BACKGROUND: Bacterial cell wall (CW) arthritis develops in susceptible strains of rats after a single intraperitoneal injection of the CW from certain bacterial species, both pathogenic and non-pathogenic. For the development of chronic bacterial CW arthritis, the structure of the bacterial peptidoglycan (PG) has been found to be decisive.
OBJECTIVE: To define the role of PG subtypes in the pathogenesis of chronic bacterial CW arthritis.
METHOD: Arthritis was induced with CWs of Lactobacillus plantarum, L casei B, L casei C, and L fermentum. Gas chromatography-mass spectrometry was used to m

BACKGROUND: Serious adverse events may occur from the use of disease modifying antirheumatic drugs (DMARDs) used to treat rheumatoid arthritis. We describe preliminary data from a regional surveillance scheme. Our aims were to identify a broad range of potential adverse events, to identify deficiencies in care and examine the management of common events in order to improve care.
METHODS: Adverse events were sought by regular postcards to clinicians in the West Midlands region of the UK. Each reported case was carefully described and the opinions of at least three peer-reviewers were sought on

UNLABELLED: The aim of the study was to evaluate relationship between clinical picture of Lyme disease and risk of Borrelia burgdorferi infection, expressed as an exposure to ticks in an endemic area of the disease.
METHODS: Questionnaires regarding frequency of tick bites, symptoms of the disease in an acute phase and symptoms likely associated with previous infection were collected from 131 residents of Białowieza village, who suffered from Lyme disease in the past.
RESULTS: Lower prevalence of erythema migrans and significantly higher prevalence of arthritis was demonstrated in the group

The condition of septic arthritis was treated in 12 foals with 21 affected joints (Group I) and in 27 adult horses. The adult horses were divided into three groups, based on aetiology of the condition: haematogenous (Group II, n = 6), iatrogenic (Group III, n = 6), and perforating trauma (Group IV, n = 15). The treatment consisted of an initial systemic antibiotic that anticipated the microbial agents that were considered most likely per group, repeated through-and-through joint lavages every other day and non-steroidal anti-inflammatory drugs. The antibiotics were adjusted to the results of b

One of the most important prognostic factors in patients with musculoskeletal infections is the delay in establishing therapy. Early diagnosis of septic arthritis requires analysis of joint fluid. Ultrasonography (US) is a rapid, portable, sensitive technique for confirming the presence of joint effusions. The study can be easily repeated for follow-up of lesions. US allows real-time guidance of fluid aspiration and can reduce the risk of contaminating other anatomic compartments, especially in the hands, wrists, and feet. Radiography provides complementary information and should be performed

Pinhole magnification scintigraphy is an effective means of evaluating the pediatric skeleton because it provides optimal high-resolution images. This technique is indicated when diagnostic uncertainty persists after high-resolution imaging with parallel hole collimation. Pinhole magnification scintigraphy requires approximately 20 minutes of acquisition time per image and meticulous attention to details such as choice of pinhole insert, collimator positioning, and patient immobilization. However, the technique is superior to planar imaging in demonstrating acute osteomyelitis in bone adjacent

The clinical features, diagnosis and treatment of haematogenous multifocal osteomyelitis, a rare illness, are described. Of clinical note are the pain, pseudoparalyses and restriction of movement. Staphylococcus aureus was the pathogen in both of the cases we described. Sonography is a useful diagnostic tool for the localization of subperiosteal abscesses; scintigraphy is a supplementary examination for cases where it is difficult to establish a diagnosis. MR-tomography is the most reliable method for examination of proximal infected foci (spondylitis, pelvis osteomyelitis). In our view, the e

Arthritis and carditis were mildly improved upon adoptive transfer of T cell enriched lymphocyte populations from Borrelia burgdorferi-infected (B. burgdorferi) (immune) compared with naive immunocompetent mice into B. burgdorferi-infected, severe combined immunodeficient (SCID) mice. Despite the relative purity of T cells in transferred cells, recipient mice seroconverted to B. burgdorferi. Thus, the effect could not be attributed to T cells alone. Passive transfer of serum from actively infected immunocompetent mice (immune serum) to SCID mice at the time of or before B. burgdorferi inoculat

We report a case of B-cell chronic lymphatic leukaemia (B-CLL) complicated by constrictive pericarditis. The pericardial involvement was confirmed histologically to be leukaemic in nature. We draw attention to this complication which is amenable to surgical correction. To our knowledge this has been described only once before as an autopsy finding and has not been encountered ante-mortem.

We describe a 53-year-old alcoholic man who presented with hip septic arthritis due to Bacteroides fragilis. This arthritis involved a severe destruction of the femoral head, which was completely devitalized. Recovery was achieved after 4 months of antimicrobial therapy with imipenem/cilastatin plus metronidazole, surgical debridement of the necrotic tissues and four sessions of hyperbaric oxygen.

The examination of 231 rheumatic patients, 159 patients with reactive arthritis and 117 rheumatoid arthritis patients identified diagnostic titers of polyvalent antibodies to Borrelia in 6.6% of all the examinees with rheumatism, 11.9% with reactive arthritis and in 7.7% with RA. Lyme arthritis was diagnosed in 1.3% of reactive arthritis patients. To diagnose Lyme arthritis accurately, it is recommended to rest upon clinical symptoms which must be associated with high titers of IgG antibodies to Borrelia growing still higher in the follow-up period.

Physicians are familiar with the "slapped cheek" rash seen in childhood cases of erythema infectiosum. Less well known is that infection with its pathogen, parvovirus B19, often becomes manifest in adults as acute viral arthropathy. In evaluation of suspected cases, a complete blood cell count and blood chemistry profile are useful additions to thorough history taking and physical examination. Differential diagnosis includes Lyme disease, other viral infections, acute rheumatoid arthritis, systemic lupus erythematosus, and psoriatic arthritis. Treatment is symptomatic with nonsteroidal anti-in

To evaluate the efficacy and safety of teicoplanin in the treatment of gram-positive bone and joint infections, parenteral teicoplanin was used once daily in an average dose of 10.1 mg/kg (range 6 to 12 mg/kg) after loading to treat patients with acute or chronic osteomyelitis and septic arthritis. From the 66 patients who met criteria for initial enrollment, 60 gram-positive isolates were recovered from bone and joint fluid, and each was sensitive to teicoplanin at < or = 2 micrograms/mL (median 0.25 microgram/mL). Teicoplanin was more active in vitro against these isolates than either vancom

To improve the appropriateness and efficiency of diagnostic serological tests and subsequent antibiotic treatment, clinical data from 102 patients with unclassified arthritis were analysed to investigate whether the presence of positive IgG antibodies to Borrelia burgdorferi could be predicted. The clinical data were blindly ranked from 1 to 4 (1, Lyme arthritis unlikely; 4, Lyme arthritis very likely). Antibodies to B burgdorferi were positive in nine of 102 patients (9%). Six of 15 (40%) patients with rank numbers 3 and 4 were positive for antibodies to B burgdorferi, in contrast with only t

Teicoplanin is a new glycopeptide antibiotic with activity against Gram-positive bacteria, including methicillin-resistant organisms. Teicoplanin is administered once daily, either intravenously or intramuscularly. Teicoplanin was given once daily, intravenously or intramuscularly, in the treatment of hospitalized or ambulatory patients with Gram-positive bone or joint infections. A total of 90/98 patients were evaluated for efficacy; 41 had acute osteomyelitis, 41 had chronic osteomyelitis, and 8 had septic arthritis. At the end of therapy, 37 acute osteomyelitis patients were cured/improved

Cloned T lymphocytes reactive with Borrelia burgdorferi proteins were isolated from a patient with chronic Lyme arthritis. All of the T cell clones which proliferated in response to Borrelia proteins were CD3 + CD4 + CD8 - TCR alpha beta + and HLA-DR restricted. One T cell clone (GN30) exhibited HLA-DR-restricted cytotoxic activity against antigen-presenting cells pulsed with Borrelia antigen. In response to Borrelia antigen, the T cell clones produced TNF-alpha, INF-gamma, and GM-CSF. There are at least three distinct spirochetal proteins recognized by the four T cell clones analyzed.
