Management of chlamydia and gonorrhoea infections diagnosed in primary care using a centralised nurse-led telephone-based service: mixed methods evaluation.

BACKGROUND: Up to 18% of genital Chlamydia infections and 9% of Gonorrhoea infections in England are diagnosed in Primary Care. Evidence suggests that a substantial proportion of these cases are not managed appropriately in line with national guidelines. With the increase in sexually transmitted infections and the emergence of antimicrobial resistance, their timely and appropriate treatment is a priority. We investigated feasibility and acceptability of extending the National Chlamydia Screening Programme's centralised, nurse-led, telephone management (NLTM) as an option for management of all

Environmental impact of accelerated clinical care in a high-volume center.

BACKGROUND: "Fast-track" surgery, involving multimodal care, improves efficiency and short-term outcomes in patients undergoing bowel resection. The sustainability of the benefits and the "drag" effect on non-participating surgeons through changed nursing and resident practice is undetermined. METHODS: 297 consecutive elective colon resections (DRG149) within three study periods (pre-change, immediate post-change, long-term post-change) were retrospectively reviewed. Two surgeons began to "fast-track" their patients in 1999 independently from the other surgeons in the hospital. Surgeons were

Optimizing Organ Donation Outcomes in the ICU: An Evidence-Based Practice Quality Improvement Initiative.

Despite scientific advancements, demand for life-saving organs continues to outpace supply. Together, hospitals and organ procurement organizations facilitate deceased organ donations; however, a lack of structured inpatient processes and quality improvement mechanisms for potential donor management can lead to delayed referrals by organ procurement organizations, physiological instability, and suboptimal donation outcomes. Goal-directed strategies applied during the inpatient and preprocurement phases of care can significantly increase the number and quality of organs recovered. The aim of th

Best Evidence Summary of Brain Function Monitoring and Management in Patients Receiving Extracorporeal Membrane Oxygenation.

BACKGROUND: Neurological complications are among the most serious adverse events in patients receiving extracorporeal membrane oxygenation (ECMO). Brain injury during ECMO is associated with increased mortality, long-term neurological deficits and impaired quality of life. Current evidence regarding brain function monitoring and management in ECMO patients is scattered, varies in methodological quality and remains controversial in certain areas. Furthermore, standardised recommendations for monitoring and management across different ECMO configurations and patient populations have yet to be es

Optimizing care for children with difficult-to-treat and severe asthma through specialist paediatric asthma centres: expert practical experience and advice.

Severe asthma in children carries an unacceptable treatment burden, yet its rarity means clinical experience in treating it is limited, even among specialists. Practical guidance is needed to support clinical decision-making to optimize treatment for children with this condition. This modified Delphi convened 16 paediatric pulmonologists and allergologists from northern Europe, all experienced in treating children with severe asthma. Informed by interviews with stakeholders involved in the care of children with severe asthma (including paediatricians, nurses and carers), and an analysis of Euro

Nutritional support practices and opinions toward gastrostomy use in pediatric bone marrow transplant centers: A national survey.

OBJECTIVE: Previous surveys have shown deviations in nutritional practices from international guidelines during bone marrow transplant (BMT). Guidelines recommend enteral nutrition first-line and nasogastric tubes are the mainstay for its provision. Gastrostomies provide an alternative, yet their use is less common. This national survey investigated nutrition support practices in pediatric allogeneic BMT centers and compared clinicians' opinions on gastrostomy use. The aim of this study was to identify the national picture of nutritional support practices across pediatric allogeneic BMT center

A local response to implementing Saving Lives in a large acute Trust.

Healthcare-associated infections (HCAIs) are a high priority for health professionals. Under the Health and Social Care Act (2008), NHS organizations that fail to comply with the duties in the act could, in extreme cases, be liable for prosecution. Saving Lives (2007) is a national strategy to prevent and control HCAIs which incorporates seven high-impact interventions or care bundles. These relate to clinical procedures that must be performed correctly and in the same order for each patient, every time, to reduce the risk of a patient contracting an HCAI. Within a large Trust in the north eas
