Pruritus in female patients.

Pruritus is a frequent symptom in many dermatological diseases. In this review we want to focus on not only itch problems specific to women, namely, pruritic vulvodermatoses, but also the specific pruritic dermatoses of pregnancy. The specific characteristics of the vulva and the hormonal changes during the different age periods make these dermatoses very particular. It seems that vulvar diseases are still underdiagnosed and undertreated. Pruritic vulvar diseases have a huge impact on quality of life. The most common pruritic diseases will be discussed, such as atopic and contact dermatitis, p

Management and treatment of pruritus.

Pruritus, or itch, is a common sensation that causes a person to want to scratch. It is a complex process that may negatively impact quality of life and commonly occurs with skin disorders such as atopic dermatitis and urticaria. It could also be a symptom related to an underlying disease process such as cholestasis or hyperthyroidism, or simply be caused by dry skin, especially in the cold, winter months. Therapy is often aimed at eliminating the underlying cause first, followed by the management of the itchy sensation. Treatment may include prescription and over-the-counter (OTC) medications

Antibiotic responsive ulcerative dermatoses in German Shepherd Dogs with mucocutaneous pyoderma.

Mucocutaneous pyoderma is a disease of unknown aetiology affecting mucocutaneous skin and is responsive to antibacterial therapy. It is reported to affect the lips, nasal planum, nares, perioral skin and less commonly, the eyelids, vulva, prepuce and anus. Three cases of mucocutaneous pyoderma are presented. Two of the cases showed ulcerative lesions in the inguinal and axillary regions in addition to more typically reported lesions. Two of the dogs had concurrent atopic dermatitis and the third had clinical signs suggestive of hyper-sensitivity disease. The clinical and histopathological feat

[The use of tacrolimus in dermatology].

This article aims to review the literature on the use of tacrolimus in dermatology. We tried to focus on the local application of tacrolimus. The local application of tacrolimus is known to be effective for several skin diseases, especially atopic dermatitis. Since the major action site of tacrolimus is activated T-lymphocytes, the drug showed efficiency mostly in inflammatory dermatological diseases including psoriasis, lichen planus, alopecia areata and pyoderma gangrenosum. Tacrolimus lacks most of side effects of local steroids and seems to have better results for short and long term appli

Trachyonychia: a case report and review of manifestations, associations, and treatments.

Trachyonychia ("rough nails") is best considered a reaction or morphologic pattern with a variety of clinical presentations and etiologies. It may involve only 1 or as many as 20 nails (20-nail dystrophy). It can be a manifestation of lichen planus, psoriasis, alopecia areata, immunoglobulin A deficiency, atopic dermatitis, and ichthyosis vulgaris. Nail matrix biopsy results and physical examination findings help in establishing the cause of this condition, though often trachyonychia is an isolated finding. When trachyonychia occurs in childhood as a manifestation of lichen planus, it tends to

Subclinical coeliac disease.

The prevalence of coeliac disease (CD) was investigated in an epidemiological study of school children from a district of Rome. Mass screening of IgA-AGA levels (anti-gliadin antibody) were determined in blood samples from the study group and the results were positive in 19 children. Eighteen of these agreed to undergo endoscopic biopsy. Histological study of the duodenal specimens revealed villous atrophy in twelve children none of whom had the characteristic symptoms of CD. The most frequent complaints were abdominal pain, aphtous stomatitis, and atopic dermatitis. The high prevalence of CD

Some recent advances in HLA and skin diseases.

During the past 15 years, much has been learned about the human major histocompatibility complex. These new findings have been applied to the whole field of medicine, including transplantation immunology, blood transfusions, association with disease, human genetics, and the mechanism of human immune responses. In clinical medicine, the association between the major histocompatibility complex and various diseases has attracted the greatest interest. In this paper, we summarize the basic information on the human histocompatibility complex and its association with dermatologic disorders. Data fro

Clinical results of photochemotherapy. The Cleveland Clinic experience.

PUVA therapy has proved to be beneficial for dermatologic conditions such as psoriasis, vitiligo, and mycosis fungoides. Attempts have been made to treat other diseases with the modality; five patients with pityriasis rubra pilaris were treated, with only a partial response noted in one. A poor to fair response was seen in treating a small group of patients with cystic acne, atopic dermatitis, and neurodermatitis. As might be expected, no response was seen in one patient with Darier's disease. At present, photochemotherapy must be viewed with cautious optimism. Strict adherence to a rational t

Modulatory effects of interferon-γ and interleukin-13 on tumor necrosis factor-α-induced chemokine gene expression in canine keratinocytes.

Keratinocytes play a crucial role in the pathogenesis of canine atopic dermatitis (cAD). Tumor necrosis factor (TNF)-α, a proinflammatory cytokine, is highly expressed in the lesional skin of dogs with atopic dermatitis. Additionally, interferon (IFN)-γ, a T-helper (Th)1-type cytokine, and interleukin (IL)-13, a Th2-type cytokine, are also detected in the lesional skin during the acute and/or chronic phases of cAD. However, their effects on chemokine expression in canine keratinocytes remain unclear. This study investigated the effects of TNF-α on chemokine mRNA expression in canine keratinocy

Emerging treatments for dermatologic diseases in infants, children, and adolescents: a systematic review of clinical trials on biologics and small molecule inhibitors.

BACKGROUND: Recent advancements in the treatment of paediatric dermatological conditions have emerged with the introduction of biologics and small molecule inhibitors (SMIs). These therapies target specific inflammatory pathways, which may enhance treatment outcomes for diseases like atopic dermatitis, psoriasis, and alopecia areata. OBJECTIVES: This systematic review seeks to assess the effectiveness and safety of biologics and SMIs for dermatologic conditions in children and adolescents, with an emphasis on randomised clinical trials. METHODS: We performed an extensive literature search ac

Carcinogenic risk in patients treated with UVA-1 phototherapy: A 5-year retrospective study.

BACKGROUND: UVA-1 phototherapy was first used to treat atopic dermatitis and afterwards to several other skin diseases. The contribution of UVA-1 in human photocarcinogenesis, skin photoaging, immune suppression, and hyperpigmentation is now well established. The actual contribution of UVA-1 radiation to the development of malignant melanoma (MM) in humans cannot be excluded. PURPOSE: The aim of the study is to evaluate the risk of developing skin cancers (non-melanoma skin cancers (NMSCs) and MM) in patients treated with UVA-1 phototherapy with a 5-year dermatological follow-up. METHODS: We

Infection risk with JAK inhibitors in dermatoses: a meta-analysis.

Evolving evidence suggests that Janus Kinase Inhibitors (JAKi) may predispose to certain infections, including tuberculosis and human herpes viruses. This review aimed to compare the infection risk in patients on a systemic JAKi for a dermatologic indication to a placebo. A systematic review was carried out from inception to June 2023, using the EMBASE, Medline, SCOPUS, and Cochrane Library of Registered Trials databases. Eligible studies included placebo-controlled randomized trials that investigated the incidence of infection in patients with a dermatologic indication. Primary outcomes inclu

An epidemiological survey of anhidrotic/hypohidrotic ectodermal dysplasia in Japan: High prevalence of allergic diseases.

Anhidrotic/hypohidrotic ectodermal dysplasia (A/HED) is a congenital disorder characterized by anhidrosis/hypohidrosis and inadequate hair and dental dysplasia. Large-scale case studies of patients with A/HED have already been conducted overseas, while there has been no large-scale study, but only a few case reports in Japan. Furthermore, an epidemiological study of this disease has not been conducted in Japan to date. The purpose of this study was to investigate the clinical characteristics of A/HED patients, the status of genetic aberrations and complications of A/HED in Japan. Initially, we

Co-morbidity between dermatologic diseases and psychiatric disorders in Papua New Guinea.

BACKGROUND: Neurodermatitis (now subdivided into lichen simplex chronicus and atopic dermatitis) was included among the psychosomatic disorders. Psychologic causes have been suggested for many skin conditions, such as urticaria, lichen simplex chronicus, atopic dermatitis, psoriasis, alopecia areata, and pruritus. The evidence for psychologic causation is neither strong nor convincing; however, psychiatric disorders are common among people with established skin diseases. Recent investigators have published data reporting co-morbidity between certain psychiatric disorders and skin diseases. The
