The role of hyperbaric oxygen in current emergency medical care.

HBO is the definitive treatment for carbon monoxide poisoning, air or gas embolism, and decompression sickness. It is an important adjunct to other treatment modalities in the management of patients with cyanide poisoning, exceptional blood loss (anemia), crush injury, and gas gangrene. In addition, HBO has many investigational uses such as reduction of edema in acute bowel obstruction before surgical intervention. It should be remembered that the best hope for tissue recovery occurs when HBO is initiated within 4 to 6 hours of injury. After this time irreversible muscle and peripheral nerve d

Histiocytosis-X isolated to the cervical spinal cord.

Histiocytosis-X may involve many tissues, including the central nervous system. This report is the first documentation of histiocytosis-X occurring exclusively in the spinal cord. We present a patient with cervical spinal cord disease causing spastic paraplegia, flaccid right arm weakness, and sensory loss. Magnetic resonance imaging of the spinal cord clearly demonstrated the entire lesion. Following surgical decompression and biopsy, the cervical spinal cord was treated with low-dose radiation, which resulted in complete regression of the lesion by radiologic assessment. The child became neu

Somatosensory evoked potentials following trigeminal nerve stimulation in trigeminal neuralgia.

The examination of somatosensory evoked potentials following trigeminal nerve stimulation in 20 subjects with idiopathic trigeminal neuralgia revealed a pathological increase in latency of the first positive component on the involved side in 7 (41%) of the 17 patients without antecedent surgical treatment. The mean difference in latency between the two sides was 1.3 msec (2p greater than or equal to 0.005). These results are indicative of microtraumatization of the trigeminal nerve in the posterior fossa by blood vessels, for example, leading to local demyelinization. The method seems to be of

Lumbosacral stenosis in dogs.

Fifteen dogs with stenosis of the lumbosacral portion of the spinal canal were treated by laminectomy, bilateral facetectomy, and foraminotomy. Concomitant disk extrusion, spondylosis, fracture, infection, neoplasia, hemivertebrae, and spinal bifida were not associated with the stenosis and attendant cauda equina compression. The predominant neurologic signs were intermittent hindlimb lameness and motor weakness; hindlimb muscle atrophy; presumed paresthesia with resultant dermatologic manifestations from self mutilation of the hindlimbs, tail, perineum, anal area, and genitalia; urinaryu or f

Spinal metastasis in the elderly.

Bony metastases are a frequent problem in elderly patients affected by cancer, and those with bony metastases involve the spine in approx. 50%. The most frequent spinal metastases (60%) are from breast, lung, or prostate cancer. The chance that an elderly patient (60-79 years old) is affected by bony metastases is four times higher in men and three times higher in women than a middle-aged patient (40-59 years old). Since the medical treatment with all the adjuvant treatment options prolong the survival of this particular patient group, the spinal metastases may become a mechanical issue, thus

Exploring the impact of extraocular disorders and their surgical correction on corneal astigmatism and higher-order aberrations: a comprehensive review.

Extraocular disorders and their surgical management can significantly influence corneal astigmatism and higher-order aberrations (HOAs). This review aimed to evaluate the relationship between extraocular conditions, their surgical correction, and subsequent changes in corneal astigmatism and HOAs. A comprehensive literature search was conducted using PubMed, Scopus, and Web of Science, including studies published between 1992 and 2024. Search terms included astigmatism, eyelid surgery, thyroid eye disease (TED), orbital decompression, blepharoplasty, chalazion, dermatochalasis, ptosis, pterygi

Surgical Management of Lower Back Pain: Is Optimizing Spinopelvic Alignment Beneficial for Patient Outcomes?

For the past two decades, the consideration of spinopelvic parameters, sagittal balance, and spine shape has gained importance in the diagnosis and optimal surgical management of painful adult spinal deformity. These principles are used with increasing frequency in the surgical planning and treatment of degenerative mechanical lower back pain. Several parameters exist to analyze both global and regional spinal balance. Chronic lower back pain due to degenerative disc disease, degenerative spondylolisthesis, or adult spinal deformity can be surgically managed in a multitude of ways ranging from

Factors associated with long-term deterioration in back pain after surgical treatment for low-grade lumbar spondylolisthesis at 2 and 5 years: an evaluation from the Quality Outcomes Database spondylolisthesis data.

OBJECTIVE: Symptomatic, low-grade spondylolisthesis is usually well treated by surgical intervention. While some patients obtain less than optimal improvement, low-grade spondylolisthesis deteriorates in a few patients. The purpose of this study was to investigate what factors predict deterioration in back pain scores after surgical treatment of low-grade spondylolisthesis. METHODS: The Quality Outcomes Database (QOD) was queried for patients who underwent single-level surgery for management of grade 1 spondylolisthesis, including decompression with fusion and decompression alone. Patient-rep

Comparative efficacy of UBE simple decompression versus UBE-PLIF in treating degenerative lumbar spondylolisthesis secondary to spinal stenosis: A retrospective study.

The optimal surgical strategy for Meyerding grade I degenerative lumbar spondylolisthesis secondary to spinal stenosis remains controversial. We compared unilateral biportal endoscopic (UBE) simple decompression versus UBE-assisted posterior lumbar interbody fusion (UBE-PLIF) to identify the most effective approach based on real-world data. A retrospective study of 58 patients treated between January 2022 and January 2024: 35 underwent UBE decompression; 23 had UBE-PLIF. We analyzed operative time, fluoroscopy frequency, hospital stay, cost, pain (Visual Analogue Scale for back and leg), funct

An unusual case of ventral spontaneous thoracic epidural hematoma.

Spontaneous spinal epidural hematoma (SSEH) rarely occurs. Without early diagnosis, SSEH can lead to the acute onset of neurologic deficits. We report the case of a 65-year-old male with diabetes mellitus who was admitted to our emergency department with a chief complaint of sharp and severe pain in the left scapula and behind the sternum. He was misdiagnosed with cardiovascular disease until the onset of progressive bilateral paraplegia and lower limb numbness. Magnetic resonance imaging revealed a ventral thoracic SSEH. Surgical treatment to remove epidural hematoma and laminectomy for decom

Occurrence of multidrug-resistant tuberculous meningitis associated with injury during spinal surgery: A case report.

Here we describe a case of multidrug-resistant tuberculous meningitis associated with injury inflicted during spinal surgery. A 54-year-old man was diagnosed with multidrug-resistant spinal tuberculosis (TB). Anterior debridement and fusion with posterior fusion and instrumentation were conducted to remove damaged bone and TB-associated granulomatous tissue. After surgical decompression, a lancet wound (about 5mm in length) was discovered in the cerebral dura mater at the L3 position of the vertebral body. At four weeks post-surgery, the patient experienced fever, hemiparesis, and aphasia. Cer

Intracranial extradural hematoma: Spontaneous rapid decompression - not resolution.

The surgical option to evacuate an intracranial extradural hematoma (EDH) was postponed in a 2-year-old female child who appeared fully alert and active after a brief spell of unconsciousness following a fall from height. The child was received, with a swelling on and around the right parietal eminence, by the emergency staff just half an hour after the time of injury. The immediate X-ray skull and first computed tomography (CT) scan head showed a parietal bone fracture, EDH, and cephalhematoma. However, follow-up CT scan head after about 4½ h revealed the dramatic absence of EDH but increased

Definitive single-stage posterior surgical correction of complete traumatic spondyloptosis at the thoracolumbar junction.

Complete dislocation at the thoracolumbar junction is a rare occurrence, with only 4 previously reported cases in 3 separate series. Surgical procedures in the reported cases of spondyloptosis at the thoracolumbar junction have been described using instrumentation, reduction, decompression, and stabilization techniques. In this report the authors' patient presented with spondyloptosis at the thoracolumbar junction, resulting in a T-11 American Spinal Injury Association Grade A injury. The authors corrected the patient's thoracolumbar spondyloptosis with surgical reconstruction without the use

Surgical management of medically refractory trigeminal neuralgia.

The role of surgery for patients with medically refractory trigeminal neuralgia (TN) is well established. High-quality magnetic resonance imaging, including gadolinium-enhanced and volume acquisition sequences, should be performed to exclude intracranial tumors or demyelinating disease as the cause of the pain, as well as to clearly demonstrate the trigeminal nerve and adjacent blood vessels. For physiologically healthy patients with Type 1 TN, a microvascular decompression (MVD) is the preferred surgical approach because of its high rate of complete pain relief, the durability of the pain rel

Advances in surgical rehabilitation in thyroid eye disease.

BACKGROUND: Surgery has evolved over the past 20 years. Conceptual and technical advances have helped improve the surgical rehabilitation of Graves' orbitopathy. A historical comparison of methods and approaches has shown clear progress. SUMMARY: Surgical rehabilitation of Graves' orbitopathy is traditionally staged: orbital decompression, extraocular muscle surgery, eyelid repositioning, and the soft tissue volume and redraping. Improvements in techniques have evolved from the problems that historical methods have presented, and an evaluation of each will help appreciate the improvements tha

The management of acute visual loss after sinus surgery--two cases of rhinogenic optic neuropathy.

INTRODUCTION: Different causative mechanisms of ophthalmic complications during endonasal sinus surgery have been reported. Only a few cases of blindness caused by affections of the optic nerve due to inflammatory paranasal sinus disease have been described. OBJECTIVE: Inflammatory optic neuropathy shall be considered among the causative factors for amaurosis after sinus surgery. MATERIAL: We present two patients with dramatic visual decrease occurring two weeks after sinus surgery as a result of inflammatory posterior paranasal sinus disease. RESULTS AND CONCLUSION: Our therapy including s

Cerebellopontine angle epidermoids presenting with trigeminal neuralgia.

We studied the clinical characteristics of cerebellopontine angle (CPA) epidermoids presenting with trigeminal neuralgia (TN). Twenty-four patients were analyzed retrospectively and the literature reviewed with regard to clinical manifestation, imaging features, surgical procedures and prognosis. TN may be the initial symptom of CPA epidermoid, particularly in young patients. Epidermoids are characteristically hypodense nonenhancing lesions on CT scans, while on MRI they exhibit long T1 and T2 relaxation times. Although complete removal is ideal in the surgical management of CPA epidermoid, pr

Surgical options for the treatment of cervical spondylotic myelopathy.

Cervical spondylotic myelopathy is a disease of the cervical spinal cord that results from circumferential compression of the degenerative cervical spine, often in a congenitally narrow spinal canal. Surgical recommendations must be based on patient characteristics, symptoms, function, and neuroradiologic findings. ACDF is an excellent option for one- or two-level spondylosis without retrovertebral disease. Anterior corpectomy and strut grafting may provide an improved decompression and is ideal for patients with kyphosis or neck pain. Laminectomy historically yields poor results from late def

Unilateral laminectomy for bilateral decompression of lumbar spinal stenosis: a prospective comparative study with conservatively treated patients.

The authors performed single- or multiple-level unilateral laminectomy to treat lumbar spinal stenosis in patients with mild to moderate leg pain and compared the results with those from patients treated with conservative therapy in a prospective study. This decompression technique produced a 68% rate of improvement compared with a 33% rate for conservatively treated patients. The surgical group exhibited significant and sustained improvement, whereas the functional and clinical status of the conservatively managed group had returned to baseline during the same period. Degenerative Spondylolisthesis: Diagnosis and Treatment.

Degenerative spondylolisthesis is most often seen at the L4-5 level. The most common complaint is back pain, but the advent of leg symptoms, such as claudication and restless legs syndrome, is often the reason for seeking specialized medical attention. Conservative treatment usually suffices; extended bed rest is of little value. The 15% of patients who are surgical candidates are those with clinical signs and symptoms of cauda equina abnormality, progressive muscular weakness, or progressive incapacitating radicular pain or claudication. The author advocates pedicle-to-pedicle decompression w
