A comparison of Web- with paper-based surveys of gay and bisexual men who vacationed in a gay resort community.

Internet Web page survey responses were compared with those collected using traditional paper-and-pencil strategies to assess relative inclusion of a geographically dispersed population and comparativeness in responses. Three hundred and seven gay or bisexual men who had vacationed in a particular gay community completed Web-based and 244 paper-and-pencil questionnaires. Each questionnaire contained the same wording and question order. More Internet respondents than paper reported bisexual identity, nonmetropolitan residence, greater numbers of nonmain male partners for unprotected anal interc

Gay Asian men in Sydney resist international trend: no change in rates of unprotected anal intercourse, 1999-2002.

Against a background of increasing unprotected anal intercourse (UAI) in the Sydney gay community (and internationally), complementary cross-sectional surveys of sexual practice were conducted among gay men of Asian background in 1999 (N = 319) and 2002 (N = 457). Self-complete questionnaires were used with recruitment at gay bars, gay social functions, and gay sex-on-premises venues. In 2002, self-report HIV status was 73.7% HIV-negative, 3.6% HIV-positive, and 22.8% unknown status (no significant change from 1999). Over time, the proportion of gay Asian men who reported any UAI with regular

Resurgence in infectious syphilis in Ireland: an epidemiological study.

INTRODUCTION: In 2000, a syphilis outbreak was identified in Ireland. MATERIALS AND METHODS: A prospective enhanced database was established in 2000. Crude incidence rates for the general population, men who have sex with men (MSM), and HIV-positive MSM were calculated. RESULTS: Three hundred fifty-six cases of infectious syphilis were diagnosed at 1 center. Eighty-five percent of cases were identified as MSM. Crude incidence rates in MSM, ranging in age between 20 and 44 years, peaked in 2001 at 719 cases per 100000 of the MSM population. A total of 17.4% of cases occurred in HIV-infected i

Effects of immunosuppression and disease severity upon neuropsychological function in HIV infection.

Effects of immunosuppression and illness severity upon neuropsychological function were assessed in a group of homosexual men with AIDS across 6 months. Participants included 62 who were seronegative (HIV-), 74 asymptomatic seropositives (HIV+A), 31 symptomatic seropositives (HIV+S), 23 with AIDS defining illnesses (AIDS-DI), and 10 who were diagnosed with AIDS solely on the basis of CD4+ levels falling below 200 /mm3 (AIDS-CD4). Groups were equivalent in age, education, and IQ. None were drug users, and none experienced a change in disease status across the 6-month inter-test interval. There

Zoster incidence in human immunodeficiency virus-infected hemophiliacs and homosexual men, 1984-1997. District of Columbia Gay Cohort Study. Multicenter Hemophilia Cohort Study.

Zoster is an important clinical problem for human immunodeficiency virus type 1 (HIV)-infected patients. Risk factors for zoster and trends in incidence in HIV-infected hemophiliacs and homosexual men (n=1218) were examined. From 1984 to 1997, 174 zoster cases were identified (average yearly incidence, 2.5%). Prior zoster episodes were associated with increased risk for a subsequent episode (relative risk [RR], 4.30; 95% confidence interval [CI], 3.11-5.95). Among hemophiliacs, children and adolescents had the highest zoster risk, and zoster risk declined with age (RR, 0.80 per decade; 95% CI,

Estimating the effect of zidovudine on Kaposi's sarcoma from observational data using a rank preserving structural failure-time model.

Researchers commonly express scepticism about using observational data to estimate the effect of a treatment on an outcome the treatment is intended to affect. In this paper, we consider using data from the Multicenter AIDS Cohort Study (MACS) to determine whether zidovudine prevents the development of Kaposi's sarcoma among HIV-positive gay men. Several methodologic issues common to observational data characterized the study: information on potentially important confounders was missing at some study visits; investigators did not always know the time of changes in treatment level, nor the valu

Brain atrophy in HIV infection is more strongly associated with CDC clinical stage than with cognitive impairment.

HIV infection often results in MRI-detectable brain atrophy and white matter signal hyperintensities (WMSHs). Magnetic resonance images were obtained from 31 HIV+ male patients and 10 high-risk controls. Variation within the HIV+ group on neuropsychological (NP) impairment and stage of systemic disease were relatively independent, allowing examination of the relative association of MRI measures with NP impairment versus with systemic stage of disease. HIV+ patients compared to high-risk controls evidenced global atrophy, reduced caudate nuclei volume, and a trend to gray matter volume loss but

Effects of group discussion and guided patient care experience on nurses' attitudes towards care of patients with AIDS.

Nurses with limited experience in caring for persons with AIDS (PWAs) were randomized into three groups receiving an educational session on universal precautions and (a) three group discussion sessions; (b) three sessions of caring for a PWA; or (c) assessment materials only. Both interventions were conducted by a nursing instructor experienced in care of PWAs and infectious diseases over a 3-4 week period. Measures included instruments for assessing prejudicial attitudes towards homosexual PWAs, self-ratings of infectious disease knowledge, ratings of perceived comfort and confidence in carin

Epidemiology of cryptosporidiosis among European AIDS patients.

OBJECTIVE: To study epidemiology and possible risk factors associated with the development of cryptosporidiosis among European patients with AIDS. METHODS: An inception cohort of 6548 patients with AIDS, consecutively diagnosed from 1979 to 1989, from 52 centres in 17 European countries was studied. Data on all AIDS defining events were collected retrospectively from patients' clinical records. Kaplan-Meier estimates, log rank tests and Cox proportional hazard models were used to examine for possible risk factors associated with cryptosporidiosis. RESULTS: Cryptosporidiosis was diagnosed in

Sociodemographic predictors and temporal trends of extrapulmonary tuberculosis as an AIDS-defining disease in Spain.

OBJECTIVE: To identify the factors associated with the presentation of extrapulmonary tuberculosis (EPTB) in AIDS diagnosis and to analyse its temporal trend. DESIGN: Analysis of AIDS surveillance data. METHODS: The study was based on AIDS cases diagnosed in Spain between 1988 and 1993, and reported up to September 1994. The proportion of patients presenting with EPTB at AIDS diagnosis was analysed by sociodemographic characteristics and year of diagnosis. RESULTS: Of the 22,445 AIDS cases diagnosed in Spain from 1988 to 1993, 6526 patients (29.1%; 95% confidence interval, 28.5-29.7) presen

Anal and penile condylomas in HIV-negative and HIV-positive men: clinical, histological and virological characteristics correlated to therapeutic outcome.

Clinical, histological and HPV DNA hybridization findings were analyzed for 73 homosexual and 38 heterosexual men attending for anal warts; therapy results were evaluated retrospectively for 76 of these patients. Concurrent anal and penile warts occurred most commonly in the heterosexual men (p < 0.001). While perianal warts were most common in heterosexuals (p < 0.05), intraanal warts were most common in homosexuals (p < 0.001). Altogether 23 homosexual men were HIV-infected; 13 HIV-positive men followed regarding therapeutic outcome were immunologically relatively intact with mean CD4 counts

Psychosexual problems in people with HIV infection: controlled study of gay men and men with haemophilia.

HIV infection can be associated with major psychological and social disturbance. Psychosexual problems would be expected to arise in the context of the infection, in view of the contribution that sexual behaviour can make to the acquisition and spread of HIV infection. Here the results of a study of the psychosexual consequences of HIV infection in gay men and men with haemophilia are presented, with the inclusion of data from control groups. Sixteen HIV-positive and 23 HIV-negative gay men, and 20 HIV-positive and 24 HIV-negative men with haemophilia with sexual partners were studied. HIV inf

Sexual behaviour of gay and bisexual men in eight European countries.

To compare the sexual behaviour and HIV risk reduction strategies of gay and bisexual men in Europe, a survey, disseminated via the gay press and gay associations, was conducted amongst gay and bisexual men in Austria, Switzerland, Germany, Denmark, France, Great Britain, Italy and the Netherlands during the autumn and winter of 1991. By end March of 1992, 12,347 completed questionnaires had been obtained. A preliminary analysis shows striking similarities in patterns of sexual behaviour of gay men in the 8 European countries but indicates that strategies of risk management concerning HIV and

Etiology of nonvesicular genital ulcers in Winnipeg.

Twenty-eight patients with recent onset of nonvesicular ulcerative genital lesions were studied prospectively to define the microbial etiology. Causative agents identified included herpes simplex virus (HSV), Chlamydia trachomatis, Haemophilus ducreyi, and Treponema pallidum. Three women and 25 men (seven of whom were homosexual) were studied. Ten patients did not have an etiology identified. Nine patients had syphilis, six had genital herpes, one had chancroid, one had granuloma inguinale, and one had a human bite injury. Six of seven homosexual men had syphilis, as compared with one of eight

Pathogen Disgust and Mate Preferences for Femininity in Women's Faces, Voices, and Personality Traits Among Chinese Lesbian and Bisexual Women.

Recent studies have demonstrated that pathogen-related factors are associated with preferences for exaggerated sex-typical characteristics in potential partners' faces and voices. However, most of these studies have primarily examined individual differences in heterosexual men, heterosexual women, and gay men. The present study investigates whether pathogen disgust, as measured by the Three-Domain of Disgust Scale, predicts individual differences in preferences for feminine characteristics in faces, voices, and personality traits of romantic partners among Chinese lesbian and bisexual women (n
