Success of scaphoid nonunion surgery is independent of proximal pole vascularity.

UNLABELLED: We followed 35 consecutive patients with scaphoid nonunions in a prospective longitudinal registry. All nonunions were treated with curettage, non-vascularized autogenous grafting and headless screw fixation. Preoperative magnetic resonance imaging, intraoperative bleeding points and histopathological analysis of cancellous bone in the proximal pole were recorded as measures of viability. Healing was categorized as ≥50% bony bridging on computed tomographic images in the plane of the scaphoid. Nine of 23 proximal poles demonstrated ischaemia on magnetic resonance imaging but none w

First in Man: Sternal Reconstruction with Autologous Stem Cells.

Sternal nonunion is associated with high morbidity and treated using rigid plate and screw fixation. This is the first reported example of successful sternal reconstruction using adipose-derived stromal vascular fraction (SVF) stem cells in addition to traditional techniques. Mesenchymal stem cells, one component of the SVF, play an important role in bone healing and were therefore used to promote remedial processes in a patient with sternal nonunion. A 3D printed model of the patient's sternum was used for preoperative planning of the plating. Intraoperatively, SVF was isolated using ultrason

[Isolated fractures of the scaphoid: classification, treatment and outcome].

Scaphoid fracture is the most common fracture in carpal bone of the wrist and represents 11% of all upper extremity fractures. In most cases, the mechanism of injury is a fall with an outstretched hand. Scaphoid fracture detection can be made by clinical examination and standard radiography. Computed tomography (CT), bone scan and MRI improve the sensitivity of the diagnosis. Non-displaced fractures of the scaphoid are treated with cast immobilization in most cases. Operative treatment should be considered in displaced fractures or non-displaced fractures in young active patients by using inte

[4-year results following Insall anterior cruciate ligament-plasty].

From May 1984 until December 1987 56 anterior cruciate ligament reconstructions were done in cases with single plane anteromedial instability, using the bone block iliotibial band transfer. The anterior distal part of the iliotibial tract with its osseous insertion from Gerdy's tubercule was used. The immediate fixation of the bone block by a screw or by threads to the tibia allows early motion of the knee, because no immobilization is necessary. There were 56 patients: 14 women, 42 men; 34 right, 22 left. About the same number (65) with more severe multiplane laxity had been treated by other

[Patient-specific subperiosteal implant for the treatment of upper denture retention problems].

A 66-year-old man presented with a loose complete denture in the edentulous maxilla, 1 year after receiving treatment for mucormycosis. This fungal infection had caused progressive necrosis of the maxilla, eventually resulting in a partial maxillary resection. Due to the defect in the jawbone, the denture was unstable and the retention poor. To improve denture retention, a patient-specific subperiosteal implant was fabricated. Computed tomography was used to create a 3-dimensional design and the implant was manufactured from titanium using selective laser melting. The implant was fixed to the

Total knee replacement in a dog with an articular epiphyseal medial condylar fracture of the distal femur, a case report.

The objective of this study was to describe outcome in a dog with a failed atypical articular medial femoral condyle fracture salvaged with total knee replacement. We present a retrospective case study of a 2-year-old male neutered Whippet dog. An articular medial condylar fracture was repaired primarily with extraarticular positional cortical screws. Construct failure was diagnosed 16 days after surgery when the dog was represented with lameness and radiographs confirmed fragment displacement. Total knee replacement was used for surgical salvage. Long-term follow up with the owner and patient

Immediate full-arch fixed rehabilitation of a narrow mandible with newly conceived connection system implants: A case report.

Rehabilitating thin jaws without reconstructive surgery entails using narrow implants. The proposed treatment adopted an innovative implant system, allowing the mini-screws to be parallel and immediately loaded. A mandible, wearing an overdenture, was functionalized contextually to the residual dental extraction and the placement of six 2.4-mm thick one-piece implants. Low-profile intermediate abutments, the LEMs, able to rotate over the spherical heads of the fixtures, were connected after suturing, oriented, and blocked in a mutual parallel position. The copings, engaging with a tapered junc

Anterior Cervical Discectomy and Fusion: A Surgical Intervention for Treating Cervical Disc Disease.

Cervical disc disease includes chronic disc degeneration, stenosis, spondylosis, and disc herniation; providers initially treat these conditions conservatively through symptomatic care. When conservative measures fail, surgery may be indicated. It is important to explore all the surgical options available and the risks and benefits of each procedure. An anterior cervical discectomy and fusion (ACDF) is a procedure involving the removal of disc material to achieve neural tissue decompression and placement of a bone graft or interbody implant and a cervical plate and screws to stabilize the spin

Midface distraction osteogenesis using a modified external device and 3D virtual simulation: technical note.

INTRODUCTION/BACKGROUND: Distraction osteogenesis (DO) with an external distraction device such as the rigid external distraction frame has become an established method for treating midface hypoplasia in faciocraniosynostosis. It allows for greater advancement of the midface in comparison with traditional Le Fort III osteotomies, associated or not with fronto-orbital osteotomies (Le Fort IV). However, the forward movement of the bone segments may not always be performed obeying an ideal distraction vector, resulting in asymmetries, anterior open bite, and loosening of screws. In addition, the

Rupture of an Intramedullary Elastic Bundle Nail Following Humeral Shaft Nonunion: A Case Report.

A 77-year-old woman reported a spiral right humeral shaft fracture, which was treated by closed reduction and fixation with elastic bundle retrograde intramedullary nail. Despite a proper closed reduction and a minimally invasive surgery obtained, nonunion arose. The patient refused the advised second surgical treatment. At 12 months, the patient was admitted to our department as a consequence of persistent pain and unnatural humeral shaft mobility. New X-ray imaging highlighted intramedullary nail rupture due to excessive implant mobility at the nonunion site. Consequently, nail removal was p

Failure of Osseointegrated Dental Implant After Alveolar Nerve Transposition: A Report of an Unusual Complication and Surgical Management.

PURPOSE: To describe an unusual case of mandibular fracture after osseointegrated dental implant removal placed after inferior alveolar nerve transposition. REPORT OF CASE: The patient underwent inferior nerve transposition for implant placement due to lack of interarch space. Two months after metal-ceramic crowns installation, the patient began to complain of pain in the region of the implant. In an attempt to remove the implant, there was a fracture of the implant, and only two third of it was removed, leaving only the apical portion of the implant in the mandible. The patient opted for the

Simultaneous bilateral snowboarder's fractures in a young woman: a rare entity.

BACKGROUND: Talus is a well-supported bone in the foot. A fracture of talus requires a high-impact injury. A wedge-shaped inferolateral component is the lateral process of the talus (LPT). A fracture of LPT is also known as a 'snowboarder's fracture'. Simultaneous bilateral snowboarder's fracture is rarely reported in English literature. CASE: We present here a case of simultaneous bilateral snowboarder's fractures in a slow-moving motor vehicle accident in the icy conditions. The injuries were unique because the snowboarder's fractures were accompanied by fractures of the inferior aspect of

Late collapse osteoporotic vertebral fracture in an elderly patient with neurological compromise.

Vertebral fracture is a frequent phenomenon in people with osteoporosis and does not lead to clinical problems in most cases. Only a small number of patients suffer from serious neurological complications related to late collapse. Acute or subacute painful osteoporotic vertebral fracture can be tackled using reinforcement techniques; however, neural compression by displaced bone fragments and late kyphosis can produce neurological deficit and require surgical decompression and stabilization. The identification of risk factors associated with a patient's poor recovery is important for adequate

Displaced fracture of the waist of the scaphoid.

A displaced fracture of the scaphoid is one in which the fragments have moved from their anatomical position or there is movement between them when stressed by physiological loads. Displacement is seen in about 20% of fractures of the waist of the scaphoid, as shown by translation, a gap, angulation or rotation. A CT scan in the true longitudinal axis of the scaphoid demonstrates the shape of the bone and displacement of the fracture more accurately than do plain radiographs. Displaced fractures can be treated in a plaster cast, accepting the risk of malunion and nonunion. Surgically the displ

Surgical management of unstable elbow dislocation without intra-articular fracture.

The purpose of this study was to describe soft tissue injury patterns and report the clinical results of primary ligament repair with use of protected early mobilisation in unstable elbow dislocations with pure capsulo-ligamentous injuries. Twenty-four patients who presented with traumatic unstable elbow dislocation without associated intra-articular fracture were reviewed. Anatomical repair was performed using metal anchor screws and the bone tunnel method. Ligament avulsion was noted in 55% for the medial collateral ligament, 80% for the lateral collateral ligament, 60% for the flexor tendon

Uppsala screw fixation versus the von Bahr technique in displaced cervical hip fractures: preliminary report.

A study of 75 patients with displaced cervical hip fractures was done utilizing two different multiple pinning techniques with threaded screws. One was the refined method of internal fixation by which the strongest bone of the femoral head was utilized to increase the stability of the fracture, and the other was the von Bahr method. As much as possible, other factors influencing healing were standardized in both groups. Forty-three of the patients were treated with the refined method and 32 with the von Bahr method. Complications occurred in 5 of 43 patients in the group treated with the new m

Pretibial abscess associated with instrumentation-specific infection after anterior cruciate ligament reconstruction: A case report.

Pretibial abscess is a rare but serious complication after anterior cruciate ligament (ACL) reconstruction. We report six cases of pretibial abscesses from 364 ACL reconstructions at our center between January 2022 and June 2023. Patients presented with poor wound healing, sinus tract formation, and magnetic resonance imaging showed fluid accumulation and bone marrow edema in the tibial tunnel but no deep infection. Bacterial cultures identified Pseudomonas aeruginosa and Burkholderia cepacia. Further investigation found contamination in the hexagonal reamers used for screw implantation, which
