Spatial transcriptomics differences of histopathological growth patterns in colorectal cancer liver metastases.

BACKGROUND: Histopathological growth patterns (HGPs) have emerged as prognostic and predictive biomarkers in colorectal liver metastases (CLM). The desmoplastic/encapsulating HGP (EHGP) is associated with improved survival after surgery, whereas the replacement HGP (RHGP) is associated with poorer outcomes. However, HGPs can only be reliably assessed postoperatively, limiting their use as preoperative biomarkers. A deeper molecular understanding of HGPs could inform biomarker development and therapeutic strategies, including approaches to promote EHGP. METHODS: We applied in situ sequencing (

Preoperative MMR testing in colorectal cancer: Large-scale cohort confirms reliability but identifies tumor heterogeneity as a challenge.

BACKGROUND: Although previous studies have indicated that preoperative biopsy is reliable for MMR testing in colorectal cancer, they were limited by small sample sizes and interpretation based on traditional binary criteria. METHODS: This retrospective cohort of 5307 CRC patients with paired biopsy-resection specimens underwent a two-tier re-review: first with NCCN/CAP criteria, then with an optimized integrated interpretation standard, plus retesting of indeterminate cases. RESULTS: Concordance between biopsy and resection was 99.6% and the kappa for binary MMR status was 0.984 (95% CI 0.97

Integrating Tumor Budding and the Invasive-Front Microenvironment in Colorectal Carcinoma: An Exploratory Histopathological Score-Based Association Study.

The invasive tumor front (ITF) of colorectal carcinoma (CRC) is a biologically active interface where epithelial dissociation, stromal activation, immune remodeling, and angiogenesis converge. This retrospective histopathological association study examined whether tumor budding combined with selected ITF microenvironmental markers could better characterize aggressive clinicopathological features of non-metastatic CRC. Fifty-two surgically resected primary CRCs were analyzed after exclusion of patients with distant metastatic disease at diagnosis or surgery. Tumor budding was scored according t

Association Between ABO Blood Group and Survival in Early-Stage Colon Cancer: A Single-Center Retrospective Cohort Analysis.

BACKGROUND: ABO blood group antigens influence cell adhesion, inflammation, and metastatic processes, raising interest in their potential prognostic role in colorectal cancer. However, previous studies have reported inconsistent findings. This study evaluated the association between ABO blood group and overall survival (OS) in early-stage colon cancer patients undergoing curative surgery. METHODS: This retrospective cohort included patients with pathologic stage II-III colon adenocarcinoma treated surgically between 2016 and 2024 at a single-center institution. Demographic, clinicopathologica

Feasibility and early patient-reported outcomes after elective laparoscopic sigmoid resection for diverticular disease: a pilot study.

BACKGROUND: Patient-reported quality-of-life (QoL) outcomes are increasingly recognized as relevant endpoints in elective colorectal surgery. However, the feasibility and interpretability of systematic QoL assessment after elective laparoscopic sigmoid resection for diverticular disease remain incompletely defined, particularly in single-center settings. METHODS: This retrospective single-center pilot study analyzed consecutive patients undergoing elective laparoscopic sigmoid resection for diverticular disease. The primary objective was to assess the feasibility and completeness of postopera…

Robotic right hemicolectomy with intracorporeal anastomosis versus laparoscopic right hemicolectomy with extracorporeal anastomosis (PRORHEM): protocol for a randomized controlled trial.

BACKGROUND: In laparoscopic right hemicolectomy (lapRHC) for colorectal cancer, intra-corporeal anastomosis (ICA) has been associated with faster recovery and lower incidence of incisional hernia (IH) compared with extra-corporeal anastomosis (ECA). However, due to technical constraints, ICA is not widely adopted, and ECA remains the standard approach. Robotic surgery, with enhanced dexterity and suturing capability, facilitates ICA and may therefore improve short-term outcomes and reduce IH incidence. METHODS: We designed a monocentric, double-blind randomized controlled trial to compare rob

Double-stapled intracorporeal anastomosis for minimally invasive right and left colectomies: a standardized technical approach.

BACKGROUND: Minimally invasive surgery (MIS) for colorectal procedures has become increasingly common, with intracorporeal anastomosis (ICA) gaining traction due to its documented patient benefits. However, widespread adoption and teaching of ICA can be challenging, often due to the reliance on advanced intracorporeal suturing skills. This technical note describes a standardized, fully stapled intracorporeal side-to-side anastomosis technique for minimally invasive right and left colectomies, aiming to simplify its execution and improve reproducibility. TECHNIQUE: The procedure begins with de

Photodynamic therapy combined with systemic treatment achieves significant remission in microsatellite instability-high locally advanced colorectal cancer: A case report and literature review.

Patients with locally advanced colorectal cancer (LACRC) are at high risk of recurrence and metastasis and benefit little from neoadjuvant radiotherapy (nCRT) or neoadjuvant chemotherapy (nCT). Although the Microsatellite instability-high (MSI-H) status has been identified as a valid predictive marker for immunotherapy response, heterogeneity in the treatment response remains. In this study, we report a case of a 70-year-old woman with locally advanced rectal cancer with MSI-H status who had no chance of surgery due to tumor invasion of the uterine wall (cT3N2Mx) and who underwent a systemic t

Human Papillomavirus (HPV)-Associated Primary Squamous Cell Carcinoma of the Rectosigmoid Colon: A Case Report and Literature Review of a Rare Malignancy.

Primary colonic squamous cell carcinoma (SCC) represents a small minority of all colorectal cancer cases. Common clinical presentations include abdominal pain, weight loss, anorexia, dyschezia, and hematochezia. While adjuvant chemotherapy/radiation may be considered, treatment of colonic SCC is primarily surgical. We report a case of primary colonic SCC. A 66-year-old man with multiple comorbidities and a 42-pack-year smoking history presented with abdominal pain, constipation, and rectal bleeding. Colonoscopy findings included an anorectal ulcer and an ulcerated rectal mass, diagnosed as poo

Durable complete response to pembrolizumab after BRAF/MEK inhibition in recurrent MSI-H/dMMR, BRAF V600E-mutant colon cancer: a case report.

Patients with metastatic colorectal cancer (mCRC) harboring both microsatellite instability-high/deficient mismatch repair (MSI-H/dMMR) and the BRAF V600E mutation represent a distinct clinicomolecular subgroup characterized by aggressive biology and distinct therapeutic challenges. While MSI-H/dMMR predicts sensitivity to immune checkpoint inhibitors (ICIs), the concurrent BRAF V600E mutation may contribute to secondary resistance or shortened durability of response in a subset of patients treated with ICI monotherapy. Here, we present the case of an 85-year-old female with locally advanced a

Simple definition of biologically borderline resectable colorectal liver metastases based on early surgical failure.

BACKGROUND: The benefit of neoadjuvant chemotherapy (NAC) in patients with resectable colorectal liver metastasis (CRLM) is debatable. This study aimed to establish a definition of biologically borderline resectable CRLM based on early surgical failure. METHODS: One hundred forty-two patients who underwent upfront surgery for resectable CRLM were examined. Potential predictors of early surgical failure were investigated to establish a definition of biologically borderline resectable CRLM. The impact of NAC on overall survival (OS) in patients with borderline resectable CRLM was examined, as w

Exploring the Impact of Chemotherapy on the Emergence of Antibiotic Resistance in the Gut Microbiota of Colorectal Cancer Patients.

With nearly half of colorectal cancer (CRC) patients diagnosed at advanced stages where surgery alone is insufficient, chemotherapy remains a cornerstone for this cancer treatment. To prevent infections and improve outcomes, antibiotics are often co-administered. However, chemotherapeutic interactions with the gut microbiota cause significant non-selective toxicity, affecting not only tumor and normal epithelial cells but also the gut microbiota. This toxicity triggers the bacterial SOS response and loss of microbial diversity, leading to bacterial mutations and dysbiosis. Consequently, pathog
