[Basics of emergency ultrasound].

Focused ultrasound is a key methodology of critical care medicine. By referencing few ultrasound differential diagnosis, it is possible to identifying in real-time the reason of the critical state of a patient. Therefore typical focused ultrasound protocols were developed. The well known Focused Assessment with Sonography for trauma (FAST) was incorporated into the Advanced Trauma Life Support (ATLS) for shock room. Focused echocardiographic evaluation in life support (FEEL) has been designed to be conformed with the universal Advanced Life Support (ALS) algorithm and to identify treatable con

Generating a minimal set of templates for the hippocampal region in MR neuroimages.

OBJECTIVES: We detail a procedure for generating a set of templates for the hippocampal region in magnetic resonance (MR) images, representative of the clinical conditions of the population under investigation. METHODS: The first step is robust standardization of the intensity scale of brain MR images, belonging to patients with different degrees of neuropathology (Alzheimer's disease). So similar tissues have similar intensities, even across images coming from different sources. After the automatic extraction of the hippocampal region from a large dataset of images, we address template gener

Patients with multiple sclerosis with structural venous abnormalities on MR imaging exhibit an abnormal flow distribution of the internal jugular veins.

PURPOSE: To evaluate extracranial venous structural and flow characteristics in patients with multiple sclerosis (MS). MATERIALS AND METHODS: Two hundred subjects with MS from two sites (n = 100 each) were evaluated with magnetic resonance (MR) imaging at 3 T. Contrast-enhanced time-resolved MR angiography and time-of-flight MR venography were used to assess vascular anatomy. Two-dimensional phase-contrast MR imaging was used to quantify blood flow. The MS population was divided into two groups: those with evident internal jugular vein (IJV) stenoses (stenotic group) and those without (nonste

Magnetic resonance imaging in transient global amnesia: lessons learned from 198 cases.

PURPOSE: Patients with transient global amnesia (TGA) present with a characteristic clinical syndrome although other differential diagnoses have to be considered. Diffusion-weighted imaging (DWI) represents a highly specific diagnostic tool in the context of TGA; however, standard clinical DWI often fails to detect the small characteristic hippocampal lesions. The diagnostic success of DWI sequences in TGA patients was analyzed with respect to slice thickness and time interval between symptom onset. METHODS: Magnetic resonance imaging (MRI) studies of 198 patients with clinically diagnosed TG

Network-specific effects of age and in-scanner subject motion: a resting-state fMRI study of 238 healthy adults.

Cognitive aging is accompanied by a range of structural and functional differences in the brain, even in the absence of neurodegenerative disease. Functional magnetic resonance imaging (fMRI) studies have reported increased bilateral activation during task performance in elderly participants compared to their younger counterparts, particularly in frontal regions. Alterations have also been observed in the functional architecture of the resting brain, suggesting that aging is associated with changes in the organization of the networks of the brain. However, previous studies have largely focused

Differences in cortico-striatal-cerebellar activation during working memory in syndromal and nonsyndromal children with prenatal alcohol exposure.

Although children with heavy prenatal alcohol exposure may exhibit the distinctive facial dysmorphology seen in full or partial fetal alcohol syndrome (FAS/PFAS), many lack that dysmorphology. This study examined the functional organization of working memory in the brain in three groups of children-those meeting diagnostic criteria for FAS or PFAS, heavily exposed (HE) nonsyndromal children, and healthy controls. A verbal n-back task (1-back and 0-back) was administered to 47 children (17 with FAS/PFAS, 13 HE, and 17 controls) during fMRI. Intra-group one-sample t-tests were used to identify a

Reduced left ventricular compacta thickness: a novel echocardiographic criterion for non-compaction cardiomyopathy.

BACKGROUND: Left ventricular noncompaction (LVNC) is characterized by a two-layered myocardium consisting of a noncompacted inner and a compacted outer layer. The ratio of the thicknesses of these two layers is a major diagnostic criterion, which is, however, often difficult to apply in clinical practice. METHODS: Transthoracic echocardiography was performed in 41 patients with LVNC, 41 patients with moderate or severe aortic valve stenosis (AVS), and 41 age-matched normal controls. The maximal systolic thicknesses of "noncompacta" and "compacta" were measured in standard short-axis views at

Intraoperative magnetic resonance imaging and neuronavigation for transoral approaches to upper cervical pathology.

BACKGROUND: The transoral transpharyngeal surgical approach is a recognized technique for management of ventral lesions at the clivus and upper cervical spine. This report examines the use of neuronavigation and intraoperative magnetic resonance imaging as surgical adjuncts for lesions in this region. METHODS: A retrospective review of patients undergoing transoral transpharyngeal surgery in the intraoperative magnetic resonance imaging (iMRI) unit from 1997 to present was performed. Preoperative demographic data, clinical history, physical examination, and imaging studies were reviewed. Data

Preoperative evaluation of neurovascular relationship by using contrast-enhanced and unenhanced 3D time-of-flight MR angiography in patients with trigeminal neuralgia.

BACKGROUND: Microvascular decompression is an etiological strategy for the therapy of trigeminal neuralgia (TN). Preoperative identification of neurovascular compression, therefore, could have an impact on the determination of appropriate treatment for TN. PURPOSE: To evaluate the value of contrast-enhanced and unenhanced three-dimensional (3D) time-of-flight (TOF) MR angiography in the visualization of neurovascular relationship in patients with TN. MATERIAL AND METHODS: Thirty-seven patients with unilateral TN underwent unenhanced and contrast-enhanced 3D TOF MR angiography with a 3.0-T MR

Differential impact of ApoE ε4 on cortical activation during famous face recognition in cognitively intact individuals and patients with amnestic mild cognitive impairment.

This study explores the neurofunctional correlates of the recognition of famous faces in patients with amnestic mild cognitive impairment (aMCI) and healthy controls depending on the genetic risk factor, Apolipoprotein E (ApoE) ε4. An event-related functional magnetic resonance imaging experiment was conducted while participants discriminated between famous and nonfamous faces. We compared the results of 32 healthy controls [17 ApoE ε4 carriers (E4+); 15 noncarriers (E4-)] with those of 30 patients with aMCI (16 E4+; 14 E4-). Despite comparable task performance, patients with aMCI, E4+ showed

Altered resting-state functional connectivity of thalamus in earthquake-induced posttraumatic stress disorder: a functional magnetic resonance imaging study.

BACKGROUND: Thalamic dysfunction has been found in patients with posttraumatic stress disorder (PTSD), suggesting that the thalamus may be implicated in the etiology of PTSD. However, no studies have explored the functional connectivity between the thalamus and other brain regions during resting-state. The objective of the present study was to investigate the resting-state functional connectivity of the thalamus in recent onset medication-naive PTSD sufferers who went through an earthquake in the Sichuan province of China. METHODS: Fifty-four participants with PTSD and seventy-two age and gen

Magnetic navigation in patients with coronary artery bypass grafting.

Magnetic navigation (MN) can precisely control a percutaneous coronary interventions (PCI) guidewire or a device in three-dimensional space within the body without requiring reshaping of the tip to access vessels or areas of the heart that are often challenging using conventional wires. In this article we review and report on the use of magnetic navigation system in secondary revascularisation of coronary arterial bypass grafts (CABG). MN was successfully used in the secondary revascularisation of failed conventional CABG cases. Retrograde PCI through a LIMA is not only feasible but the wires

Magnetic resonance evidence of cortical onset of multiple sclerosis.

BACKGROUND: Despite clinical symptoms and signs of central nervous system dysfunction, conventional magnetic resonance imaging (MRI) of the brain and spinal cord may appear normal in multiple sclerosis (MS) at clinical onset. OBJECTIVE: To demonstrate cortical pathology, namely the presence of cortical lesions (CLs) in patients with symptoms/signs suggestive of MS but having normal appearing white matter (WM) on MRI. METHODS: CLs were disclosed by double inversion recovery (DIR) MRI sequence. The final diagnosis of MS was achieved by the demonstration of the dissemination in space and time o

Abnormal white matter appearance on term FLAIR predicts neuro-developmental outcome at 6 years old following preterm birth.

UNLABELLED: Preterm infants are at significant risk of neuro-developmental disorders at school-age. MRI is a potentially useful screening tool of such disorders. Using FLAIR imaging in the preterm infants at term, here we demonstrate that abnormal low-intensity signal in the white matter predicts the neuro-developmental outcome at 6 years. STUDY DESIGN: Clinical factors associated with white matter appearance on MRI obtained at term were investigated in 210 preterm infants. RESULTS: Low-intensity signal on FLAIR imaging was commonly observed (69%) at <2 months corrected-age. Its incidence co

What is the best approach for the assessment of left atrial size? Comparison of various unidimensional and two-dimensional parameters with three-dimensional echocardiographically determined left atrial volume.

BACKGROUND: Left atrial (LA) size has prognostic importance in a variety of cardiac conditions. Diameters, area, or volume derived from 2-dimensional (2D) echocardiography (2DE) are commonly used to measure LA size, but involve numerous assumptions. This study examined the accuracy of these 2DE parameters compared to 3-dimensional echocardiographic LA volume (3DV). METHODS: LA diameters, area, and volume measured from 2DE were correlated with 3DV in 118 patients with dilated cardiomyopathy, hypertrophic cardiomyopathy, or without structural heart disease. RESULTS: Diameter (anterior-posterio

Quantitative echocardiographic evaluation of myocardial perfusion using interrupted contrast infusion technique: in vivo validation studies and feasibility in human beings.

BACKGROUND: We recently developed a new approach for contrast echocardiographic quantification of myocardial perfusion, based on brief interruptions of contrast infusion, which was designed to overcome the limitations of existing techniques. In this study, our technique was initially validated in a series of animal experiments designed to detect regional perfusion variations in vivo. Subsequently, clinical feasibility of perfusion measurements was tested. METHODS: Regional perfusion was measured transthoracically in 6 anesthetized pigs during baseline, partial left anterior descending coronar

99mTc-sestamibi uptake and histological malignancy grade in invasive breast carcinoma.

The aim of this study was to investigate whether there is a correlation between (99m)Tc-sestamibi uptake and histological malignancy grade in breast carcinoma. Such a correlation could, prior to surgery and histopathological analysis, facilitate selection of patients who need adjuvant therapy. Ninety-six patients with mammographically determined lesions and/or a palpable tumour suspected for malignancy underwent (99m)Tc-sestamibi scintimammography prior to surgery. The final diagnosis was determined by histopathological examination. Benign lesions, cancer in situ and tumours located medially i
