[Dental pain].

"The extreme variability of toothache is such that a good rule for any examiner is to consider all pains about the mouth and face to be of dental origin until provide otherwise" (Welden E. Bell). Dental caries is a disease of the calcified tissues of the teeth, characterised by demineralization of the inorganic portion and destruction of the organic substance of the tooth. The acids which affects the primary decalcifications is derived from the fermentation of starches and sugar logged in the retaining centers of the teeth. Dental caries can be classified in relation with the depth of the lesi

Foot care: focus on the elderly.

Problems in the foot develop as a result of the aging process or systemic disease. Common causes of pain and disability in the elderly are nail and skin problems, predominantly corns and calluses, along with circulatory and structural problems. Because patients with orthopaedic conditions may have preexisting foot problems, it is important for nurses to distinguish between minor foot problems that can easily be treated and more serious conditions that require referral to a specialist. This article discusses pathophysiology of the aging foot, a comprehensive foot assessment, common foot problem

Clinical dilemmas posed by patients with psychosomatic halitosis.

Patients affected by psychosomatic halitosis never wish to visit a psychologic specialist, because they cannot recognize their own psychosomatic condition. They also never doubt that they have offensive oral malodor. Other people's behavior, such as covering the nose or averting the face, is interpreted by these patients as an indication that their breath is offensive, and these behaviors or attitudes reinforce their belief that they have a strong oral malodor. To clarify whether the patient's perception of another individual's attitude is affected by his or her delusion, this article is focus

[Health status and living conditions in medical district of Sandnes during the period 1866-1900].

The municipality of Sandnes became a separate medical district in 1866. The district physicians' reports give us an insight into health and living conditions in this district in the last half of the 19th century. This paper is based on a study of all statistical data and physicians' comments concerning the Sandnes district for the 1866-1900 period. The incidence of various diseases is estimated. Mental illness seems to have been more frequent in Sandness than in the rest of the county. In the physicians' reports, this was explained by the dominance of religious Puritanism in the district. The

[Amalgam problems: recommendations on patient assessment and counseling].

Assessment of patients with alleged or supposed amalgam-associated problems is difficult. Various methods, classified as medical, dental and "traditional" are discussed. Medical methods comprise epicutaneous sensitivity testing and determination of the mercury concentration of various body fluids such as saliva, urine and blood. Patients with amalgam restorations regularly exhibit increased readings. However, such measurements are not very appropriate since there are no generally applicable threshold values. Dental examination focuses primarily on oral lichenoid lesions of the mucosa in the vi

Triclosan: applications and safety.

Triclosan (2,4,4'-trichloro-2'-hydroxydiphenyl ether) is a nonionic, broad spectrum, antimicrobial agent that, because of its favorable safety profile, has been incorporated into a variety of many personal care products, including deodorant soaps, underarm deodorants, shower gels, and health care personnel handwashes. Triclosan exhibits a moderate degree of substantivity to the skin, and, in many products, it imparts a remnant antimicrobial effect. Although direct contact with the material under exaggerated exposure conditions causes dermal irritation in laboratory animals, it has only rarely

[Legionella pneumophila in the hot water system of Danish hospitals and institutions. A questionnaire study and a random sample test].

Hitherto, nosocomial epidemic outbreaks of legionnaire's disease have not been reported in Denmark. Only sporadic cases have been described. A survey of 75 Danish hospitals concerning the dimensions and operation of the hot-water systems revealed that only 13% had a hot water tank temperature above the recommended 60 degrees C. Temperatures of coldest tap water and of returning water were above 50 degrees C in only 31% and 24% respectively. Twelve representative hospitals with 35 independent water systems were investigated for the presence of Legionella pneumophila (Lp). Lp was demonstrated in

Dental findings of patients who underwent liver transplantation.

Cyclosporin-induced gingival enlargement was presented. Enlargement was noted at several sites in the mouth of five patients taking Cyclosporin following liver transplantation. Papillary gingival enlargement was observed to affect the entire maxilla and mandible. Enlargement was particularly marked in the interdental papillary region on the labial side of the anterior teeth of the maxilla. Discoloration of the teeth was observed. Radiographic findings were unremarkable. Oral management of these patients is the prevention of dental caries and infection originating in the area of gingival enlarg

[The mentally retarded and the dental health services. Treatment needs and preventive strategies].

Mentally retarded individuals are, since 1984 a priority group for the Norwegian public dental health service. This group of patients may have many problems which affect oral health. Difficulties with feeding are common, as are various oral motor problems like rumination, bruxism, mouth breathing and drooling. The impaired oral function may cause retention of food, reduced self-cleaning from lips and tongue and long-term use of puree food. Obstipation is a common problem which is often treated with prunes, raisins or sweetened laxatives. In behaviour modification programs sweets are often used

Mechanical devices versus antimicrobial rinses in plaque and gingivitis reduction.

The effectiveness of mechanical oral cleaning and oral antimicrobial rinses was compared for gingivitis and bacterial plaque control in 158 subjects. Teeth were brushed ad lib throughout; four of the five groups used either an interdental cleaner, dental floss, an essential oil mouthwash or a cetypyridinium mouthwash. Gingival bleeding (EIBI), visual inflammation (VGI), and tooth plaque coverage were evaluated at zero, six and 12 weeks of product use. After six weeks, bleeding reduction was 42% greater for the interdental cleaner and 21% greater for the dental floss than for the control. All g

The safety of dental extractions in patients with hematologic malignancies.

Dental disorders have been recognized as major sources of infection in patients with hematologic malignancies (HM). Management of severe dental infections usually includes dental extractions (DE), but the safety of extractions in patients with HM who are at risk for bleeding, sepsis, and poor wound healing has not been well established. In conjunction with an aggressive program of dental care, 142 DE were performed in 26 patients with acute leukemia, myelodysplastic syndromes, and myeloproliferative disorders. Granulocytopenia (less than 1,000 granulocytes/microL) was present during or within

[Toxic shock syndrome in a 14-year-old girl].

The toxic shock syndrome in a 14-year old girl is described. This syndrome occurs most frequently - but not exclusively - in the teens and young women during the first days of menstruation, if tampons are used. The patients are acutely ill with high fever, diarrhea and/or vomiting, with a rash, with loss of consciousness, and signs of shock (occasionally shock lung syndrome and renal insufficiency). During convalescence desquamation of hands and feet shows up. Patients with much less severe symptoms have been seen. The primary lesion is a local infection (e.g. vaginitis) with staphylococcus au
