significant coronary artery disease sufficient to cause ventricular remodelling. This definition has been recognised as overly restrictive, as left ventricular hypokinesis without dilation could be the initial presentation of dilated cardiomyopathy. The causes of dilated cardiomyopathy comprise genetic (primary dilated cardiomyopathy) or acquired factors (secondary dilated cardiomyop

Diagnosis and management of cardiac sarcoidosis.

Cardiac sarcoidosis is an underdiagnosed disease that may be present in as many as 25% of patients with systemic sarcoidosis. Although most commonly recognized in patients with other manifestations of sarcoidosis, it may occur in isolation and its presence is often not appreciated. Cardiac sarcoidosis may present as asymptomatic left ventricular dysfunction, congestive heart failure, atrioventricular block, atrial or ventricular arrhythmia and sudden death. Although untested in clinical trials, early use of high-dose steroid therapy may halt or reverse cardiac damage. This article reviews the

Supraventricular arrhythmias limit effective cardiac resynchronization therapy: diagnosis using intracardiac electrograms and device based pacing maneuvers.

Cardiac resynchronization therapy is an effective tool for the treatment of drug-refractory heart failure in patients with left ventricular dysfunction and inter/intra ventricular conduction delay. Supraventricular tachycardias may prevent effect delivery of this therapy. We report three cases in which effective therapy was limited by asymptomatic supraventricular tachycardia. Diagnostic pacing maneuvers were performed via the implanted device to determine the underlying arrhythmia mechanism. These cases highlight the importance of (1) treating supraventricular tachycardias before and after im

Myocardial deformation and rotational profiles in mitral valve prolapse.

We studied whether evaluation of overall left ventricular (LV) and left atrial (LA) mechanics would be useful to detect subclinical dysfunction in patients with mitral valve prolapse (MVP), mitral regurgitation (MR), and normal LV ejection fraction (EF). Fifty consecutive patients (27 men, mean age 61 ± 19 years) with MVP, MR, and normal systolic function (LVEF ≥60%) were prospectively enrolled and compared with 40 age- and gender-matched healthy subjects (22 men, mean age: 59 ± 16 years). At baseline, 2-dimensional and color-flow Doppler transthoracic echocardiography were performed for MR qu

Diastolic dysfunction evaluated by cardiac magnetic resonance: the value of the combined assessment of atrial and ventricular function.

OBJECTIVES: We sought to evaluate the role of cardiac magnetic resonance imaging (CMR) in the evaluation of diastolic function by a combined assessment of left ventricular (LV) and left atrial (LA) function in a cohort of subjects with various degrees of diastolic dysfunction (DD) detected by echocardiography. METHODS: Forty patients with different stages of DD and 18 healthy controls underwent CMR. Short-axis cine steady-state free precession images covering the entire LA and LV were acquired. Parameters of diastolic function were measured by the analysis of the LV and LA volume/time (V/t) c

Transesophageal echocardiography-guided anesthetic management of a patient with dilated cardiomyopathy, severe heart failure, and septic shock: a case report.

Perioperative management of severe dilated cardiomyopathy (DCM) with markedly reduced left ventricular ejection fraction (LVEF) is particularly complex, especially when complicated by septic shock, as standard resuscitative measures may trigger acute cardiac decompensation. This report describes a 41-year-old male with undiagnosed severe DCM (LVEF 16% by transthoracic echocardiography) and left ventricular thrombus, who presented with Aeromonas hydrophila-induced necrotizing fasciitis that rapidly progressed to septic shock and multiple organ dysfunction. The severity of septic manifestations

Effects of cardiac growth on electrical dyssynchrony in the single ventricle patient.

Single ventricle patients, including those with hypoplastic left heart syndrome (HLHS), typically undergo three palliative heart surgeries culminating in the Fontan procedure. HLHS is associated with high rates of morbidity and mortality, and many patients develop arrhythmias, electrical dyssynchrony, and eventually ventricular failure. However, the correlation between ventricular enlargement and electrical dysfunction in HLHS physiology remains poorly understood. Here we characterize the relationship between growth and electrophysiology in HLHS using computational modeling. We integrate a per

Resveratrol ameliorates myocardial injury and inflammatory response and cell apoptosis in Kawasaki disease rats by activating the Nrf2/HO-1 pathway.

BACKGROUND: Kawasaki disease (KD) is an acute, self-limiting systemic vasculitis of unknown etiology, which often involves the coronary arteries, leading to severe complications such as myocardial injury (MI). It aimed to explore the mechanism of action of Resveratrol (RES) on MI induced by KD in rats based on the Nrf2/HO-1 pathway.  METHODS: Thirty-six rats were randomly grouped: control, model, RES intervention, Nrf2 inhibitor + RES intervention groups (CG, MG, RG, IRG). The KD rat model was established. After modeling, RES was administered to the RG via intraperitoneal injection, while ster

Cardiovascular failure and weaning.

Cardiac patients are at high risk of weaning failure due to the abrupt burden to the cardiovascular system resulting from the transition from positive-pressure ventilation to spontaneous breathing. Similarly, numerous patients with borderline cardiac function, left ventricular diastolic dysfunction, chronic obstructive pulmonary disease, especially with associated fluid overload or cumulative positive fluid balance, are at high risk of weaning failure of cardiac origin. The diagnosis of weaning-induced pulmonary oedema (WiPO) relies on the measurement of elevated left ventricular filling press

Safety and effectiveness of combined percutaneous coronary angioplasty and aortic valvuloplasty in an elderly patient with cardiogenic shock: effect on concomitant severe mitral regurgitation.

An 80-year-old diabetic man with severe aortic stenosis was admitted to our hospital for cardiogenic shock complicating non-ST-elevation myocardial infarction. Echocardiographic evaluation showed also a severe degree of both left ventricular dysfunction and mitral regurgitation. The patient was initially stabilized with inotropes and mechanical ventilation was necessary because of concurrent pulmonary edema. The day after, he was submitted to coronary angiography showing bivessel coronary disease. Given the high estimated operative risk, the patient was treated with angioplasty and bare metal

Prospective validation of the prognostic usefulness of B-type natriuretic peptide in asymptomatic patients with chronic severe aortic regurgitation.

OBJECTIVES: The purpose of this study was to determine the independent and additive prognostic value of B-type natriuretic peptide (BNP) in patients with severe asymptomatic aortic regurgitation and normal left ventricular function. BACKGROUND: Early surgery could be advisable in selected patients with chronic severe aortic regurgitation, but there are no uniform criteria to identify candidates who could benefit from this strategy. Assessment of BNP has not been studied for this purpose. METHODS: We prospectively evaluated 294 consecutive patients with severe asymptomatic organic aortic regu
