Many transplant centers are opposed to the use of living unrelated kidney donors and donors at added risk. To see how the public and nontransplant physicians view these issues, we surveyed samples of these groups in Rochester, NY. We found that the majority of respondents supports the use of unrelated donors and believes that donors should be allowed to accept added risk. 1,200 extracorporeal lithotripsies have been performed in 816 patients. 58% of the stones had a diameter of 3-10 mm, 41% measured between 11 and 20 mm and 1% were larger than 20 mm. The patients were all treated on an outpatient basis without either anesthesia or analgesia. The mean time spent at the lithotripsy center was 2 h: the mean treatment time was 46 min, i.e. 3,450 shocks at a frequency of 1.25/s. 530 patients were reviewed after 3 months. We report a case of lipomatosis of the renal hilus which presented with macroscopic painless haematuria. The appearance of the pyelocalyceal system on the intravenous urogram set the diagnosis of renal carcinoma, a diagnosis that both ultrasound and computed tomography did not refute. Repeat cadaver kidney transplantation using azathioprine immunosuppression carried a higher risk of graft loss than primary transplants. We analyzed the results of repeat cadaver kidney grafting with cyclosporine A immunosuppression. A total of 33 cyclosporine A-treated patients received the second kidney transplant at varying intervals after failure of the first transplant. Graft survival at 1 year was 66 per cent. It is reported on long-term courses in five patients with Wegener's granulomatosis. Under a treatment with glucocorticoids and immunosuppressive agents lasting several years in all cases a remission developed. A case of Wilms' tumour in a horseshoe kidney is presented. Until 1980, twenty-one cases were reported. Diagnosis may be difficult, especially as x-ray investigations, including IVP and renal angiograms, give the impression of a tumour arising outside the kidney. The distortion and dispersion of the renal pelvis and calyces usually seen with a Wilms' tumour are lacking. This is probably due to the special anatomic pattern of the horseshoe kidney, specifically the rotation of the renal pelvis around the long axis of the kidney.

In a combined retrospective and prospective study, 11 patients with adult polycystic kidney disease were found to have one or more cardiac or aortic lesions. Acute abdominal pain during pregnancy can be of urologic origin. Hydronephrosis of pregnancy can be complicated by symptomatic renal colic, pyelonephritis, and secondary renal abscess formation. In this report, rupture of a hydronephrotic kidney with retroperitoneal urinoma formation was treated by retroperitoneal drainage and internal ureteral until a term delivery was achieved. However, if severe renal hemorrhage accompanies renal rupture, surgical exploration of the kidney with partial or complete nephrectomy may be necessary.
