[Sclerotic inflammation of the orbit and multifocal fibrosclerosis syndrome].

CLINICAL CASE: A fifty-two-year-old woman consulted our department because of a bilateral proptosis. The appearance of acute symptoms suggestive of a digestive disorder during her admission to hospital for the study of the proptosis, led to a diagnosis of multifocal fibrosclerosis with simultaneous affliction of orbital tissue, mesenteric fat and the brain stem. DISCUSSION: Idiopathic Sclerotic Inflammation of the orbit is a rare pathological entity, with similar clinical and radiological characteristics to orbital pseudo-tumour. The differential diagnosis is important because both these cond…

Metabolic stroke in three years old boy as the consequence of metabolic derangement. A case report of recidiving Reye's-like syndrome.

Three years old boy with developmental renal dysplasia was hit as newborn child by attack of cerebral edema with metabolic disturbances (hypoglycemia, hypophosphatemia, ketoacidosis and with hypocoagulation state) and was classified as child at risk in the pediatric evidence. In the third year of the age he went through nephrectomy and after the operation, the similar metabolic disturbances occurred (hypoglycemia, ketoacidosis, derangement of the metabolic situation). Cerebral edema and the metabolic stroke developed. Reye's-like syndrome was considered and serious functional disturbances of b

[Role of electrocochleography in identification of labyrinthine hydrops].

We studied potentialities of extratympanic electrocochleography (ETECG) in detection of labyrinthine hydrops (LH) in 50 patients who had an audiological picture of unilateral neurosensory hypoacusis. The patients were divided into two groups: group 1 patients suffered from attacks of systemic vertigo typical for Meniere's disease; group 2 patients had no vestibular complaints. ETECG results were compared to the dynamic range, results of otoacoustic emission and brain stem response, vestibulometry with electronystagmography (data of bithermal caloric test as most informative). The comparison in…

Intramedullary hemorrhage caused by arteriovenous malformation: a case of mixed lateral and medial medullary syndrome.

A 48-year-old man with no known risk factor for cerebrovascular disease, other than cigarette smoking, experienced the sudden onset of a mixed lateral and medial medullary syndrome. Computed tomography scan failed to show any definite abnormality. Magnetic resonance imaging scans revealed hemorrhage restricted to the left dorsolateral medulla. Angiography showed abnormal arteries originating from the left vertebral artery with small niduses located on the surface of the medulla and contralateral cerebellum. Small brain-stem hemorrhages are a contraindication to thrombolytic or anticoagulant th

[Transitory cranial dyskinesias and tubercular meningitis. Report of three cases].

Three patients who suffered transitory (from 18 to 49 days) cranial dyskinesias during the course of tubercular meningitis are described. The diagnosis was confirmed by the presence of acid-alcohol resistant bacilli in spinal fluid. Cranial tomography or magnetic resonance images failed to demonstrate anatomical lesions. The blink reflex and brain stem auditory evoked potentials were abnormal, indicating dysfunction of motor nuclei of the cranial nerves, possibly secondary to changes in their regulation by basal ganglia. No recurrences have been observed during follow-up (from 3 to 7 years).

The effects of four, commercial ceruminolytic agents on the middle ear.

Four, commercially available ceruminolytic agents and physiological saline were screened for ototoxic and inflammatory reactions on the middle ear mucosae of guinea pigs (n = 38) and dogs (n = 24). Each solution was injected transtympanically in anesthetized animals. The effects were assessed by brain stem auditory evoked response (BAER) tests to evaluate hearing function and by histological examination of the middle ear structures. Varying degrees of hearing loss and inflammation were observed in some guinea pigs and dogs treated with solutions A, C, and D, whereas no abnormal finding was ass…

[Morphological and morphometric study of sphenoidal sinus].

Thanks to the help of anatomical dissections and imaging techniques we way present this paper in which are described the morphologic characteristics of the sinus, its pneumatization types more frequently encountered, the traits more noteworthy related to the intersinusal partition and bone crests inside the sinus as well the distances detaching the sinus from the anterior wall of the sella turcica and clivus, through which wall the sinus cavity keeps in touch with noble cranial structures as the hypophysis and the brain stem. Ultimately our outcomes are compared with those given by other resea…

Concurrent trigeminal, abducens, and facial nerve palsies presenting as false localizing signs: case report.

Multiple cranial nerve dysfunction presenting as false localizing signs is rare. We report a 20-year-old woman who had concurrent trigeminal sensory disturbance, abducens, and peripheral facial nerve palsies in association with a contralateral acoustic neuroma. After surgery, the trigeminal nerve disturbance and the abducens nerve palsy completely recovered, but the peripheral facial nerve palsy persisted. The nature of tumor, the presence of brain stem distortion, the anatomic variation of posterior fossa, and the relationships of cranial nerves and nearby blood vessels, which are factors inf…

Pontine myelinolysis following correction of hyponatraemia.

A patient with severe hyponatreamia secondary to chronic renal failure was treated with peritoneal dialysis (PD). On the third day of admission, she developed progressive obtundation. Neurological examination showed bilateral brisk reflexes with intact brain stem reflexes. Magnetic resonance imaging demonstrated patchy demyelination of the pontine area indicating central pontine myelinolysis (CPM). Despite supportive measures, the patient died on the fifteenth day of admission. The rate of correction of hyponatraemia with peritoneal dialysis can be rapid and detrimental to hyponatraemic chroni…

Basilar artery occlusion in a young patient with Wegener's granulomatosis.

BACKGROUND AND PURPOSE: Stroke is an uncommon sequela of Wegener's granulomatosis, resulting primarily from vasculitic infiltration of small to medium-sized vessels. We describe a young patient with Wegener's granulomatosis and basilar artery occlusion. CASE DESCRIPTION: A 26-year-old man with histopathologically confirmed Wegener's granulomatosis suffered a brain stem stroke 24 hours after open lung biopsy. Angiography revealed midbasilar artery occlusion. CONCLUSIONS: Patients with Wegener's granulomatosis may experience premature large cerebral vessel occlusion. Putative etiologies in our…

Acute otitis media with transient sensorineural hearing loss. A case study.

A patient had acute otitis media with transient sensorineural hearing loss. Early symptoms included otalgia, progressive sensorineural hearing loss, and tinnitus with inflammation of the tympanic membrane. During the second week a conductive component emerged resulting in a 60-dB mixed loss in the 2- to 4-kHz region. The conductive component lasted 15 days while the sensorineural hearing loss recovered more slowly, with complete recovery at 6 months. Audiologic measures included conventional and extended high-frequency audiometry, 226-Hz and multifrequency tympanometry, binaural speech recogni

The effect of chronic one-eye patching on ocular myoclonus.

Ocular myoclonus developed in our 34-year-old patient 4 months after massive brain stem hemorrhage due to eclampsia. On chronic patching of the left eye, the vertical pendular nystagmus in the fixating right eye disappeared, whereas the covered eye was esotropic. While fixating, the left eye showed horizontal pendular nystagmus. With both eyes fixating, the right eye presented markedly vertical pendular nystagmus and the left eye a horizontal pendular nystagmus. It seems that chronic patching of one eye has a beneficial modulating effect on the vertical pendular nystagmus in the fixating eye.

[Magnetic resonance imaging of acoustic neuroma].

Thirteen patients with acoustic neuroma were studied on a 1.5 T superconductive magnetic resonance (MR) imager. Acoustic neuromas appeared as lower signal intensity than the surrounding brain stem on T1 weighted image (W.I.), and as higher signal intensity on T2 W.I.. Axial and coronal sections of T1 W.I. were very useful in observing the tumor in the auditory canal and in investigating the anatomical relations of the tumor and the surrounding structures. MR imaging is very excellent examination to make early diagnosis of the acoustic neuroma and preoperative anatomical evaluation.

[The effect of morphine and opiate receptor agonists on arterial pressure and the bioelectrical activity of the renal nerve during stimulation of the midbrain central gray substance].

It has been demonstrated in experiments on unrestrained and unanesthetized curarized cats that periaqueductal gray matter stimulation produce sympathetic-activating action, raise arterial pressure and heart rate, but at the same time is not effective enough to suppress the nociceptive shifts of haemodynamic reactions. Opioid mechanisms of spinal cord plays an essential role in sympathetic-activating action of periaqueductal gray matter. It is suggested that the influence of antinociceptive areas of the brain stem on sympathetic haemodynamic regulation is one of the causes of resistance of noci…

Congenital hydranencephaly and arthrogryposis of Corriedale sheep.

A syndrome of congenital malformations in Corriedale sheep characterised by brachygnathia inferior, campylognathia, tetraemlic arthrogryposis, kyphoscoliosis, hydranencephaly and hypoplasia of the brain stem, cerebellum and spinal cord occurring in various combinations is described. Histologically there was generalised hypomyelinogenesis and hypoplasia of the central nervous system with neurogenic atrophy of skeletal muscle. The syndrome resembled that caused by congenital infection with Akabane virus, however, serological, pathological and epidemiological data indicated that Akabane virus was…

[Pemphigoid and cerebral infarct. Syntropy of 2 diseases? Case report].

We report on two patients who suffered from bullous pemphigoid and developed cerebral infarctions during the course of this autoimmune dermatosis. The first patient who had a bullous dermatosis for three weeks presented with a sudden paresis of her right arm. She improved under steroid therapy and developed a left sided hemiparesis after steroids were reduced for diagnostic purposes. The second patient developed signs of brain stem infarction during the course of full-blown bullous pemphigoid. Neither patient had known risk factors, signs of atherosclerosis or cardiac embolism. Both patients i…

Acute obstructive hydrocephalus: an unusual complication after cardiopulmonary bypass.

A case of acute obstructive hydrocephalus in a patient undergoing an aortic valve replacement is presented. This condition came about as a result of massive left cerebellar hemisphere infarction, probably due to a calcific embolism from the aortic valve. The cerebrospinal fluid circulation was blocked by compression of the fourth ventricle and the Sylvius aqueduct secondary to ischemic edema. Following external ventricular drainage, the hydrocephalus resolved but a neurological deficit secondary to the cerebellar and brain stem infarction became evident. The diagnostic, prognostic and therapeu…
