[Case report of germinoma with synchronous lesions in the thalamus and para lateral ventricle].

A 25-year-old man presented with headache and consciousness disturbance. MRI revealed well enhanced mass lesions using gadolinium at bilateral thalamus and right para lateral ventricle with mild perifocal edema. Obstructive hydrocephalus was observed. On neuroendoscopic observation, the tumor was not appeared on the surface of the ventricle, therefore biopsy of the lesion was not sufficient to make a definitive pathological diagnosis. Third ventriculostomy was successful and the patient's consciousness improved. One week later, a stereotactic biopsy for right para lateral ventricle lesion made

Comparison of combined hepatocellular and cholangiocarcinoma with hepatocellular carcinoma and intrahepatic cholangiocarcinoma.

PURPOSE: Combined hepatocellular and cholangiocarcinoma (HCC-CC) is a rare primary hepatic neoplasm (PHN) with features of both hepatocellular and biliary differentiation. We compared the outcome of hepatic resection in patients with HCC-CC, those with hepatocellular carcinoma (HCC), and those with cholangiocarcinoma (ICC). METHODS: Between November 1994 and December 2003, 952 patients underwent hepatic resection for a PHN. RESULTS: The incidence of HCC-CC was 3.5%. Hepatitis B surface antigen was positive in 51.2% of these patients and the HCV antibody was positive in 12.2%. Positive hepati

Multi-institutional analysis of early squamous cell carcinoma of the hypopharynx treated with radical radiotherapy.

PURPOSE: To analyze the outcomes of patients with early hypopharyngeal cancer treated with radical radiotherapy (RT). METHODS AND MATERIALS: Ten institutions combined the data from 115 patients with Stage I-II hypopharyngeal squamous cell carcinoma treated with definitive RT between 1990 and 2001. The median patient age was 67 years; 99 patients were men and 16 were women. Of the 115 patients, 39 had Stage I and 76 had Stage II disease. Conventional fractionation was used in 98 patients and twice-daily RT in 17 patients; chemotherapy was combined with RT in 57 patients. The median follow-up p

Familial multiple cutaneous and uterine leiomyomas associated with papillary renal cell cancer.

Multiple cutaneous and uterine leiomyomas is an autosomal dominant condition that results in benign smooth muscle tumours of the skin and, in females, uterine fibroids. This syndrome overlaps with hereditary leiomyomatosis and renal cell cancer syndrome in which affected individuals may develop the rare type II papillary renal cell cancer, in addition to skin leiomyomas. Recently, heterozygous mutations in the gene encoding fumarate hydratase have been found to underlie both conditions. Fumarate hydratase is an enzyme that catalyses the conversion of fumarate to malate in the Kreb's cycle and

Osteosarcoma with rhabdomyosarcomatous component or so-called malignant mesenchymoma of bone.

BACKGROUND: Primary malignant mesenchymoma of the bone is a rare neoplasm consisting of two or more unrelated malignant mesenchymal components. The literature reports fewer than 20 cases, most of which were composed of osteosarcoma and liposarcoma. OBSERVATION: We report an exceedingly rare case of primary malignant mesenchymoma of bone composed of rhabdomyosarcoma, osteosarcoma, and a minor chondrosarcoma component, arising in the right proximal humerus of a 15-year-old girl. The rhabdomyosarcomatous component was present in the initial biopsy and persisted in surgical specimen following che

Mucosa-associated lymphoid tissue lymphoma of the stomach: long term outcome after local treatment.

BACKGROUND: Although antibiotic therapy is emerging as effective initial treatment for patients with gastric lymphoma of mucosa-associated lymphoid tissue (MALT), there is a subset of patients for whom antibiotics are ineffective or inappropriate. Surgical resection can be curative, but total gastrectomy may be required for the eradication of all disease. To identify the optimal nonantibiotic therapy for early stage gastric MALT lymphoma, the authors retrospectively evaluated the Massachusetts General Hospital experience with gastric MALT lymphoma. METHODS: Disease patterns and treatment outc

Malignant tumors of the nasal cavity: review of a 60-case series.

Sixty cases of primary malignant tumor of the nasal cavity treated in our hospital between 1962 and 1993 were reviewed. Males were affected 2.8 times more frequently than females. The age at the first consultation ranged from 11 to 92 years, with a mean of 55.1 years. The peak distribution was seen in the 6th decade. Twenty-six cases were epithelial malignancies (squamous cell carcinoma 15; adenocarcinoma 3; adenoid cystic carcinoma 3; undifferentiated carcinoma 3; mucoepidermoid carcinoma 1; malignant mixed tumor 1), while 34 cases were non-epithelial malignancies (malignant melanoma 14; mali

Carcinoma in situ of the glottic larynx.

Carcinoma in situ (CIS) is part of the histopathologic spectrum of laryngeal disorders where invasive squamous cell carcinoma is the endpoint of cellular disarray. Few reports consider prognostic indicators that predict which lesions become invasive. Forty-one patients with CIS of the glottic larynx were analyzed for risk factors that would predict invasive cancer. Anterior commissure involvement by CIS resulted in 92% conversion to invasive squamous cell cancer compared to 17% of lesions limited to the mobile fold. Epidermal growth factor receptors were also analyzed and were found not to be

Apocrine adenocarcinoma of the bilateral axillae.

A case of apocrine adenocarcinoma arising in the bilateral axillae is reported. The patient was an 88-year-old Japanese male who complained of a mass lesion and pus-like discharge in the right axilla. Another mass was also noticed in the left axilla. No other neoplastic lesion was found in other sites of the body. The histologic appearances of the bilateral axillary tumors were almost identical. Both were adenocarcinoma with varying degrees of differentiation, composed of glands and nests of atypical epithelial cells with abundant eosinophilic cytoplasm. Some neoplastic cells exhibited cytopla

Kaposi's sarcoma developed in a patient with a thymoma in the setting of excess numbers of CD8-positive cells in the peripheral blood.

We describe an autopsy case of Kaposi's sarcoma in a patient with an increased count of suppressor T lymphocytes. This case was distinct from the acquired immunodeficiency syndrome. First, it did not have any detectable viral antibody relating to the acquired immunodeficiency syndrome. Second, the patient's blood sample showed that the ratio of CD8+ cell (suppressor/cytotoxic T lymphocyte) to all lymphocyte population was increased to 0.61, as compared with the normal value of 0.12 to 0.16; however, absolute number of CD4+ T cell (helper/inducer T lymphocyte) was not decreased (1800/mm3). [Clinical features and pathogenesis of Carney complex, a rare form of multiple endocrine neoplasia syndromes].

Carney-complex is the rarest and most recently described form of multiple endocrine neoplasia syndromes that is unique both from clinical and pathogenetic aspects. Clinical features include spotty skin pigmentation, cutaneous and cardiac myxomas, multiple endocrine abnormalities and schwannomas. The most characteristic endocrine abnormality is primary pigmented nodular adrenal hyperplasia that may result in clinically apparent Cushing's syndrome. Acromegaly, hyperprolactinaemia, tumours of the testis and ovaries have also been described. Approximately fifty percent of Carney-complex cases are

Clinical features and prognosis of patients with renal cancer and a second malignancy.

OBJECTIVE: To evaluate the epidemiologic aspects, the clinical features, and the prognosis of patients with renal cancer affected by a second malignancy. MATERIALS AND METHODS: Since 1983, at our institution, a database concerning all the patients who underwent surgery for renal neoplasia has been prospectively compiled. In the present study, we compared patients with renal cancer and a second primary malignancy, diagnosed before, at the same time, or after the renal cancer, to those affected only by a renal malignancy. RESULTS: Out of 1,673 patients with renal cancer, 285 (17%) were diagnos

Endobronchial malignancy as a manifestation of advanced ovarian cancer recurrence: a case report and literature review.

BACKGROUND: Endobronchial metastasis from primary ovarian cancer (OC) is very rare. To enhance our understanding of this disease, we present a case report and retrospective analysis of a patient with a bronchial tumor as a manifestation of primary OC recurrence. CASE DESCRIPTION: A 51-year-old woman presented with a history of intermittent cough and expectoration over 3 months by suffocating pneumonia for 3 weeks. Chest X-ray revealed multiple nodular masses at the right upper lobe, soft tissue thickening with bronchial invasion in the left upper lobe, enlargement of the right and left upper

A patient with an adrenal metastasis and periadrenal lymph node metastases of a laryngeal squamous cell carcinoma: a case report.

BACKGROUND: Adrenal tumors are quite common. If malignancy is suspected, the question arises whether the adrenal tumor is the primary or a secondary (= metastasis) malignancy. Surgery on adrenal metastases is generally considered to be of limited value if other distant metastases exist. We present a case of laryngeal squamous cell cancer that subsequently developed a large adrenal metastasis and multiple lymph node metastases treated successfully with surgery and immunotherapy. CASE PRESENTATION: The white male patient in his 60s was treated 20 months earlier with radiochemotherapy owing to l

Patterns and Clinicopathological Features of Histologically Proven Metastases in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common primary liver malignant neoplasm. Multiple risk factors have been identified for several decades for this overly aggressive tumor. HCC is an overly aggressive malignancy with frequent intrahepatic and extrahepatic metastasis. In our practice, we have observed that HCC has the propensity to metastasize to very unusual sites and can sometimes show variable patterns leading to diagnostic difficulty. In this study of 257 patients, we aim to discuss the unusual sites of HCC metastasis, the various patterns of metastasis, clinicopathological features

Novel germline variants of CDKN1B and CDKN2C identified during screening for familial primary hyperparathyroidism.

PURPOSE: CDKN1B mutations were established as a cause of multiple endocrine neoplasia 4 (MEN4) syndrome in patients with MEN1 phenotype without a mutation in the MEN1 gene. In addition, variants in other cyclin-dependent kinase inhibitors (CDKIs) were found in some MEN1-like cases without the MEN1 mutation. We aimed to describe novel germline mutations of these genes in patients with primary hyperparathyroidism (PHPT). METHODS: During genetic screening for familial hyperparathyroidism, three novel CDKIs germline mutations in three unrelated cases between January 2019 and November 2021 were id

Metastatic ductal carcinoma of the breast to the thyroid gland diagnosed with fine needle aspiration: A case report with emphasis on morphologic and immunophenotypic features.

Metastases to the thyroid are uncommon [<0.2% of thyroid fine needle aspirations (FNA)]. Of metastases to the thyroid, breast carcinoma is relatively common. The diagnosis of metastasis to the thyroid has important therapeutic and prognostic implications. To our knowledge, a morphologic and immunophenotypic comparison of metastatic ductal carcinoma of the breast and primary thyroid carcinomas has not been reported. Here, we report the case of a 37-year-old female with a history of metastatic ductal carcinoma of the breast (modified Bloom-Richardson grade 2; ER+, PgR+, HER2+) diagnosed 6 years

Evaluation of prognostic factors and outcomes in primary versus secondary myeloid sarcoma.

Myeloid sarcoma (MS) is a rare extramedullary manifestation of myeloid neoplasia that may present as an isolated disease without concurrent bone marrow involvement, primary disease or secondary to an antecedent myeloid neoplasm. The molecular landscape and prognostic determinants of these groups remain incompletely defined. This multi-institutional retrospective cohort study of 145 histologically confirmed MS cases from multiple academic centers incorporates clinical, cytogenetic, and next-generation sequencing data to characterize disease biology and outcomes. Patients with primary MS (56.6%

Diagnostic Workup of Primary Adrenal High-Grade Undifferentiated Pleomorphic Sarcoma Initially Presenting as Brain Metastasis.

Primary adrenal high-grade undifferentiated pleomorphic sarcoma (UPS) is an exceptionally rare malignant mesenchymal neoplasm, and presentation with brain metastasis as the initial manifestation is even more uncommon. This report highlights the diagnostic evaluation and multidisciplinary management of a patient presenting with an undifferentiated brain metastasis accompanied by an adrenal mass. A 75-year-old woman presented with speech disturbance, cognitive slowing, memory decline, gait instability, and right-sided weakness. Brain computed tomography and contrast-enhanced magnetic resonance i
