Traumatic Anterior Hip Dislocation With an Open Book Pelvic Fracture and Femoral Artery Compression: An Atypical Case Report.

CASE: A 34-year-old patient was involved in a motorcycle accident, resulting in an anterior hip dislocation associated with an open book pelvic fracture and compression to the femoral artery. He required an emergent closed reduction of the hip dislocation and open reduction and internal fixation of the pelvic fracture. CONCLUSION: This case underscores the importance of maintaining a high level of suspicion for anterior hip dislocation, particularly in patients involved in motorcycle accidents, where a flexed, abducted, and externally rotated hip is common. Timely diagnosis allows for prompt

[Traumatic posterior atlantoaxial dislocation without fracture of the upper cervical spine in a female geriatric patient : Case report and literature search].

Traumatic posterior atlantoaxial dislocation (PAAD) without detection of a fracture of the upper cervical spine is a very rare injury that usually occurs in younger patients and in most cases leads to immediate death due to distraction of the spinal cord. In contrast, the present case describes this injury in a female geriatric patient at the age of 75 years. In the literature there are also clinical case reports, where traumatic PAAD without a fracture did not result in neurological deficits and where initially existing neurological deficits were completely reversible through closed or open r

Traumatic Bifocal whereas and Ipsilateral Posterior Knee Dislocation: A Case Report.

CASE: A 19-year-old man sustained combined, ipsilateral inferior hip and posterior knee fracture-dislocations secondary to a motor vehicle collision. He underwent immediate closed reduction of the knee and delayed open reduction internal fixation but required emergent open hip reduction for an irreducible femoral head incarcerated on a pubic root fracture. At the 1-year follow-up, he demonstrated excellent functional outcome with painless and full hip and knee range of motion. CONCLUSION: Irreducible inferior femoral head dislocation in combination with a knee dislocation requires thoughtful

Rim plate fixation for a rare fracture pattern of the lateral femoral condyle: A case report.

Osteochondral fractures of the lateral femoral condyle caused by lateral patella dislocation have been rarely reported. The AO/OTA Classification is not suitable for this uncommon injury. Comminution of the anterior cartilage surface of the lateral condyle with bone impaction is challenging to reduce and repair accurately, leading to uncertainty in joint function recovery. The treatment for this rare fracture is not commonly reported. We, herein, report a unique case where the lateral condyle osteochondral fracture occurred alongside patellar dislocation and instability of the patellofemoral j

Technique for Reduction and Fixation of a Hoffa Fracture with Ipsilateral Patella Dislocation from Low-Energy Trauma, a Rare Injury: A Case Report.

CASE: A 25-year-old man sustained a right femur Hoffa fracture with an ipsilateral patella dislocation from a twisting injury to his right knee. He underwent open reduction and internal fixation of the right femur with medial patellofemoral ligament (MPFL) reconstruction with allograft. One year postoperatively, he had returned to full activity and painless range of motion. CONCLUSION: Hoffa fracture associated with ipsilateral patella dislocation is a rare injury in young men and warrants judicious clinician awareness. The literature regarding management of this injury combination is minimal

Traumatic Avulsion of Gluteus Tendons Associated With Posterior Fracture-dislocation of the Femoral Head: A Case Report.

CASE: We report the case of a patient who suffered traumatic avulsion of the gluteus medius and minimus tendons associated with posterior fracture-dislocation of the femoral head. The patient was treated with open reduction and internal fixation, followed by gluteus tenodesis with intraosseous anchors. CONCLUSION: Although in isolation, gluteus medius or gluteus minimus tendons tear, hip dislocation, and head fractures are relatively common injuries; the concurrence of these lesions is noteworthy for its rarity. This patient had an uneventful postoperative evolution, with a complete functiona

Combined Bone Transportation and Lengthening Techniques for the Treatment of Septic Nonunion of the Forearm Followed by Tendon Transfer.

Infected nonunion of a forearm fracture complicated by a considerable skin-muscle defect poses a great challenge to orthopaedic surgeons. The treatment strategy comprises eradication of the infection, ensuring bony union and soft tissue coverage along with functional restoration. We report a case of a 23-year-old man with an open Gustilo-Anderson IIIb fracture complicated by infected nonunion after internal fixation. After thorough surgical debridement, a considerable soft tissue defect, extensor muscle loss, and posterior interosseous nerve laceration had to be addressed. He was finally treat

Posterior fracture-dislocation of the humeral head treated without the use of metallic implants.

Posterior fracture-dislocation of the humeral head is an uncommon injury, usually associated with seizures, electrocution injury and high-impact trauma. Prompt diagnosis and treatment are important to prevent avascular necrosis of the humeral head. Various methods to treat such injury have been described. We report a 34-year-old man who had a four-part humeral head fracture with concomitant posterior dislocation that was treated with open reduction and internal fixation with non-absorbable polyester sutures. The diagnosis and operative techniques are described and factors influencing our choic

[Modern principles in treatment of complex injuries of the facial bones].

Concepts in the treatment of craniomaxillofacial fractures have changed over the last 15 years. Modern imaging techniques have become a central part in establishing a proper diagnosis. Advanced life support and intensive care medicine allow for early primary fracture treatment. The former principles of minimal exposure of bone fragments using small incisions have been replaced by principles from reconstructive craniofacial surgery comprising extensive subperiosteal dissection, exposure of all fracture lines, open reduction and rigid internal fixation. Missing bony structures are replaced prima

Distal radius fractures-Design of locking mechanism in plate system and recent surgical procedures.

Recently, many studies have emphasized the importance of the comprehension of detailed functional anatomy of the distal forearm and wrist joint, and their biomechanics. A significant contribution which yields good functional outcomes of surgical treatment was the development of the locking plate technology; this technology has facilitated the improvement of the surgical technique for the fixation of fractures. This article reviews the locking mechanism and design of the fixation screws and plate, and the details of the surgical technique including the double-tired subchondral support procedure

Comparison of internal fixation vs. external fixation after corrective osteotomy in children with cubitus varus.

BACKGROUND: Cubitus varus deformity is a well-known late complication of supracondylar fractures in children. In this retrospective study, the primary objective was to compare clinical and radiologic outcomes of lateral closing-wedge osteotomy with either internal fixation or external fixation in pediatric patients with cubitus varus deformities. MATERIALS AND METHODS: From 2010 to 2017, 35 consecutive patients with cubitus varus deformities secondary to supracondylar fractures were included in this study. After corrective osteotomy was performed via a limited lateral approach, the method of

Cable plating with a single strut allograft in the treatment of periprosthetic fractures of the femur.

BACKGROUND:: Hip arthroplasties are increasing worldwide resulting in an increasing number of periprosthetic fractures. These fractures are difficult to treat with various the different fixation or revision options described, many of which have high complication rates. PURPOSE:: To investigate whether our described method of treating periprosthetic fractures is an effective, safe and reproducible method of treating patients. METHODS:: We describe the largest series of a cable plate fixation system combined with a single cortical strut allograft to treat patients with periprosthetic fractures

Rush pin fixation versus traction and casting for femoral fracture in children older than seven years.

BACKGROUND: The optimal treatment for femoral fractures in children is controversial. The purpose of this study was to compare the results of Rush pin fixation with those of conservative treatment, and to evaluate the sequels of growth plate injury by internal fixation. METHODS: Eighteen femoral shaft fractures in 17 children who had concomitant head injury or multiple traumas were treated surgically. The mean age at operation was 9 years 3 months (range, 7 years 5 months to 11 years 1 month). One Rush pin was inserted from the tip of the greater trochanter, without reaming, to fix the fractu

Late reconstruction of orbital and naso-orbital deformities.

Acute orbital fractures and naso-orbital ethmoid fractures can result in chronic orbital and naso-orbital deformities. Understanding the acute injury is the first step in reconstructing the established late deformity. The best management strategy for reconstruction of orbital hypertelorism is to avoid late complications by repairing these deformities early near the time of the original fractures. New technologies from computer-guided surgical planning and additive manufacturing technology produce passive fitting implants tailored for patient-specific needs.

Isolated comminuted trapezium fracture: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Trapezium, as a carpal bone positioned at the distal row, is often associated with other wrist injuries such as distal radius fracture. Isolated trapezium fracture, especially in a comminuted form, rarely occurs. There are only six reports of isolated comminuted trapezium fractures in the literature to the best of our knowledge. CASE PRESENTATION: We present a case with an isolated comminuted trapezium fracture presenting pain in his thenar eminence and thumb motion limitation. He was treated by Open Reduction and Internal Fixation (ORIF) with the K wire pin. CLI

Fibula Nail versus Locking Plate Fixation-A Biomechanical Study.

In the treatment of ankle fractures, complications such as wound healing problems following open reduction and internal fixation are a major problem. An innovative alternative to this procedure offers a more minimally invasive nail stabilization. The purpose of this biomechanical study was to clarify whether this method was biomechanically comparable to the established method. First, the stability (range of motion, diastasis) and rotational stiffness of the native upper ankle were evaluated in eight pairs of native geriatric specimens. Subsequently, an unstable ankle fracture was created and f

Open reduction and internal fixation of a traumatic diastasis of the pubic symphysis: one-year radiological and functional outcomes.

The aim of this study was to review the number of patients operated on for traumatic disruption of the pubic symphysis who developed radiological signs of movement of the anterior pelvic metalwork during the first post-operative year, and to determine whether this had clinical implications. A consecutive series of 49 patients undergoing internal fixation of a traumatic diastasis of the pubic symphysis were studied. All underwent anterior fixation of the diastasis, which was frequently combined with posterior pelvic fixation. The fractures were divided into groups using the Young and Burgess cl

Bilateral lower limb ischaemia following acetabular reconstruction and arthroplasty.

The incidence of acetabular fractures has increased markedly in patients over 60 in the last quarter of a century, with open reduction and internal fixation being regarded to be the treatment of choice in most patients with displaced acetabular fractures. Lower limb ischaemia following acetabular fixation or arthroplasty is rare. However, such complications can be limb or life threatening, and therefore, it is important to recognise them as early as possible. We present the case of a 70-year-old man with no significant medical history who underwent acetabular fixation with simultaneous arthrop
