Unveiling the benefits of Vitamin D3 with SGLT-2 inhibitors for hypertensive obese obstructive sleep apnea patients.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with poorer quality of life (QoL) and increased cardiovascular risks, which may be exacerbated by hypovitaminosis D. Sodium glucose transporter-2 inhibitor (SGLT2i) provides cardiovascular benefits beyond glycemic control. As vitamin D3 and SGLT2i act through different pathways with similar mechanisms in improving cardio-metabolic health, this study aimed to investigate the synergistic effects of this combination therapy in improving these parameters and QoL in hypertensive obese OSA patients. METHODS: Patients who fulfilled the study cr…

Detection and Severity Classification of Sleep Apnea Using Continuous Wearable SpO2 Signals: A Multi-Scale Feature Approach.

The use of wearable devices for sleep apnea detection is growing, but their limited signal resolution poses challenges for accurate diagnosis. This study explores the feasibility of using SpO2 signals from wearable sensors for detecting sleep apnea and classifying its severity. We propose a novel multi-scale feature engineering approach, which extracts features from coarsely grained SpO2 signals across timescales ranging from 1 s to 600 s. Our results show that traditional SpO2 markers, such as the oxygen desaturation index (ODI) and Lempel-Zip complexity, lose their relevance with the Apnea-H…

Predictive biomarkers of adequate response to continuous positive airway pressure treatment in sleep apnea: targeting cardiovascular outcomes.

INTRODUCTION: The management of obstructive sleep apnea (OSA) based solely on a single metric, the apnea-hypopnea index, constitutes a reductionist approach and has a significant limitation, as it does not adequately capture the multifactorial pathophysiology, phenotypic variability, and clinical heterogeneity of the disease. AREAS COVERED: The heterogeneity observed in clinical presentations and treatment responses suggests the potential existence of distinct subpopulations in this condition. OSA has been linked to an increased cardiovascular (CV) risk, with continuous positive airway pressu…

Skin- and hair- inspired nanofibrious hydrogel composite as multimodal sensor for polysomnographic monitoring in sleep apnea syndrome.

Flexible sensors, particularly biomimetic hair-like sensors, have recently garnered considerable interest for the real-time monitoring of sleep apnea syndrome (SAS) due to their exceptional sensitivity in detecting subtle respiratory airflow. However, most existing designs are limited to structural emulation of hair with single-signal responses, while neglecting the critical role of skin-hair functional integration in biological mechanosensation. Herein, we address this limitation by developing skin-hair mimetic sensor based on alkaline-treated polyacrylonitrile/ionic liquid/electrostatic floc…

A Possible Missing Link Between Obstructive Sleep Apnea Syndrome (OSA) Associated with Tobacco Use and Inflammation Biomarkers.

Background: Obstructive sleep apnea (OSA) is increasingly recognized as a chronic condition associated with systemic low-grade inflammation. Elevated levels of inflammatory markers such as C-reactive protein (CRP), fibrinogen, TNF-α, and IL-6 have been observed in OSA patients, independent of obesity. Tobacco use, a known pro-inflammatory factor, may further exacerbate this burden. This study aimed to evaluate whether smoking influences inflammatory markers and OSA severity in newly diagnosed patients. Methods: We conducted a retrospective, observational study on individuals newly diagnosed wi…

Effects of Hook Maneuver on Oxygen Saturation Recovery After -40 m Apnea Dive-A Randomized Crossover Trial.

To reduce the risk of syncope, trained breath-hold divers (BHDs) use a specialized breathing technique after surfacing called "hook breathing" (HB). It consists of a full inspiration followed by a Valsalva-like maneuver and with subsequent exhalation performed against resistance to generate continuous positive airway pressure during exhalation. This study analyzed the influence of HB on oxygen saturation recovery after a -40 m depth apnea dive in trained BHDs. Thirteen BHDs performed two dives to -40 m at different days, one followed by HB after a dive and the other using usual breathing (UB).…
