Managing epidermolysis bullosa during the coronavirus pandemic: Experience and ideals.

The 2019 novel coronavirus pandemic has tremendously affected health-seeking behaviors. Fear of contracting the disease has been a major factor keeping patients from presenting to hospitals, even when urgent or emergent medical attention is needed. Hospitals limiting staff exposure and capacity to accommodate patients also limits opportunities to seek care. Although physical distancing is encouraged to curb infections, this call needs to be tempered with public health education for what constitutes emergencies and urgent medical conditions needing face-to-face attention. Measures to assuage fe…

Environmental transmission of generalized anxiety disorder from parents to children: worries, experiential avoidance, and intolerance of uncertainty.

Generalized anxiety disorder (GAD) runs in families. Building on recent theoretical approaches, this review focuses on potential environmental pathways for parent-to-child transmission of GAD. First, we address child acquisition of a generalized pattern of fearful/anxious and avoidant responding to potential threat from parents via verbal information and via modeling. Next, we address how parenting behaviors may contribute to maintenance of fearful/anxious and avoidant reactions in children. Finally, we consider intergenerational transmission of worries as a way of coping with experiential avo…

Blood-Injection-Injury Phobia and Dog Phobia in Youth: Psychological Characteristics and Associated Features in a Clinical Sample.

Blood-Injection-Injury (BII) phobia is a particularly debilitating condition that has been largely ignored in the child literature. The present study examined the clinical phenomenology of BII phobia in 27 youths, relative to 25 youths with dog phobia-one of the most common and well-studied phobia subtypes in youth. Children were compared on measures of phobia severity, functional impairment, comorbidity, threat appraisals (danger expectancies and coping), focus of fear, and physiological responding, as well as vulnerability factors including disgust sensitivity and family history. Children an…

Development of social anxiety disorder secondary to attention deficit/hyperactivity disorder (the developmental hypothesis).

Social anxiety disorder (SAD) may develop secondary to childhood attention deficit/hyperactivity (ADHD) in a subgroup of the patients with SAD. Patients pass through a number of identifiable stages of developmental pathways to SAD as they grow up. Patients with ADHD have maladaptive behaviours in social settings due to the symptoms of ADHD. These behaviours are criticized by their parents and social circle; they receive insults, humiliation and bullying. After each aversive incident, the individual feels shame and guilt. A vicious cycle emerges. The patients then develop social fears and a cog…

Patients' experience of adolescent idiopathic scoliosis surgery: a phenomenological analysis.

Background Adolescent idiopathic scoliosis is a three-dimensional curvature of the spine of unknown cause that occurs in often otherwise fit young people. A complex surgical procedure is required for the most severe curves. Quantitative literature suggests scoliosis surgery improves patients' lives, while qualitative literature focuses on patients' concerns rather than their experience. Aims To explore how adolescents interpret their perioperative experience. Method Six participants, aged 15-18, were interviewed and transcripts were analysed. Findings Four themes were identified: shock, fears …

Gender differences in social anxiety disorder: results from the national epidemiologic sample on alcohol and related conditions.

This study examined gender differences among persons with lifetime social anxiety disorder (SAD). Data were derived from the National Epidemiologic Survey on Alcohol and Related Conditions (n=43,093), a survey of a representative community sample of the United States adult population. Diagnoses of psychiatric disorders were based on the Alcohol Use Disorder and Associated Disabilities Interview Schedule-DSM-IV Version. The lifetime prevalence of SAD was 4.20% for men and 5.67% for women. Among respondents with lifetime SAD, women reported more lifetime social fears and internalizing disorders …

An examination of the relation between traumatic event exposure and panic-relevant biological challenge responding among adolescents.

The current study uniquely extended research that has linked traumatic event exposure to panic-spectrum problems among adolescents. It was hypothesized that among 127 adolescents (age range: 10 to 17 years; M = 14.63, SD = 2.24), those who endorsed a history of traumatic event exposure would evidence significantly greater anxious and fearful reactivity to a well-established 3-min voluntary hyperventilation procedure compared to nonexposed individuals. Results were consistent with hypotheses, suggesting traumatic event exposure is associated with anxious and fearful reactivity to abrupt increas…

The prognosis and incidence of social phobia in an elderly population. A 5-year follow-up.

OBJECTIVE: To examine the prognosis and incidence of social fears and phobia in an elderly population sample followed for 5 years. METHOD: A general population sample (N = 612) of non-demented men (baseline age 70) and women (baseline age 70 and 78-86) was investigated in 2000-2001 and in 2005-2006 with semi-structured psychiatric examinations including the Comprehensive Psychopathological Rating Scale, and the Mini International Neuropsychiatric Interview. Social phobia was diagnosed according to the DSM-IV criteria. RESULTS: Among nine individuals with DSM-IV social phobia in 2000, 5 (55.6…

Examining patient conceptions: a case of metastatic breast cancer in an African American male to female transgender patient.

An African American male to female transgender patient treated with estrogen detected a breast lump that was confirmed by her primary care provider. The patient refused mammography and 14 months later she was diagnosed with metastatic breast cancer with spinal cord compression. We used ethnographic interviews and observations to elicit the patient's conceptions of her illness and actions. The patient identified herself as biologically male and socially female; she thought that the former protected her against breast cancer; she had fears that excision would make a breast tumor spread; and she …

Cognitive-behavioral therapy for panic disorder: a review of treatment elements, strategies, and outcomes.

This article provides an overview of cognitive-behavioral therapy (CBT) for panic disorder. CBT is currently considered a first-line treatment for panic disorder. It offers benefit after short-term intervention, typically consisting of 12-15 sessions conducted in either an individual or a group format. The treatment focuses on the elimination of the patterns that underlie and perpetuate the disorder. Through CBT, patients learn about the nature of the disorder and acquire a set of strategies that counter the fears of panic attacks themselves, and break the recurring cycle of anticipatory anxie…

Brain function and conditioning in posttraumatic stress disorder.

Posttraumatic stress disorder (PTSD) is commonly treated by psychotherapy, which may draw upon behavioural psychology or cognitive-behavioural psychology, thereby making use of desensitisation techniques--amongst others; hypnotherapy may also be used. Hypnotherapy and psychotherapy are also available for helping patients who suffer from symptoms associated with general stress or who show phobic symptoms, such as a fear of heights or of walking across bridges. Studies of patients with such disorders have not always linked emotional (affective) and behavioural symptoms with psychophysical factor…

Radiation risk in the context of liability for injury.

It is perceived by the man in the street that low-level radiation from a nuclear facility is more dangerous than that from other practices. The radiation protection system, in particular the ALARA principle, leads to concerns that even the smallest exposure to radiation is abnormal and dangerous. Public perception of the radiation risk leads to fear in the minds of the public. A consequence of this fear itself may be damage to health in the form of psychological damage or nervous shock. The paper draws attention to the liability for damages by radiation, in particular under the common law of t…

The relationship between disgust sensitivity and avoidant behavior: studies of clinical and nonclinical samples.

Researchers have reported links between specific phobias (of small animals and blood/ injection/injury (BII)) and disgust sensitivity. Avoidance is one route by which disgust sensitivity may play a role in the etiology or maintenance of pathological fear. This paper discusses a series of three studies to examine behavior associated with disgust sensitivity. Study I examined disgust sensitivity and avoidance of disgust-evoking tasks among spider fearful and nonfearful participants. Study 2 reanalyzed previously published data on the relationship between disgust sensitivity and visual avoidance …

We discuss these results in light of widespread fears of harm caused by parent-child bedsharing. Clinical features of panic attacks in schizophrenia.

Since reports have underscored that panic attacks (PA) may be an identifiable state occurring in schizophrenia, we studied the symptomatology of PA in a group of schizophrenic patients. Of 40 patients (21 males and 19 females) attending a clinic for maintenance therapy of schizophrenia, 19 (36.8%) had a lifetime history of PA. Seven among those 19 patients (36.8%) had or had had spontaneous panic attacks, not related to phobic fears or delusional fears, and for the 12 remaining patients, the PA were related to paranoid ideas. Moreover, the paranoid subtype of schizophrenia tends to be more oft…

Stability over situations in temperamental traits of horses as revealed by experimental and scoring approaches.

Individual behavioural reactions of adult horses in a variety of experimental tests were compared with ratings by riding teachers. The tests were made in a non working situation, with the animals being released in an arena, a box (arena test, new object test, learning tests) or handled (new object/handling situation). The traits rated by teachers were fearfulness, nervousness, gregariousness and learning abilities at work (ridden or handled). Despite a great homogeneity in the reactions exhibited by the horses in the different situations, large individual differences were present. Correlations…

Is there an adult form of separation anxiety disorder? A brief clinical report.

OBJECTIVE: The aim of this clinical report is to investigate whether symptoms of separation anxiety disorder can occur in adulthood. CLINICAL PICTURE: Three cases are described to illustrate that adults may experience: wide-ranging separation anxiety symptoms, such as extreme anxiety and fear, when separated from major attachment figures; avoidance of being alone; and fears that harm will befall those close to them. Symptoms of panic appeared to be secondary to separation anxiety, and none of the patients fulfilled criteria for dependent personality disorder. TREATMENT: Group cognitive behav…

Psychopathological and neuropsychological symptoms in patients with subclinical and remitted hyperthyroidism.

To investigate relationships between hyperthyroidism and behavior, 45 formerly hyperthyroid patients (now euthyroid) and 51 control subjects were investigated by (a) a semi-structured psychiatric examination, (b) self-rating scales to assess mood states and personality, and (c) neuropsychological tests. Patients with "subclinical" or "remitted" hyperthyroidism showed more abnormalities than the controls in all dimensions investigated. Forty-three percent of patients (10% of controls) complained of "seriously reduced" well-being with feelings of fear, hostility, and inability to concentrate. Wh…

First-time postpartum motherhood during armed conflict: experiences, challenges, coping, and support needs among women with mobilized partners.

Armed conflict creates profound challenges for first-time mothers navigating the early postpartum period without the presence of their partners. The abrupt loss of expected support, combined with heightened fear and prolonged uncertainty, intensifies emotional vulnerability and disrupts maternal adjustment to early parenthood. Despite extensive research on trauma and crisis, little is known about how large-scale armed conflict shapes first-time mothers' lived postpartum experiences and their engagement with care systems. This study aimed to explore these experiences among women who gave birth …

Low Reward and Heightened Threat Neural Activity are Both Associated with Heightened Anhedonia: A Transdiagnostic Perspective.

Psychiatric neuroscience typically examines categorical diagnoses and brain systems in isolation. This approach is complicated by comorbidity and within-diagnosis heterogeneity and fails to evaluate whether relationships are unique to, or common across, neural circuits. To address this, we examined associations between reward and threat neural activity and symptoms of General Distress, Fears, and Anhedonia-Apprehension from the tri-level model of depression and anxiety. Data were collected on 339 participants aged 18-19; 272 completed a monetary incentive delay task (MID) assessing reward neur…
