An Integrative Approach for Subtyping Mental Disorders Using Multi-modality Data.

Mental disorders exhibit significant heterogeneity, posing challenges for accurate subtyping and diagnosis. Traditional clustering methods do not integrate multi-modal data, limiting their clinical applicability. This study introduces the Mixed INtegrative Data Subtyping (MINDS) method, a Bayesian hierarchical joint model designed to identify subtypes of Attention-Deficit/Hyperactivity Disorder (ADHD) and Obsessive-Compulsive Disorder (OCD) in adolescents using multi-modality data from the Adolescent Brain Cognitive Development (ABCD) Study. MINDS integrates clinical assessments and neuro-cogn

The role of maladaptive personality domains across multiple levels of the HiTOP structure.

This study aimed to examine associations between maladaptive personality traits and psychopathology from the perspective of the hierarchical taxonomy of psychopathology (HiTOP). We tested hierarchical structural models to further validate a portion of the structural components of HiTOP. We also tested a priori personality and psychopathology associations with three levels of the HiTOP hierarchy: general psychopathology, spectra and syndromes/disorders. We used a large sample from the general Australian population who completed a large set of personality and psychopathology inventories online.

Non-invasive Brain Stimulation for the Rehabilitation of Children and Adolescents With Neurodevelopmental Disorders: A Systematic Review.

In the last years, there has been a growing interest in the application of different non-invasive brain stimulation techniques to induce neuroplasticity and to modulate cognition and behavior in adults. Very recently, different attempts have been made to induce functional plastic changes also in pediatric populations. Importantly, not only sensorimotor processing, but also higher-level functions have been addressed, with the aim to boost rehabilitation in different neurodevelopmental disorders. However, efficacy and safety of using these techniques in pediatric population is still debated. The

Explaining the sex difference in dyslexia.

BACKGROUND: Males are diagnosed with dyslexia more frequently than females, even in epidemiological samples. This may be explained by greater variance in males' reading performance. METHODS: We expand on previous research by rigorously testing the variance difference theory, and testing for mediation of the sex difference by cognitive correlates. We developed an analytic framework that can be applied to group differences in any psychiatric disorder. RESULTS: Males' overrepresentation in the low performance tail of the reading distribution was accounted for by mean and variance differences ac

Gestational overgrowth and undergrowth affect neurodevelopment: similarities and differences from behavior to epigenetics.

The size of an infant at birth, a measure of gestational growth, has been recognized for many years as a biomarker of future risk of morbidity. Both being born small for gestational age (SGA) and being born large for gestational age (LGA), are associated with increased rates of obesity and metabolic disorder, as well as a number of mental disorders including attention deficit/hyperactivity disorder, autism, anxiety, and depression. The common risks raise the question of what neurobiological mechanisms are altered in SGA and LGA offspring. Here we review recent findings allowing for direct comp

Early intervention in pediatrics offices for emerging disruptive behavior in toddlers.

BACKGROUND: This study provides preliminary data about a parenting intervention for families of preschoolers with early attention deficit hyperactivity disorder/oppositional defiant disorder symptoms carried out in two diverse primary care pediatric offices. METHOD: Parents of toddlers completed behavioral screening questionnaires at well-child visits. Eligible parents participated in a 10-week parenting education group using the Incredible Years program. Mothers completed several outcome measures at three time points: before participating in the group, immediately after the group ended, and

Tourettism and dystonia after subcortical stroke.

The term "tourettism" has been used to describe Tourette syndrome (TS)-like symptoms secondary to some specific cause. Tics associated with attention deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD), or both, are commonly present in TS, but this constellation of symptoms has been rarely attributed to stroke. We describe two boys who suffered a subcortical stroke and subsequently developed hemidystonia, tics, and behavioral comorbidities. Both had right hemispheric stroke involving the basal ganglia at 8 years of age, and in both the latency from the stroke to the onse

Psychiatric Comorbidity and Associated Professional Mental Health Utilization among Students in Recovery from Addiction in Southern Collegiate Recovery Communities.

OBJECTIVES: This pilot study explores the additional psychiatric problems and their associated professional mental health utilization by students in recovery from addiction in collegiate recovery communities (CRCs) across the US South. The article has three goals: introduce CRCs to a broad medical audience; identify coexisting psychiatric concerns among students in CRC programs; and determine these students' type, rate, and location of professional mental health services. METHODS: During the 2014-2015 academic year, this study recruited participants through e-mail to all known CRC directors,

A unique case of food restriction and OCD diagnosed as PANDAS and a review of the literature.

Pediatric autoimmune neuropsychiatric disorder associated with streptococcal infections (PANDAS) is recognized as a significant causative element in the emergence of childhood-onset obsessive-compulsive disorder (OCD) and movement disorders, including tic disorders, and choreiform movement. It is associated with various behavioral and psychiatric manifestations in children, such as separation anxiety disorder, body dysmorphic disorder, attention deficit hyperactivity disorder, and eating disorders. We describe a unique case of progressive weight loss in a previously healthy 13-year-old child w

Neuropsychiatric Manifestations of Mast Cell Activation Syndrome and Response to Mast-Cell-Directed Treatment: A Case Series.

Mast cell activation syndrome (MCAS) is an immune disease with an estimated prevalence of 17%. Mast cell chemical mediators lead to heterogeneous multisystemic inflammatory and allergic manifestations. This syndrome is associated with various neurologic and psychiatric disorders, including headache, dysautonomia, depression, generalized anxiety disorder, and many others. Although MCAS is common, it is rarely recognized, and thus, patients can suffer for decades. The syndrome is caused by aberrant mast cell reactivity due to the mutation of the controller gene. A case series is presented herein

Sympathomimetic Overdose With Lisdexamfetamine in a Pediatric Patient Resulting in Stress Cardiomyopathy-A Case Report.

BACKGROUND: Sympathomimetic overdoses, as seen in amphetamine ingestions, are known to have cardiac effects ranging from tachycardia and hypertension to coronary vasospasm and myocardial infarction. Stress cardiomyopathy is rare in pediatric patients and has not been reported in association with the extended-release preparation of lisdexamfetamine, indicated for the treatment of attention-deficit hyperactivity disorder. CASE SUMMARY: A 17-year-old girl presented with an intentional ingestion of lisdexamfetamine and signs and symptoms of a sympathomimetic toxidrome. She was found to have sever

Social Cognition in Adult ADHD: A Systematic Review.

The aim of this systematic review was to collect and align the research on social cognition impairments in adults with Attention-deficit/hyperactivity disorder (ADHD). In particular, we selected and analyzed papers on emotion recognition and processing, Theory of Mind (TOM), empathy, and other facets of social cognition as decision making. We identified 16 papers published between 2012 and 2022 which meet inclusion criteria. Papers search, selection, and extraction followed the PRISMA guidelines. In order to summarize data from papers, we used a narrative synthesis approach. Results show diffe

Fatal oral methylphenidate intoxication with postmortem concentrations.

Methylphenidate (MPD) is a widely prescribed stimulant used primarily for the treatment for attention-deficit/hyperactivity disorder (ADHD). Suicide attempts involving MPD ingestion have been well described; however, deaths attributed solely to MPD ingestion have not been reported. A 62-year-old woman was found dead on her floor. The only discrepancy in among her medication quantities was that >three hundred 10 mg MPD tablets were missing. Analysis utilizing gas chromatography-mass spectrometry revealed elevated postmortem MPD peripheral and central blood, liver and vitreous humor concentratio

Questionnaire-based assessment of executive functioning: Psychometrics.

The psychometric properties of the Learning, Executive, and Attention Functioning (LEAF) scale were investigated in an outpatient clinical pediatric sample. As a part of clinical testing, the LEAF scale, which broadly measures neuropsychological abilities related to executive functioning and learning, was administered to parents of 118 children and adolescents referred for psychological testing at a pediatric psychology clinic; 85 teachers also completed LEAF scales to assess reliability across different raters and settings. Scores on neuropsychological tests of executive functioning and acade

Impaired task switching performance in children with dyslexia but not in children with autism.

Problems with cognitive control in both autism and dyslexia have already been reported in different studies. The present study specifically examined task-switching performance in children with autism and dyslexia. For this purpose, a multiple-trial paradigm was used with cues for colour- and shape-matching tasks presented before a run of trials. The cue could imply a task switch (when the cue changed the task) or a task repetition (when the cue did not change the task). Both reaction times and error rates were measured for switching, restarting, and general task performance. Participants were

Sustained attention deficit in bipolar disorder is not a working memory impairment in disguise.

Euthymic patients with bipolar disorder have been reported to show persistent deficits in sustained attention. However, the sustained attention task which was used also placed demands on working memory. Bipolar disorder patients in the euthymic state were therefore compared with healthy controls on two measures of sustained attention with and without a working memory component. Signal detection methodology was applied to the results. Euthymic patients with bipolar disorder were particularly impaired at detecting targets in the sustained attention task without a working memory component. This d

Supporting Neurodiverse Junior Doctors: Challenges, Strategies, and Policy Implications for Inclusive Medical Training.

Neurodiversity encompasses natural variations in brain function, including autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), dyslexia, and dyspraxia. These differences influence cognitive processing, communication, and sensory perception, shaping how individuals engage with demanding clinical environments. In medicine, junior doctors face intense workloads and high emotional demands, which may amplify difficulties associated with neurodivergent traits. This editorial highlights the challenges experienced by neurodiverse junior doctors and reflects on the need for
