[A study of the prevalence of infectious markers in blood donors in rural areas. The case of Kamina hospital].

INTRODUCTION: The transmission of infectious agents such as the human immunodeficiency virus (HIV), hepatitis B (HBV), hepatitis C (HCV) and syphilis is the greatest threat to transfusion recipient safety. The purpose of this study was to determine the seroprevalence of infectious markers with a view to contributing to transfusion safety by selecting donors. METHODS: A retrospective analysis of the medical records ofblood donors covering the period 1 January to 31 December 2008 was conducted. The global seroprevalence of HIV HBV HCV and syphilis was, respectively, 2.9%, 1.6%, 0.2% and 0.2%. A

Maternal sickle cell anemia and neonatal isoimmunization.

OBJECTIVE: To study the risk of alloimmunization in pregnant women with sickle cell disease (SCD) and of isoimmunization in their offspring. METHOD: Thirty mothers with SCD were studied and their 35 neonates (group 1) were compared with 538 infants of mothers without hemoglobinopathies (group 2). RESULT: Six mothers with SCD developed alloantibodies. There was no correlation with maternal age (P = 0.6), parity (P = 0.18) or blood transfusions (P = 0.4). The risk of alloimmunization by transfusion was 20%. Six neonates were isoimmunized, a higher incidence than in group 2 (P = 0.02; relative …

[Pathology of the placenta. XI. Feto-fetal transfusion syndrome].

An account is given in this part of the series of placental alterations in the foeto-foetal transfusion syndrome of monochorial twin pregnancies. Arteriovenous or less often arterio-arterial anastomoses between the two parts of the placenta were found to be conditions for such foeto-foetal transfusion. Blood is discharged on the foetal side from one twin to the other, with one of the twins being adversely affected in growth and general development (donor), which may be as bad as acardia in case of very early onset of transfusion. The other twin (recipient) will undergo better development but w

[Acute pulmonary infiltrates in hematologic malignancies in aplasia (author's transl)].

Extensive pneumonias are usually implicated as the sole cause for acute respiratory failure complicating severe neutropenia in hematologic malignancies and are often fatal. We report study of 11 patients investigated and treated in intensive care unit, using transtracheal aspiration, continuous positive airway pressure (CPAP) via face mask and granulocyte transfusions Two groups of patients emerged from this study. The first group with immediately diffuse pulmonary infiltrates, positive blood cultures, negative tracheal cultures, marked improvement in hypoxemia during CPAP, benefits from granu

Evaluation of thromboelastometry (Rotem) - guided and conventional hemostasis management effectiveness in neonatal congenital cardiac surgery.

IntroductionPost-CPB bleeding remains a critical challenge in neonatal cardiac surgery, and early detection of coagulopathy to guide transfusion decisions may offer significant benefits. We hypothesized that routine use of intraoperative ROTEM as a decision parameter would reduce perioperative transfusions, modulate transfusion patterns and improve the outcomes in newborns undergoing congenital heart surgery.MethodsThis retrospective cohort study was conducted at a single-center tertiary academic hospital and included 58 consecutive neonates who underwent congenital cardiac surgery with cardio

Impact of Transfused Citrate on Pathophysiology in Massive Transfusion.

UNLABELLED: This narrative review article seeks to highlight the effects of citrate on physiology during massive transfusion of the bleeding … hypocalcemia, coagulopathy, and need for further transfusion resulting in a new citrate load. Whole blood transfusion typically requires lower volumes of transfused product than component therapy alone, resulting in a lower citrate burden. Efforts should be made to limit the amount of citrate infused into a patient in hemorrhagic shock while simultaneously addressi …

Sickle Hepatopathy.

Sickle hepatopathy is an umbrella term describing various pattern of liver injury seen in patients with sickle cell disease. The disease is not uncommon in India; in terms of prevalence, India is second only to Sub-Saharan Africa where sickle cell disease is most prevalent. Hepatic involvement in sickle cell disease is not uncommon. Liver disease may result from viral hepatitis and iron overload due to multiple transfusions of blood products or due to disease activity causing varying changes in vasculature. The clinical spectrum of disease ranges from ischemic injury due to sickling of red blo

Volume incompliance and transfusion are essential for transfusion-associated circulatory overload: a novel animal model.

BACKGROUND: Transfusion-associated circulatory overload (TACO) is the predominant complication of transfusion resulting in death. The pathophysiology is poorly understood, but inability to manage volume is associated with TACO, and observational data suggest it is different from simple cardiac overload due to fluids. We developed a two-hit TACO animal model to assess the role of volume incompliance ("first-hit") and studied whether volume overload ("second-hit") by red blood cell (RBC) transfusion is different compared to fluids (Ringer's lactate [RL]). MATERIALS AND METHODS: Male adult Lewis

A flow cytometric method for phenotyping recipient red cells following transfusion.

BACKGROUND: Reticulocyte phenotyping is used for transfused patients, who have red cell antibodies, to match blood for subsequent transfusion. Current methods are labor-intensive and require a significant amount of sample. STUDY DESIGN AND METHODS: A simple dual-color flow cytometry method developed for antigen typing of reticulocytes in mixed red cell populations is reported. Antigens were labeled by an indirect immunofluorescence technique using undiluted reagent sera as the primary label, biotinylated goat anti-human IgG as the secondary label, and avidin-phycoerythrin as the fluorescent s

Effectiveness of pre-operative anaemia screening and increased Tranexamic acid dose policies on outcomes following unilateral primary, elective total hip or knee replacement: a statistical analysis plan for an interrupted time series and regression discontinuity study.

Perioperative blood transfusion is associated with poorer postoperative outcomes following hip and knee replacement surgery. Evidence for the effectiveness of some measures aimed at reducing blood transfusions in this setting are limited and often rely on weak pre-post study designs. Quasi-experimental study designs such as interrupted time series (ITS) and regression discontinuity design (RDD) address many of the weaknesses of the pre-post study design. In addition, a priori publication of statistical analysis plans for such studies increases their transparency and likely validity, as readers…

[Hemolytic-uremic syndrome in pneumococcal meningitis and infection. Importance of T-transformation].

A 4 month old girl developed a severe hemolytic uremic syndrome (HUS) following pneumococcal sepsis and meningitis. As a result of the hemolytic anemia and thrombocytopenia with associated gastrointestinal bleeding several red blood cell and thrombocyte transfusions became necessary. No plasma was transfused to the patient and all cellular blood components to be transfused were washed thoroughly in order to avoid the administration of eventually dangerous donor anti-T antibodies. Continuous peritoneal dialysis was performed for 23 days until sufficient spontaneous urine production was resumed.

Proposed revised nomenclature for transfusion-related acute lung injury.

A decade ago, definitions of "transfusionߚrelated acute lung injury (TRALI)" and "possible TRALI" were standardized for research and clinical diagnosis. Since then, evidence has confirmed that TRALI is often due to transfusion of white blood cell antibodies to at-risk patients, and the term "TRALI, antibody mediated" is appropriate for such cases. Other TRALI cases are non-antibody mediated. Because specific, nonantibody transfusion factors have not yet been confirmed to cause TRALI in humans, the general term "TRALI, non-antibody mediated" is appropriate for such cases. In contrast, evidence

Three per cent dextran-60 as a plasma substitute in blood component therapy. I. An alternative in surgical blood loss replacement.

Microaggregate-poor erythrocyte concentrate with 3% dextran-60 as a plasma substitute was compared with microaggregate-poor whole blood for replacement of intra-operative and immediately postoperative blood loss. Sixty patients undergoing total hip arthroplasty randomly received either of these two forms of therapy. In accordance with the clinical routine of our orthopedic department, an infusion of 500 ml of 6% dextran-70 (Macrodex) was given as thrombo-prophylaxis in both groups. Clinical presentation of delayed hemolytic transfusion reactions and hyperhemolysis in sickle cell disease.

Red blood cell (RBC) transfusion therapy is a key component in the comprehensive management of patients with sickle cell disease (SCD). Consequently, most adult SCD patients will receive at least one, and many will receive more than a hundred RBC transfusions in their lifetime. SCD patients develop RBC alloantibodies much more frequently than non-SCD transfused patients, which often make the selection of compatible RBCs extremely difficult, in addition to placing patients at significantly higher risk of suffering from delayed hemolytic transfusion reactions (DHTRs). Similar to alloimunization,

Transfusion-Transmitted Zika Virus Infection in Pregnant Mice Leads to Broad Tissue Tropism With Severe Placental Damage and Fetal Demise.

Zika virus (ZIKV) infection during pregnancy can cause significant problems, particularly congenital Zika syndrome. Nevertheless, the potential deleterious consequences and associated mechanisms of transfusion-transmitted ZIKV infection on pregnant individuals and their fetuses and babies have not been investigated. Here we examined transmissibility of ZIKV through blood transfusion in ZIKV-susceptible pregnant A129 mice. Our data showed that transfused-transmitted ZIKV at the early infection stage led to significant viremia and broad tissue tropism in the pregnant recipient mice, which were n…
