Agreement between parent-proxy and child self-report in pediatric hypermobile Ehlers-Danlos syndrome.

Hypermobile Ehlers-Danlos syndrome (hEDS) is a common disorder in children and adolescents that negatively impacts health-related quality of life (HRQOL). It can include chronic pain, fatigue, autonomic dysfunction, and mood problems. The objective of this study was to examine levels of agreement between children and parents in the setting of hEDS and HRQOL. Individuals with hEDS, ages 10-20 years, and their parents were recruited to complete a series of surveys. Instruments included pediatric quality of life generic and multidimensional fatigue scales, Functional Disability Index, Pain-Freque

Recognizing medical child abuse in children presenting with chronic pain.

Medical child abuse (MCA), previously referred to as Munchausen by proxy, can present as chronic pain. We report the presentation of five children seeking treatment for chronic pain who we identified as victims of MCA. The index case had essentially not eaten for the 6 years of her life due to alleged allergies to all foods, developed severe pain, used a wheelchair for ambulation beyond a few blocks, and was alleged to have dysautonomia requiring oxygen monitoring at night. Other cases posed as arthritis that resulted in foot amputation and total body pain, fibromyalgia with alleged mutation n

Reading Disability Spectrum: Early and Late Recognition, Subthreshold, and Full Comorbidity.

Several studies have reported high comorbidity for reading disability (RD) and psychiatric disorders. The aim of this study was to investigate the comorbidity of subthreshold and full psychiatric disorders with RD while comparing subgroups based on age of RD recognition (early vs. late). We analyzed data from 130 children with RD and 82 typically developing children aged 7 to 18 years. RD was assessed with the Dyslexia Differential Diagnosis Maastricht-Hungarian Standard Test. Psychiatric diagnoses were based on the Mini International Neuropsychiatric Interview Kid. Chi-square tests were used

Musculoskeletal Evaluation of Children with Cerebral Palsy.

Cerebral Palsy (CP) is the most common chronic disability of childhood. The problems involved are complex; not only do these children have problems of mobility, but a plethora of associated problems [1]. A recent definition of CP includes secondary musculoskeletal problems [2]. The inclusion of this in the definition recognises the significance of musculoskeletal problems and the impact these problems have on the lives of children with CP and their families. Orthopedic management of the child with CP aims to reduce the impact of these musculoskeletal problems to help the child with CP to reach

Pre-Morbid Developmental Vulnerabilities in Children With Newly Diagnosed Acute Lymphoblastic Leukemia (ALL).

BACKGROUND: Not all children with acute lymphoblastic leukemia (ALL) were developing in a typical manner prior to diagnosis. Pre-existing developmental vulnerabilities (DV) may be related to long-term neuropsychological sequelae following ALL treatment, yet little is known about the prevalence or nature of prior DV in this population. PROCEDURE: Children with newly diagnosed ALL aged 2-18 years (n = 115) were screened for DV by asking parents about the child's prior developmental history and with the Developmental Profile-3 (DP-3). RESULTS: Twenty-six participants (23% of total sample) scree

Bilateral orbital complications of paediatric rhinosinusitis.

BACKGROUND: The spread of infection from the ethmoid sinuses to the orbit occurs directly through a congenital dehiscence of the lamina papyracea or via haematogenous spread through ophthalmic venous system. Hence orbital complications of paediatric rhinosinusitis are usually unilateral at presentation. METHODS: We describe three children with bilateral orbital cellulitis that occurred as a complication of rhinosinusitis without intracranial spread of the infection. The children ranged in the age group from 4 to 7 years. All these children had a prolonged hospital stay from 14 to 25 days and

Early identification and prognosis: parental developmental assessment of 18-month-old children.

The overall purpose of the study was to develop a screening instrument for the identification of developmental delay which could be administered entirely by parents. Reliability, concurrent and predictive validity was investigated in a Swedish population of 3,245 18-month-old children. The prevalence of mental retardation and learning disabilities implied by educational support provided in the regular school system was investigated in follow-up studies at 8 and 14 years. At 8 years, 20 children were administratively classified as mentally retarded. At 18 months they were among lowscorers or at

The breakthroughs of caregivers in a parent-implemented intervention for children with neurodevelopmental disorders in Hong Kong, China: an exploratory qualitative study of change mechanisms.

INTRODUCTION: Parent-implemented intervention (PII) trains caregivers to be the interventionists to offer tailored learning opportunities at home and is used to promote the positive development of children with neurodevelopmental disabilities. Its effectiveness hinges not only on learning and implementing the strategies, but also on the beliefs, wellbeing and capacities of the caregivers. The existing literature focused on the acquisition and application of skills, yet little is known about the inner experiences and transformation of caregivers. This study explored their changing processes and
