Effectiveness of eHealth Interventions for Adolescents and Young Adults With Congenital Heart Disease: Systematic Review.

BACKGROUND: Adolescents and young adults (AYAs) with complex congenital heart disease (CHD) may benefit from eHealth interventions, particularly when early signs of deterioration or self-management challenges arise. eHealth can enhance self-management skills and communication with health care professionals (HCPs). However, systematic reviews examining eHealth interventions for this population remain limited and heterogeneous, indicating a need for a systematic review of the literature to guide the development, implementation, and evaluation of such interventions. OBJECTIVE: In this systematic

WeChat-Delivered Mobile Medical Nutrition Therapy Intervention in Gestational Diabetes Mellitus: Randomized Controlled Trial.

BACKGROUND: Medical nutrition therapy (MNT) serves as the foundational intervention in the clinical management of gestational diabetes mellitus (GDM) management. However, inadequate supportive care often hinders patients' ability to sustain dietary modifications and self-management behaviors, particularly for complex regimens. Flexible online interventions are thus gaining interest as adjuncts to clinical care, with the potential to improve the outcomes of GDM self-management. OBJECTIVE: This study sought to evaluate the benefits of WeChat-delivered medical nutrition therapy (WeMNT), a WeChat

Healthcare-seeking behavior for obstetrical fistula and its determinant factors in sub-Saharan Africa: systematic review and meta-analysis protocol.

OBJECTIVE: An obstetrical fistula is an abnormal communication between the vagina and adjacent tubular structures, usually the bladder and rectum. If timely and effective treatment is not given, women with a fistula face lifetime embarrassment, isolation, social stigmatization, and marital separation. Although early detection and management of obstetrical fistula often result in improved treatment outcomes, most women do not have access to care due to either the cost, stigma, or lack of awareness. This study aimed to determine the pooled prevalence and predictors of healthcare seeking for obst

Interpreting Context in Rural and Remote Aged Care Facilities in Readiness for a New Care Model: A Mixed Method Study.

BACKGROUND: Geographical isolation compounds limited access to healthcare services and skilled workforce for the provision of rural aged care. Residents have complex chronic disease management and end-of-life care needs. An undersupply of general medical practitioners due to retirement, attrition or unfilled training places in Australia has impacted recruitment to rural areas. Nurse practitioners have been identified as a potential solution. AIM: To describe and explore the inner (local and organisational) and outer (wider health system) contexts of healthcare, from the perspective of care st

Standard Versus Family-Based Screening, Brief Intervention, and Referral to Treatment for Adolescent Substance Use in Primary Care: Protocol for a Multisite Randomized Effectiveness Trial.

BACKGROUND: Screening, brief intervention, and referral to treatment for adolescents (SBIRT-A) is widely recommended to promote detection and early intervention for alcohol and other drug (AOD) use in pediatric primary care. Existing SBIRT-A procedures rely almost exclusively on adolescents alone, despite the recognition of caregivers as critical protective factors in adolescent development and AOD use. Moreover, controlled SBIRT-A studies conducted in primary care have yielded inconsistent findings about implementation feasibility and effects on AOD outcomes and overall developmental function

Medicinal plants traditionally used in the health care practices by the indigenous communities of the Trans-Himalayan region of Ladakh, India.

ETHNOPHARMACOLOGICAL RELEVANCE: Ladakh, "the land of high-rising passes," is a cold arid desert located in the India's northernmost part of the Trans-Himalayan region. Traditional knowledge of medicinal plants in this fragile ecosystem is an important part of the primary healthcare system, particularly in remote areas where modern medical facilities are not fully operational. There is a need to update the traditional information on medicinal plants from time to time to understand any addition of plants or uses to assist in developing new drugs through pharmacological and phytochemical studies.

Patient experience of telemedicine for headache care during the COVID-19 pandemic: An American Migraine Foundation survey study.

OBJECTIVE: We sought to investigate the patient experience of telemedicine for headache care during the coronavirus disease 2019 (COVID-19) pandemic. BACKGROUND: The use of telemedicine has rapidly expanded and evolved since the beginning of the COVID-19 pandemic. Telemedicine eliminates the physical and geographic barriers to health care, preserves personal protective equipment, and prevents the spread of COVID-19 by allowing encounters to happen in a socially distanced way. However, few studies have assessed the patient perspective of telemedicine for headache care. METHODS: The American M

Effect of SMS reminders on PrEP adherence in young Kenyan women (MPYA study): a randomised controlled trial.

BACKGROUND: Pre-exposure prophylaxis (PrEP) is highly effective for preventing HIV acquisition. However, adherence among young women (aged 18-24 years) has been challenging. SMS reminders have been shown to improve adherence to antiretroviral therapy in some contexts, including in combination with real-time adherence monitoring. We aimed to determine the effect of SMS reminders on PrEP adherence among young women in Kenya over a 2-year period. METHODS: The monitoring PrEP among young adult women (MPYA) study was an open label randomised controlled trial involving young adult women at high ris

Evaluation of a Pharmacist-Managed Antidiabetic Deprescribing Program in an Integrated Health Care System.

BACKGROUND: In the elderly, use of medications may increase the propensity for adverse drug events due to alterations in pharmacokinetic and pharmacodynamic profiles from normal aging processes. Deprescribing is the planned and supervised process of dose reduction or discontinuation of medications that may lead to harm or are no longer beneficial. While there are studies detailing strategies to deprescribe medications such as benzodiazepines and antipsychotics in nursing homes or for patients with dementia, there is a lack of guidance to safely deprescribe chronic medications, such as antidiab

A Data-Driven Evaluation of the Stop TB Global Partnership Strategy of Targeting Key Populations at Greater Risk for Tuberculosis.

OBJECTIVE: Identifying those infected with tuberculosis (TB) is an important component of any strategy for reducing TB transmission and population prevalence. The Stop TB Global Partnership recently launched an initiative with a focus on key populations at greater risk for TB infection or poor clinical outcomes, due to housing and working conditions, incarceration, low household income, malnutrition, co-morbidities, exposure to tobacco and silica dust, or barriers to accessing medical care. To achieve operational targets, the global health community needs effective, low cost, and large-scale s

Human immunodeficiency virus (HIV) risk, prevention, and testing behaviors--United States, National HIV Behavioral Surveillance System: men who have sex with men, November 2003-April 2005.

PROBLEM/CONDITION: For CDC's goal of reducing the number of new human immunodeficiency virus (HIV) infections to be achieved, data are needed to assess the prevalence of HIV-related risk behaviors at a given time, monitor trends in these behaviors, and assess the correlates of risk. These data also can be used to evaluate the extent to which current HIV-prevention programs are reaching targeted communities and direct future HIV-prevention activities to reduce HIV transmission. REPORTING PERIOD: November 2003-April 2005. DESCRIPTION OF SYSTEM: The National HIV Behavioral Surveillance (NHBS) S

Clinical monograph for drug formulary review: erectile dysfunction agents.

BackgroundSignificant advances in the pharmacologic treatment of erectile dysfunction (ERD) have occurred in recent years, most notably the introduction of sildenafil, the first oral selective phosphodiesterase type 5 (PDE5) inhibitor, in 1998. Sildenafil quickly gained acceptance by the medical community and the public because of its broad efficacy for different types of ERD and its ease of use. Two PDE5 inhibitors, vardenafil and tadalafil, have since joined sildenafil to compete in the ERD market. A review was conducted by the Drug Information Service of a pharmacy benefits manager (PBM) to

Post-pandemic planning for maternity care for local, regional, and national maternity systems across the four nations: a mixed-methods study.

BACKGROUND: During the COVID-19 pandemic, significant reconfigurations were made to maternity care, to deliver this essential service while minimising the risk of infection for pregnant/post partum women and their infants, initially considered to be more vulnerable. DESIGN: This mixed-methods study had three work packages. Work package 1 used quantitative methods to analyse pregnancy outcomes over time, considering service reconfiguration and inequalities, using routinely collected maternity and offspring data from three diverse South London trusts. Work package 2 involved in-depth interviews

Why are people dying due to tuberculosis? A study from Alappuzha District, Kerala, India.

BACKGROUND: Tuberculosis (TB)is a major killer disease worldwide. It is the ninth leading cause of death worldwide and the leading cause from a single infectious agent. In India also, TB kills about 480,000 persons every year and more than 1400 every day. Vision of the National TB Control Programme is TB-Free India with zero deaths, disease and poverty due to TB. Specific targets set in the End TB strategy include a 90% reduction in TB deaths and an 80% reduction in TB incidence by 2030, compared with 2015. Understanding about real cause of death is important to plan strategies to further prev
