BACKGROUND: Atopic dermatitis is a common, chronic, relapsing, pruritic, eczematous skin condition occurring in patients with a personal or family history of atopy. The aim of this study is to describe the profile of atopic dermatitis seen at a tertiary referral skin center in a tropical multiracial country.
METHODS: A retrospective chart review was conducted of all the patients with atopic dermatitis seen during the first six months of 1994.

BACKGROUND: We report a case of acute retinal necrosis with contralateral varicella zoster dermatitis.
CASE: The patient, a 61-year-old man, developed acute retinal necrosis in the right eye 1 month after varicella zoster dermatitis in the left eyelid.
PROGRESS: Intravenous acyclovir, corticosteroids, and laser photocoagulation were effective without any surgical treatment.
CONCLUSION: We suggest that ophthalmoscopic examination of both eyes is needed in cases with varicella zoster dermatitis.

In 2 earlier studies, we found increased nickel re-test reactivity at earlier experimentally induced nickel eczema sites. Farm workers handling animal feeds are exposed to a variety of chemicals, some of which may cause allergic contact dermatitis. A case of allergy to ethoxyquin (a preservative added to chicken feed to inhibit vitamin degradation) in a chicken farmer is presented. Although the offending allergen was identified in this patient, it proved difficult to find ethoxyquin-free chicken feed products and the patient's dermatitis persisted. When facing the clinical problem of dermatitis in animal workers, the possibility of allergic contact dermatitis to components in animal feeds must be considered.

We report a case of dermatitis herpetiformis associated with bullous pemphigoid in which the clinical, histopathological and immunopathological findings were corroborated by direct immunoelectronmicroscopy performed on normal and peri-lesional skin biopsies. Western immunoblotting detected the 180 kDa BPAG2 consistent with bullous pemphigoid. HLA-typing confirmed the DR3 DQ2 haplotype associated with dermatitis herpetiformis.

For reasons that are unclear, patients with dermatitis herpetiformis (DH) have a lower than expected mortality rate from ischaemic heart disease. We have compared risk factors for ischaemic heart disease (lipids, fibrinogen levels, smoking history and social class) in 29 DH patients and 57 controls matched for age and sex. Patients with DH had significantly lower cholesterol, triglycerides, apolipoprotein B and fibrinogen and higher HDL2; they also smoked less and were of higher social class. This report shows the absolute genetic linkage of celiac disease (CD) to the HLA-DQ region, and supports the fact that dermatitis herpetiformis (DH) follows the same pattern of HLA-mediated susceptibility in extensive series of Caucasian Spanish patients. We have previously reported 18 cases of an ichthyosiform contact dermatitis caused by antiseptic solutions containing 3% cetrimide and 0.3% chlorhexidine. We have outlined rationale and problems associated with three types of patch tests that may provide diagnostic help in patients with difficult-to-manage AD. Each of these methods is imperfect and in need of greater study using a critical approach, better techniques, more attention to irritant controls, and better definition of atopic populations. Proof of validity for aeroallergen patch testing will necessarily require large quantities of purified antigens. In spite of all these problems, patch testing can be useful and sometimes essential for the diagnosis and management of difficult AD.

A lipophilic yeast, Pityrosporum orbiculare, found in normal microflora of the human skin, is considered to be a pathogenic allergen of atopic dermatitis (AD). We measured IgE antibody to P. orbiculare using a CAP RAST FEIA kit in 44 patients with AD (AD alone 17, AD+bronchial asthma (BA) 27). The incidence of positive RAST was 88.2% in the patients with AD alone and 74.1% in the patients with AD+BA. Enhancement of radiation injury to the skin and mucous membranes has been observed with a number of chemotherapeutic agents. A 32-year-old woman with metastatic breast cancer received local radiation therapy to the lumbosacral area. Six weeks later, systemic edatrexate therapy was initiated and a localized painful erythema with edema and a vesicular eruption occurred over the previous site of radiation therapy. Physicians should be aware that edatrexate can cause radiation recall, and that careful use of anti-inflammatory agents may allow continued chemotherapy treatment.
